How to Review Logbook Entries.

1. Read the surveillance description; determine what violation is this, like SAES,

SAEP, Sch Q or international Standard.

2. Find out the applicable SATIP and SAIC, point to be noted that if it is a

violation of Sch Q like no RFI or No QC inspector, so SATIP and SAIC shallnot be mentioned, only mention NA (not applicable).

3. In the Standard Category Select in the drop down menu the applicable

category

4. If it is a violation of SAES, SAEP or Sch Q at the same time, SAES shall be

mentioned in Standard1reference, then SAEP in Standard2 and then Sch Q

or international Standard in standard 3 references.

5. Indicate the specific paragraph no. or section for standard violation the Paranumber in related SAIC shall be verified for accuracy, according to latestedition of the standard.

6. Next step is that when this violation was found, during material receiving,during construction etc.

7. Is violation related to the design error or not.

8. For PID what inspection level is this, as per SATIP, Hold, witness,

surveillance or review?

9. Categorized the finding using the drop down menu, if it does not belong toany, just put it others. In the violation what is area of concern, like welding,piping, no RFI etc, point to be noted that for example if on the site weldingis going on without contractor QC, so area of concern is NO QC Inspector,not piping or welding.

10. It is important that review of LBE shall be in the same pattern, like if there isSATIP-L-350-01, so it shall be the same in all similar entries, because even

typing error like SATIP/L-350, 01 will not work, upon consolidation it cannot

be calculated, these all will show different entries only because of typing

error.

Some tips for the analysis of the recorded violation

1. Categorize the violation (what is the work involve?) and by knowing this we candetermine what particular SATIP is applicable.

2. If the recorded violation has 2 or more violation listed on it, then split it. Copy thesame violation and insert it below the original violation. Then analyze it, the sameas the original violation.

3. For consistency of our findings we must refer to the latest revision of SATIPs andSAICs. These can be accessed in the below link. 4. Point to be noted that if the LBE is a violation of SAES, SAEP or international

standard, it shall not be marked as Schedule Q, and applicable SATIP, SAIC,

SAES and paragraph shall be mentioned.

5. If the LBE is a violation of Schedule Q, so in the Colum of SATIP, SAIC only NA

shall be mentioned.

IMPORTANT:

Kindly follow some procedures to be observed as have been noticed during the review

as improvement.

1. Please note that all LBE have been analyzed previously, this is second time forconfirmation only, so some paragraphs have been changed to evaluate thereviewer understanding and it can be cross checked by the master file, so becareful to change/unchange anything.

2. Under columns in Section and Para. just put the number ex. 1.3 not sec. 1.3,this is to properly identify the nos. during sorting.

3. If the there are more paragraph or section involved in the violation, separate it,

put the other para. in column para. 2 or para. 3 or sec. in sec. 24. If the LBE violation contains more violation, put it in separate row under it.
