• Introduce participants to various evaluation forms/options– Effective Behavior Supports Survey – Team Process Evaluation– AzSET– Benchmarks of Quality– PBIS satisfaction Survey– Interview and Observation Form (from EBS)– Classroom Observation

Why the Interest in Measuring Your Program?

• PBIS is an evidence-based program

based on research and best practices

• Check you school’s fidelity to the program

• Use data to fine tune your program, justify spending, achieve the maximum results

• Completed by ALL staff

• Use results to design annual action plan

• Survey available in Spanish
