    OBJECTIVES

      Establish rules and regulations to have the better understanding the obligations and

    responsibilities with respect to the achievement of the objective of management system.

      To create awareness that HSE is an integral part of work and that production and safety are

    inseparable.

      Ensure to have better understanding to response in event of any emergencies.

      Ensure thorough investigations on work area accidents/ incidents and establish safeguards to

     prevent reoccurrence.

      Ensure the workplace is provide of the safe work place is to eliminate/reduce to the risk of

    death/injury to person/damage to property.

    1.0  SCOPE

    The scope of this HSE Manual is to describe in detail with reference and requirement of statutory

    regulations and guidelines of Occupational Health & safety management (OH & S), environmental

    Management system (EMS) about the control measures in all aspects in order ensure the HSE

    system maintained well within UNISCO Shop/Site

    2.0  PURPOSEThe Purpose of (HSE) manual is to have an established and well-controlled Health, Safety

    Environment management system within UNISCO Shop/Site

    3.0  HEALTH PLAN

    3.1 FIRST – AID FACILITY

      Competent employee shall be identified and first aid training shall be given to ensure sufficient

    first aiders are available.

      The name of the first aiders shall be exhibited in notice Boards.

     

    A Complete first aid kit shall made available in prime location and it shall identified & ensurefor easy access.

       Periodical check of availability/validity of medicine shall be verified. 

      All first aid cases shall be recorded in a first aid treatment log. [Format :UNISCO(F)/HSE-01]

    3.3 SANITATION

    3.3.1 Potable Water

      Adequate drinking water shall be made available.

      Chemical analysis test shall be done periodically once in a year for acceptable level.

      E-Coli test shall be done once in 6 months.

      Water analysis report shall be recorded in Format. UNISCO(F)/HSE-44

    3.3.2 Toilets  Sufficient toilet blocks shall be available according to the strength of employees

      Toilet blocks shall be cleaned frequently and maintained in a good sanitary condition,

    cleaning details shall be recorded. [Format :UNISCO(F)/HSE-33]

      Sewage water shall be disposed as on when required

     

    Availability of soap fluids for employees to wash their hands before taking food.

    3.4 HEAT STRESS

    Working in excessive atmosphere will cause heat stress. Heat stress is serious health issue and can lead

    to heat stroke, which could cause death. Everyone who works outdoors in hot weather need to follow the

    following precautions

      Drink plenty of water, even when not thirsty. Sip small amount often

     

    Wear light, loose fitting cotton cloth.

      Take regular break in the shade.

      Avoid alcohol or drinks with caffeine before and during work.

     

    Watch co-worker for signs of heat exhaustion or heat stroke.

      If you start feeling symptoms of headache, dizziness, etc stop the work immediately and inform

    to co-worker/ supervisor.

     

    If he notice co employee suffering with heat stroke, organize for immediate help from

    emergency medical service.

    The following are few symptoms of heat stroke

       No sweating

      Red or flushed, hot dry skin

      Rapid pulse

      Headache

    3.5 FOOD SAFETY

    3.5.1 General

    The hygienic level in the kitchen/canteen & during preparation shall be well maintained in all level at all

    the times. This includes following good practices in maintaining kitchen, kitchen equipment during food

     preparation.

      Protecting food from any kind of contamination

      Preventing any organs multiply which could result in premature decomposition of food

      Eleminating any harmful biological organisms(viruses, certain yeasts, spores, moulds etc)in

    food by good cooking practices.

      Kitchen/Canteen shall be inspected and recorded once in a month.

    3.5.2 Personal hygiene practices during handling food

    Kitchen/Canteen staff shall follow

     

    Avoid touching nose, mouth hair and skin during food preparation

      Do not smoke in kitchen and canteen.

      Do not cough or sneeze directly onto food

      Wash hands after coughing or sneezing

      Wash hands after blowing nose

    3.5.4 When to wash and sanitize of equipment & dishes

      Equipment shall be cleaned generally. The food contact surfaces of equipment & utensils shall

    cleaned thoroughly

     

    Before each use with a different type of raw food

    3.5.5 Hand washing lavatory

      Easily accessible

       Not to be used for any other purpose

      Must have liquid / powder soap (no multi use hand towels)

    3.5.6 General cleanliness

    Cleanliness should be ensured on the following by cleaning at regular intervals

       Non-food contact surfaces shall be cleaned at regular intervals.

      Stoves, grill and deep fryer surfaces

      Floor areas underneath food preparation areas; walk in freezers, refrigerators and around cooking

    4.0 SAFETY PLAN

    4.1 RESPONSIBILITIES, ACCOUNTABILITIES & AUTHORITIES

    4.1.1 General Manager-Projects

      Responsible and authority for the effective implementation of HSE manual & ensure the employees

    at all levels are clearly understood & following the same.

      Accountable to the Managing Director related to health, safety & environment and welfare of

    employees and who shall be affected by the company’s activities.

     

    Ensure all HSE issues been analyzed thoroughly & the required correction / corrective action & preventive action are implemented.

      To ensure responsible for the availability of the required infrastructure & resources at all levels.

      Shall chair safety management/committee meetings.

    4.1.2  Site / Production Manager

      Responsible for his personal safety and all employees under his authority, including sub contractors/

    visitor who shall be affected by the company’s activities. 

      Responsible for the implementation of HSE manual & ensure well maintained at all levels

      Accountable to the General Manager related to health, safety & environment for safe operations.

    4.1.3 Safety Engineer/Officer:

      Safety Engineer/Officer directly reporting to General Manager and co ordinate with Site/

    Production Managers, Site/ / Production Engineers, supervisors and foremans.

     

    Accountable to the General Manager for all health, safety & environment requirements.

      Responsible to establish the HSE manual at all levels.

      Ensure & Monitor the safe working conditions and safety practices.

      Ensure the availability of first aid kit & first aid trained personnel.

      Ensure the subcontractors, are complying with the UNISCO HSE requirements.

      Ensure personal protective equipment is worn by employees as appropriate.

      Conduct weekly safety inspection or co-ordination with the Site/Production Manager / &

    Site/Production Engineer.

      Safety Engineer / Officer shall conduct daily inspection to identify any safety deviation & initiate

    report deficiency and arrange immediate rectification.

      Conduct safety observation and Preparation of daily safety observation report. Format.

    4.1.4 Site /Production Engineer

      Ensure for the Implements the HSE manual and accountable to the Site /Production Manager

    for all health, safety & environment requirements

      Responsible for his personal safety and all employees under his authority

      Participate actively in HSE activities to establish the system is well maintained.

      Encourage employees to work in safe manner.

      Ensure availability of risk & impact assessment for the activity and implement the control

    measure as required.

      Maintain the safe work environment, tool, equipment and other resources.

      Conduct safety toolbox along with supervisors/foremans.

     Ensure the appropriate PPE is used for the activities monitored proper for the usage

    4.1.4 Maintenance Engineer (Electrical /Mechanical)

      Ensure for the Implements of HSE manual and accountable to the General Manager for all

    health, safety & environment requirements

    4.1.5 Supervisor / Foreman

      Accountable to the Site /Production Engineer for health, safety & environment with respect to

    safe practices and shall be responsible for his personal safety and all employees under his

    authority.

      Shall exercise close supervision over those employees who report or shall be assigned to him. He

    shall ensure that employees working under him perform the work safely. He must enforce safe

    rules and procedures.

      Must ensure that Safety regulations are clearly understood and that the means of escape routes,

    assembly points are thoroughly known to all his employees.

      Ensure that all tools, machinery, and equipment shall be regular and systematic in inspection,

    shall be in safe operating condition. 

      Co-operate with Safety engineer/ officer and follows instructions given. 

      Conduct toolbox talk to the employees before starting a new task.

    4.1.6 Employees

      Carry out assigned tasks and duties in a safe manner, in accordance with instructions, and to

    comply with safety rules/procedures, regulations and accountable to the Supervisor / Foreman

    for safe practices.

      Consult with foreman if any doubt in safe work practices.

    4.2  COMMUNICATION, PARTICIPATION, & CONSULTATION

      UNISCO’s Employee participation and communication shall ensure through weekly & daily

    toolbox meeting, HSE posters, notice board etc.

     

    Employees shall be encouraged to provide suggestions and report concerns regarding health, safety,and environment. 

    Safety Induction (Internal & External)

      All New UNISCO employees shall be given safety induction prior assign for work.   Format

    UNISCO[F]AD 01

      All sub contractors shall given safety induction on UNISCO HSE requirement prior assign for

    work. [ Format UNISCO(F)HSE-23 /A]

      Specific safety orientation training shall be given when such kind of job being executed.

      All visitors shall be given an induction on UNISCO HSE requirements,accompanied with UNISCO

    employee at all times during the visit [Format UNISCO(F)HSE-23/B]

      Visitors shall be providing with PPE (hardhat, safety shoes, safety glasses, earplug etc.)

    4.2.1  Tool Box Meeting

    4.2.4  Fire Extinguisher Training 

      Competent employee shall be identified and fire warden training shall be given to ensure sufficient

    firewatchers are available.

     

    Internal training shall be given to all employees in the awareness of handling fire-fighting

    equipments in the event of any fire. 

    4.2.5  Mock Drill

      The mock drill shall be planned [Format UNISCO (F) HSE-24] and conducted by safety

    engineer/ officer/ Site/production Managers.

      Mock drill shall be conducted in accordance with UNISCO’s emergency procedure clause 6.0

    for the effective awareness

      Mock drill shall be conducted once in three months on the following order to increase the

    awareness with the employees in event of any emergency.

      Corrective action will be implemented on defective actions by providing the employees with

    more training and information, as required.

     

    Emergency mock drill report shall record and maintained. [Format. UNISCO (F)HSE-15]

      Awareness for employees shall be given to contact ROP/ Civil defense in case of any major

    accident in work place during the mock drill.

      Investigate the immediate Engineer/Supervisor/Foreman/Victim for finding the root cause

    and initiate the corrective/preventive action & shall be recorded.

    [Format. UNISCO (F) HSE-07]

     

    The learning value of the accident investigation report shall be Implemented and cascadedto all employees.

    4.5  PERSONNEL PROTECTIVE EQUIPMENT (PPE)

    Hazards shall exist in workplace in many different type: sharp edges, falling objects, flying sparks,

    chemicals, noise and a innumerable of other potentially dangerous situations. Occupational Health &

    safety management (OH & S) shall requires that employers protect employees from workplace hazards

    that shall cause injury.

    Controlling a hazard at its source shall be best method to protect employees. Depending on the hazard

    or workplace conditions, UNISCO urge the use of Personal protective equipment to manage or eliminate

    hazards.

    Personal protective equipment, commonly referred to as "PPE", is equipment worn to minimize

    exposure to a variety of hazards.

    The PPE availability shall be verified & recorded monthly basis. [Format UNISCO (F) HSE-13]

    4.5.1 Safety Helmets and Safety shoe

    Safety – Helmets helmets shall do the following:

    4.5.2 Clothing and Gloves

      Protective clothing and gloves shall wear to protect the body and limbs from hazards of,

    chemical liquids and vapors, heat, electricity, radiation

     

    Welders shall wear leather aprons, hand sleeves, leg guard, leather gloves etc

      Cotton gloves with leather or suitable palms shall worn for general protection purpose,

    (handling materials)

      Rubber gloves should wear whilst working on live, low- voltage electrical switchgears.

    4.5.3 Eye Protection

      Welders shall use welding shield with appropriate filters (shades) during welding.

      Goggles, safety glasses, face shield shall be worn during the chipping, grinding, & gas

    cutting.

    4.5.4 Hearing Protection

      Hearing protectors is a very personal one and depends on a number of factors including level of

    noise, comfort, suitability of the hearing protectors for both the worker and the environment and

    compatibility.

      Public environment noise level shall not exceed 70 dB.

       Noise monitoring record shall be maintained and recorded .Once in a month

    4.5.5 Respiratory protection

      Disposable dust mask shall be used in the presence of airborne dust or particles

      Shall wear half face piece respirator during spray painting and full-face respirator during manual

     blasting.

    Work Procedures:

    4.6  Manual Handling:

     

    During lifting back shall be straight, load shall be close to the body, the leg muscle shall used for

    lifting, no twist in upper body while lifting.

      Object shall be inspected for any sharp edges, nails any other thing that may cause injury.

      Gloves shall be used during handling rough sharp material.

      During lifting objects, weight above 50 pounds power equipment shall be used.

      During two or more people carrying any heavy object prearrange signaling shall be ensured

    while lifting lowering/ dropping and releasing the load.

    4.8 Lifting operation & Equipment

      All lifting gears shall be checked for the fitness and certified by the third party inspector and it

    shall be color coded accordingly.

      All materials handling equipment cranes, forklift shall be checked for the fitness and certified

     by the third party inspector

      The weight shall be assessed properly in order to engage suitable lifting equipment.

      The pipes, beams, plates shall be stacked properly with wooden sleepers/ steel poles

       Loads shall be controlled by tag lines.

      Only qualified riggers shall be engaged in any rigging activity.

      The rigger shall be present during the loading, unloading, and any other handling by lifting

    equipment.

       No alteration, repairs modification shall be done in any lifting gears and equipment.

       Never cross/ walk/ work under or near the suspended load.

      Lifting tools & tackles shall be visually inspected on a regular basis, and third party

    inspection shall be done once in 6 months and lifting equipments once in a year.

    4.9 Hand Tools:

      Defective tools shall not be used.

      Do not carry sharp hand tools in clothing.

      While using the hammer eye protection shall be worn.

      While using the screwdriver, the correct size, type of screwdriver shall be used.

    4.10 Power Tools:

      Ensure all the electrical tools are acceptable safe condition and shall have necessary safety

    devices.

      Ensure all the metal parts, which do not carry currents (handle, housing etc), shall be grounded

    when connected to a power source.

      Ensure the equipment is disconnected while making adjustments or repairs.

      Do not use electrical tools in areas when there is a possibility of flammable dust, gases, or

    vapor.

    4.11 Compressed gas cylinders:

      Ensure cylinders are not drop, expose to extremes temperature.

     Ensure the valve cap always in place except when in use.

      Ensure the valve or valve cap is not used to lift the cylinder.

      Compressed gas cylinders shall not roll in horizontal position.

       Never tamper with safety relief devices of cylinder valves. Always use proper fittings on gauges

    suitable for the particular gas being used.

      Contents shall be properly marked on all cylinders.

      Compressed gas cylinder (whether full or empty) shall be transferred in upright position secure

    them with chains at cradle.

     

    Ensure oxygen cylinders are separated from combustible materials & acetylene cylinders by

    minimum distance of 3 meters during storage.

      Flammable gas cylinders shall be stored in proper designated and safeguarded area.

      Cylinders shall be never placed where they might become part of an electrical circuit.

    4.13  SCAFFOLDING

    Safety engineer /Officer shall inspect all erected scaffolds. Safety Engineer/ Officer will issue Scaff

    Tag permit for the fitness.

    4.13.1  Mobile Scaffold Towers 

    Mobile towers constructed from scaffold tube and fittings or cup lock must only erected by a

    competent person who has sufficient knowledge and experience of erection techniques and the load

    factor, which is likely to be imposed on the mobile tower when put to proper use,

      A ladder shall be provided to gain access to the working platform and shall be secured.

      The recommended maximum height of a mobile tower is 9.6 meters although this shall

     be increased to 12 meters if tied to a structure when is in use.

      Working platform shall be secure, fully covered using boards at least 38mm thick.Supported with transoms at not less than 1.5-meter intervals.

      A main guardrail shall be minimum of 910mm if the tower is above 2 meters height

      Sufficient numbers of intermediate guardrails or suitable alternatives shall be done; thegap does not exceed 470 mm.

      Toe boards shall be at least 150mm high, fixed with proper clamps.

      Wheels shall be secured to the standards of the scaffold tower and fitted with safety

    locks.

      Mobile scaffolds shall be used on ground, which is firm and level.

    4.14 Safety Rules When Using Ladders

    Certain safety rules shall be observed while using ladders:-

     

    Ensure that footwear is not greasy, muddy, or slippery and has a good grip on the rungs.

      Face the ladder and hold on with both hands.

      Use platforms or attached trays for tools while working on a ladder. Alternatively, a bucket

    lashed to a rung can be used as a tool holder.

      Hold on with at least one hand. If the use of both hands is necessary to do a job, a safety

    harness shall be worn.

     

    Overreaching or leaning sideways may cause overbalancing and fall from the ladder.

      Do not climb higher than the third rung from the top.

      Portable ladders shall not be used in high winds.

    4.15 FALL PROTECTION (WORK AT HEIGHT)

    Prior to allowing employee to work at heights, the Supervisor/ foreman shall obtain WAH permit

    [Format. UNISCO (F) HSE-26] & confirm their fitness to work and that they do not suffer from

    vertigo.

    To avoid fall accidents during working at height, sufficient fall protection systems shall be in

     place. The following types of prevention system shall be used wherever applicable.

    1. 

    Safety harness

    2. 

    Scaffolding platform

    3.  Working platform.(MEWP)

    Safety harness:

      All employees working above 2 meters shall use safety harness, secure properly with lifeline

    or to firm support.

      All employees working above 6 meters shall wear safety harness with lanyard attached with

    shock absorber, employees working below 6 meters shall not use lanyard attached with shock

    absorber.

    4.16.2  Gas –  Cutting

      Ensure hoses are checked thoroughly for any defects & greasy or oil soaked hoses shall be

    replaced 

     

    Regulators and torch shall be check thoroughly and cleaned before starting the job.

      Flash back arrestors shall be equipped in oxygen as well as acetylene.

      Only flint lighters shall light the torches.

      Cylinders shall never be allowed to come in contact with live electrical wire and shall be

    kept away from sparks, flames, or slag.

      Cylinder valves shall be closed when work is completed or stopped.

      Cylinders shall be always being kept in upright position secured with chain.

     Hoses shall be clear of pathways, stairs, and ladders.

       No cutting shall be performed on used drums, barrels, tanks or other containers that were used

    for holding flammable/ combustible materials until they have been cleaned.

      Cleaning must remove all flammable materials present, as well as any substances such as grease,

      Spillage shall be mopped immediately.

      Paint containers shall not be stored more than 3 no’s in column.

     

    Temperature shall be maintained below the flash point.

      Fire point shall be equipped in the storage area.

      Lid opened paint containers shall not be stacked in storage room.

     

    Ensure all electrical devices are periodically checked in storage room.

      Legal permit shall be obtained for storage of chemical and records shall be maintained in

    format UNISCO (F) HSE-40.

    4.18  PAINTING

      All painting processes/type/method shall be in accordance with UNISCO’s approved

     procedures where operation involving paints which produces flammable vapors

      Care shall be taken when mixing and transferring paints.

      All spraying areas shall be kept free from the accumulation of deposits of combustible

    residues as practicable, by cleaning.

      Scrapers, or other tools used for cleaning purposes shall be of non-sparking material.

      “NO SMOKING” signs shall be posted at all spraying areas and paint storage rooms.

      Appropriate PPE shall be used

    4.18.1  BLASTING

      Blasting equipment and ventilation system shall be checked prior to use.

      Blaster hood shall be ventilated by clean cool air through regulator / filter

    [Format No UNISCO (F) HSE-19]

      Dead man control shall be fitted at the nozzle end of the abrasive blasting hose

      The rated working pressure of a blast hose shall not exceeded

      Blasting operation shall not take place with a coiled hose

      Hose whip arrestor or hose coupling safety locks or both shall be fitted to hoses

      Pinholes shall not tape in the blast hose. The hole will enlarge quickly and will cause a blow-out

      Blasting hoses shall be coil and stored away from water, oil, and chemicals to prevent rotting.

    4.20  HYDRO TEST & PNEUMATIC TEST

    All hydro tests shall carry out in accordance with UNISCO’s approved procedures. [Internal

    work permits Format. UNISCO (F) HSE-21]

     

    The surrounding areas of the vessel/tank under pressure test shall be barricaded.

      Sign board showing “vessel/tank under pressure test’ shall be placed near the location.  

      The pressure gauges shall be verified for proper functioning and the Pressure gauge pressure

    range shall be ensured to withstand the test pressure.

      Verify that all supply lines and hose connections are secure with retaining devices before and

    during the test.

      Adequate lighting shall be available throughout testing operations.

    4.21  MACHINERY AND EQUIPMENTS

      All machinery and equipments in the shop / site shall be free from defects, with necessary guards

    in place and to safe for its intended use.

      All mobile and overhead cranes used inside the premises shall undergo regular maintenance and

     periodical load test certification.

       All rotating parts shall be guarded, including pedestrian fans covered with nets.

      Safety devices shall not manipulated.

      All the electrical cords/ leads are color coded respectively by the maintenance department.

      Cable routing shall not be routed in pathways, ladders, and staircase.

    4.23  CONFINED SPACE

    Confined space activities shall be undertaken under a permit to work system

    [Format. UNISCO (F) HSE-27].

    Following points shall be considered

      Presence of inflammable liquids / gases.

      Presence of toxic liquids / gases.

      Limited access (making entry, exit, and movement of materials difficult).

     

    Enclosure (outsiders do not know often what is going on inside confined space). 

    Limited working area 

      Oxygen concentration shall be not less than 23.5%.

      Continuous gas monitoring shall be carried out when found necessary.

     

    Multi gas detector meter shall be calibrated and details shall be recorded in format UNISCO (F)

    HSE-43. 

      A stand-by worker trained in emergency response and the use of & equipped with self-contained

     breathing apparatus/ escape set shall be stationed outside the confined space and must maintain

     permanent contact by predetermined means. Escape set shall be calibrated and details shall be

    recorded in format UNISCO (F) HSE-43.

       Not to cook, operate heater or other unauthorized electrical appliances.

     

    Don’t fire in portacabins like Agarbatti, Dip, and Dhup for Puja.

       Not to keep the mobile / other charger ON while leaving the room.

      Air conditioner and lights to switched off before leaving the room.

     

    Alcohol (Wines, drink, beer, etc) not allowed in camp premises.

      Visitors / Guest not allowed entering / staying in camp after 9 pm.

      Don’t spit in the camp other than wash area.

    6.0  EMERGENCY RESPONSE PLAN :- 

    The entire employee shall be trained to react in the event of an emergency scenario like fire or

    any other accident. Safety engineer/ officer & Site/ production Managers shall ensure the

    implementation of emergency procedures and responsible to prepare the employee to actappropriately in the event of any emergency. The emergency procedures shall be strictly

    adhered.

    Call ROP /Civil defense emergencies no.9999 and describe the details of accident in case of

    major accident.

    Definitions and Abbreviations:

    Emergency: Any event that causes injury and or damage to person, property, or the

    environment and requires an emergency response.

    Evacuation:  An emergency that requires the evacuation of facility employee to designated

    Initial response:  Make alarm, upon hearing the emergency alarm, the emergency response team will

    lead the entire employee to the emergency exit to nearest assembly point.

    Evacuation decision:  Based on the severity of the emergency situation, the emergency team leader

    decides if an evacuation of all employees is required, if he deems an evacuation

    necessary he instructs to sound the alarm (siren) and all employee in accord with

    the evacuation action plan presented later in this document. Periodic emergency

    evacuation drill will be conducted to evaluate the response time.

    Emergency team duties: Upon notification of an emergency, the emergency response team is responsible for

     performing the following duties.

    The emergency response team members must stop all the work and proceed directly

    to the emergency assembly point. If any of the members is operating crane, forklift

    or other movable machinery, he has to stop it assemble in nearest assembly point.

    Subsequent to the emergency gathering at assembly point, the emergency response

    team members must follow the instruction issued by the team leader/ coordinator

    and carry out the assigned duties.

    decide what action to be taken search and rescue of missing employee.

    Ensure all employees are aware that they may not leave the assembly points

    unless otherwise instructed in emergency or evacuations.

    First aid team actions: During evacuation, first aiders shall be stationed at the assembly point. They

    shall be responsible for ensuring all medical/ first aid kit is available and

    attending the victim and providing necessary first aid.

    If any victim needs to transfer for emergency medical attention, an

    emergency vehicle shall be used for this purpose. According to the severity

    of the injury, the Site/Production Manager (emergency response team

    coordinator) will contact ambulance for further treatment.

    electric wire by means of non-conductor such as wood, stick or nylon rope

      In case of mental shock, but conscious, make him comfortable by talking to him

      Remove the casualty to the nearest clinic/hospital as fast as possible with the available

    transport.

      An employee shall be identified for their best effort on implementing and following safety

    help of Safety Engineer/Officer to evaluate the same.

      Three candidates would be selected and among them, the best safety person would be chosen. A

    cash prize of OMR 10.00 shall be awarded with certificate as an appreciation.
