From the MD’s desk - Capt. Amit Jain – Managing Director

Safety is the topmost priority in all our business activities. The company views safe operation as a

key social obligation and is committed in forging ahead to become the world leader in safe

operation.

In order to prevent accidents, we are promoting the concept of "STOP and THINK MORE" as a

habit for our seafarers. That means to actually "STOP" when we feel anxious or think something is

wrong, imagine what are the hazards of the working process, and then identify what is the correct

and safe path forward. This is intended to help develop a solid, safety culture among all employees

at sea and on land which forms a vital part in corporate culture based on a common set of values

that places safe operation as the highest priority.

As humans, we tend to make negative assumptions, however things may not be as bad as they

look. Maintaining a positive attitude is about looking for solutions from different angles instead of

focusing on the things that didn’t go well. Instead, break the problem down, see the opportunities it

presents and start working on improving the situations. Seek support and involve your colleagues

in taking a decision in case of crisis. Sometimes it can be hard to accept help because we feel that

we need to sort it out ourselves. This doesn’t really work well for the team, which means problems

don’t get solved which can cause more serious issues and distress.

Asking for help is strength, not a weakness and accepting help from your team & colleagues helps

you to solve the problems and strengthens your team work and bonding.

We all go through tough times in our lives and it can feel overwhelming. We may not have a choice

on the crisis occurring, but we do have choice on how we react and the choices we make. Through

crises we learn and grow “In the worst of times we can find our best selves”.

Remember to STOP & THINK MORE before taking an instant decision and jumping to conclusions.

BBS – Best Spirit Cards

Best Spirit Card No.1:- While approaching Pasir Gudang, Malaysia, crew were preparing the pilot ladder on the port side. They were about to rig the ladder when AB stopped them as he found that the pilot ladder rope was having plenty of worn out strands. After that, they used the new spare ladder.

Best Spirit Card No.2:- While cargo discharging at Amsterdam from 14P & 14S along with 4P, after a while 1st Officer observed that ullage shown in level master for COT 14S was reducing, implying that cargo level was increasing in the tank. Immediately discharging was stopped from other two tanks (4P & 14P) and 1st officer rushed to investigate the matter. Cargo pump stack, pipeline line up and cargo flow from manifold was checked and no abnormality was found. Manual ullages were checked using MMC and it was observed that the ullages were increasing. Level master was malfunctioning. Subsequently these tanks were monitored manually

The BBS system has replaced the previous Crew

Commendation Award system. We deeply value the

good reports submitted by the entire fleet (including

TMS Vessels). These reports are an important

motivation tool to foster the sense of pride into what

we are doing and creating a sense of belongingness

to the organization. Amongst the various reports

submitted in the 3rd quarter of 2019, the following

three SPIRIT cards have been selected and will enter

the final round of 12 SPIRIT cards which will be

reviewed after the 4th Qtr of 2019 for final selection

of the 3 best SPIRIT cards for the yearly awards of

$1000 each.

Spirit Cards contd.

Best Spirit Card No.3:- While receiving NAOH

(25M3) from the shore side through PVC hose, 1E

observed that one of the shore worker was about

to carry out welding work near the PVC hose.

Shore worker was informed immediately not to

carry out any welding in the vicinity of the PVC

hose while vessel was receiving NAOH.

Safety and Compliance reporting (Extract from M-04 App 22)

In order to improve the compliance and safety cultures on board Company vessels, Company has set up an independent e-mail address for compliance and/or safety reporting ([email protected]) E-mails sent to this email address will be monitored by key personnel of the Company for addressing the relevant issues in timely and effective manner. You can send emails to this email address in case you have concerns regarding:

• Compliance issues on board;

• Safety issues on board;

• Any safety ideas you wish to share. For sending email to this email address, you can either use your personal email address (if you wish to identify yourself) or send anonymously (confidential – no one will know your ID).

Additionally, we have added a compliance reporting tab in our website. Ship staff as well as shore staff can utilize this option for reporting any matters pertaining to compliance, Safety, Health, MARPOL, Harassment etc. or any other non-compliances. The reports generated will be anonymous.

All staffs to be regularly motivated & briefed that they are fully empowered to use their “STOP WORK”

authority in order to prevent any unsafe & unlawful acts and remain fully obliged to inform the company of

any such cases.

Crew Injuries / Fatalities

There was no LTIF reported during the third quarter of 2019. TRCF for 3rd quarter was 0.48 and was same as 0.48 for the previous

quarter. For the year 2018, fleet LTIF was 0.38 and TRCF was 1.01. The target for the year 2019 is an average of previous 3 yrs.

Hence LTIF of 0.4 & TRCF of 1.2 is targeted.

Injury Cases: -

Case-1: Vessel was alongside at Berth N-1 Mailiao loading Caustic Soda, MOT was moving in athwartship direction due to easy rolling of vessel. Duty watchkeeper observed MOT was very close to the jetty Bollard motor & about to touch same. To avoid damage to MOT and shore Bollard motor, MOT was shifted by Duty AB, OS and Bosun. While shifting the MOT, Bosun’s left hand little finger got slightly injured. He was given first-aid on board and was later sent to shore doctor. After visiting doctor, he was declared fit for duty. Bosun returned onboard and resumed his normal duties.

Lesson learned - To be extremely careful while handling gangways. Assess the risks posed and take proper action to mitigate same. Duty officer to supervise any repositioning of the gangway.

Case-2: AB, sustained minor burns during tank cleaning operation. AB got exposed to hot water splash from the sea water valve flange as the heavily rusted nuts & bolt sheared off while changing over the tanks during butterworthing

Lesson learned – Ensure rusted nuts & bolts are timely replaced. Check the tools & equipment condition prior to commencement of any job. Crew to be very careful while handling hot water / steam lines or any hot surface.

Case-3: AB removed the vent drain cap for cargo tank 2P (previous cargo: Acetic Acid) under vent post area for vent line cleaning. After opening the cap, the cap dropped from his hand, and then one or two droplets of cargo in the cap splashed to his right eye. He was given immediate eye wash. He was not wearing safety googles, instead he just turned his head to leeside to protect his eye. He was sent ashore to doctor for a confirmatory checkup and he was declared fit for duty.

Lesson learned – Crew must wear proper PPE and be careful while working. Fellow crew members must watch each other and should use their STOP WORK authority to prevent any unsafe acts. Usage of proper PPE pertaining to the job must be briefed to the crew during the toolbox meeting prior to commencement of job.

# (LTIF = Lost time Injuries Frequency as per OCIMF. This in general terms means number of injuries for every 1million

exposure hours in the fleet. LTI includes injuries resulting in lost time, fatalities, severe injuries resulting in ability to work ashore/onboard.

TRCF = Total Recordable Case Frequency as per OCIMF. This is also number of such injuries per 1million exposure hours in

the fleet. It includes LTIF injuries as above and RWC- Restricted Work Day Case & MTC - Medical Treatment Case)

Learning from Incidents

During this quarter we had 02 unfortunate incidents, brief details are as below: -

Case-1: One of our managed vessels was involved in an incident where pilot fell overboard while boarding our vessel at

Mumbai A2 Inner anchorage. Whilst boarding the vessel, the pilot fell overboard from the pilot ladder. Attempts were made

by the pilot boat’s crew and ship’s staff to rescue the pilot but was futile. Search and rescue operations were initiated by port

authorities and coastguard but with no success. After about 2 days, body of the pilot was seen floating in water and recovered

near Pirpau jetty located 7 miles away from the vessel’s location. At the time of boarding, the Pilot was reportedly not wearing

any life jacket and helmet. He was observed wearing short sleeve with casual shoes and there was a small bag fastened to his

waist. There was no fault on part of the our vessel & crew.

Lessons Learned:

1. Vessels to ensure all personnel (Including pilots) are wearing proper PPE while embarking / disembarking

from vessel. Staff should NOT hesitate to exercise their STOP WORK authority to prevent any unsafe acts.

2. Vessels must exercise extreme caution while embarking /disembarking pilot and strictly follow the agreed

Procedure. During choppy weather, vessel must closely liaise with the pilot / pilot boat for ensuring safe

embarkation & disembarkation.

3. Ship staff to offer life vest to all personnel (Including pilots) while embarking /disembarking by boats in case they do

not bring one.

4. Reinforce BBS program to include contractor safety as well and improve intervention culture.

5. Pilot boarding arrangements(Including pilot ladders & securing arrangements) should be maintained in good condition

at all times.

6. Intervene and stop pilot / contractors in case they try to violate rules. Company shall always support

Master/crew even if there is ant retaliation for stopping an unsafe act / condition.

Case-2: One of our managed vessels collided with a fishing boat, off Taiwan. Vessel was laden with various chemicals and was

bound for CJK (CHANG JIANG KOU) pilot station. Whilst en route, about 20 NM from Taichung Port, Taiwan, vessel collided with

a fishing vessel on her starboard side forward of the manifold. No injury or pollution resulted due to the incident. Own vessel

did not suffer any structural damages except minor paint scratches on her hull. Fishing vessel suffered minor contact damage

on her bow area. Investigation revealed that AIS data was relied upon for collision avoidance. Duty officer was not keeping

proper look-out & the duty AB was carrying out cleaning of bridge while close quarter situation was developing.

Lessons Learned: 1. Safety of navigation is always of paramount importance. OOW shall ensure that proper look-out is always maintained.

There is no other important task than keeping a proper lookout during navigation watch for the OOW and watchkeeping

crew.

2. OOW shall carefully monitor all traffic in the vicinity and be situationally aware to take early & effective action to

prevent close quarter situation. Action taken to avoid collision should be in line with COLREGS & in compliance with

any other local regulations prevailing in that area.

3. OOW should bear in mind that fishing vessels prevailing in any area at times may attempt to cross vessel’s bow, may

not have AIS fitted and are generally not responsive to VHF calls or sound & light signals.

4. Vessel should take proper action to pass well clear of the fishing boats and their nets.

5. Call master early instead of waiting till the last minute. Do not hesitate to call master when OOW is in doubt whether to

call master or not.

6. OOW should carry out defensive navigation.

Near Miss Analysis

Near Miss:

Near miss reporting in the 3rd quarter has been satisfactory. Annual target is at-least 24 near misses per vessel per year. Crew are regularly briefed & motivated for identifying & reporting near misses without any fear or favor. Only few vessels were required to be reminded for near miss reporting. To further enhance the near miss reporting system and learning process, near misses categorized as HPC (High Potential Consequence) are analyzed and key learnings are sent to all vessels every month.

The following pie chart indicates the analysis of the near miss in this quarter. It may be noted that Others (In Near Miss Type chart) includes the exceptions to rest hours.

Enhanced MARPOL Compliance

Continuing our commitment towards MARPOL Compliance, company has decided to continue with the voluntary initiative of carrying out detailed MARPOL compliance audits on board all our vessels. Experienced team of Chief Engineers from our fleet shall continue to conduct another round of focused MARPOL audits.

Key learnings and action items shall be discussed during shore management meetings. These shall be disseminated to all our vessels for improvement.

Global suphur Cap – 01-Jan-2020 - The Global Sulphur Cap deadline is aproaching, and we have provided all

our vessels with an Ship Implementation Plan (SIP). The ship implementation plan for 2020 covers various

items relevant for the specific ship, including, as appropriate, but not limited to:

• Risk assessment and mitigation plan (impact of new fuels);

• Fuel oil system modifications and tank cleaning (if needed);

• Fuel oil capacity and segregation capability;

• Procurement of compliant fuel;

• Fuel oil changeover plan (conventional residual fuel oils to 0.50% sulphur compliant fuel oil);

• Documentation and reporting.

Scrubber installation updates: -

➢ GINGA LYNX was successfully fitted with hybrid type scrubber during her dry docking period.

➢ Installation of Scrubber unit is in progress on our vessel GINGA CARACAL.

➢ One Document Transmittal Note (DTN) - 02/2019 was issued in this quarter on 13-Sep-2019 which

had various revisions to M-01, M-02, M-03, M-04, M-05, M-06, M-07, & M-09. The revisions were

mainly to align our procedures with the new CDI VIR which is effective from 02-Sep-2019.

➢ 17-Sep-2019 - DTN 02-2019 (Updates were sent based on feedback received from masters).

Amendments to QHSEMS:

Regulatory Updates

➢ MARPOL Annex VI (Chapters 1 - 3) - SOx

control: The global sulphur limit will reduce to 0.50% on 1 January 2020. All new and existing ships will need to comply with the new global sulphur limit from 1 January 2020.

➢ MARPOL & IBC - New tankers

constructed after 1 Jan 2016 require approved instrument with applicable intact and damage stability requirements. Existing tankers – 1st survey after Jan 2016 but in any case, before 1 Jan 2021.

➢ SOLAS Chapter II-2/10.5 for the fire protection

of domestic boilers. Ships fitted with boilers that are protected by a water-based local application fire-extinguishing system, are no longer required to provide the approved foam-type extinguisher of 135l capacity. Applicable from 1 January 2020 for all ships.

➢ Amendments to MARPOL Annex VI - Fuel oil

used or carried for use on board a ship shall not exceed a sulphur limit of 0,50% m/m. The IAPP certificate supplement is amended accordingly. This carriage prohibition will enter into force for all ships on 01st Mar 2020.

➢ Amendments to the Ballast Water Management Convention (13-Oct-2019) - The amendments relate to the implementation of the treaty, including the schedule for ships to comply with the requirement to meet the so-called D-2 standard (amendments to section B). Other amendments (to sections A and D) make mandatory the Code for approval of ballast water management systems, which was also be adopted at the session. Further amendments relate to section E on survey and certification.

➢ Amendments to the International Code for Fire Safety Systems (FSS Code), clarifying the distribution of crew in public spaces for the calculation of stairways width. The amendments are expected to enter into force on 1 January 2020.

➢ Amendments to the model forms of the

Certificates of Fitness, clarifying the requirement for an approved loading and stability manual/booklet to be supplied to the ship, under the International Code for the Construction and Equipment of Ships Carrying Dangerous Chemicals in Bulk (IBC Code), Code for the Construction and Equipment of Ships Carrying

Senior Officers Seminar at Singapore

➢ Senior Officers Seminar conducted in Singapore from 10th to 13th July 2019, key items as per below: -

➢ Recent and Historical issues (Marine, Technical & safety) were discussed.

➢ Training on FRAMO system operation, maintenance & handling of common issues was carried out.

➢ Training on Ballast Water treatment system operation, maintenance and handling of common issues

was carried out at ALPHA LAVAL company.

Detoxification – Rejuvenate your body

Detoxification means cleansing the blood. This is done by removing impurities from the blood in the liver, where

toxins are processed for elimination. The body also eliminates toxins through the kidneys, intestines, lungs,

lymphatic system, and skin. However, when these systems are compromised, impurities aren't properly filtered,

and the body is adversely affected.

By removing and eliminating toxins, then feeding your body with healthy nutrients, detoxifying can help protect

you from various diseases and renew your ability to maintain optimum health through a range of methods

including yoga, meditation and more.

Human body has a sophisticated way of eliminating toxins that involves the liver, kidneys, digestive system, skin,

and lungs. Only when these organs are healthy, can they effectively eliminate unwanted substances. So, while

detox diets don’t do anything that your body can’t naturally do on its own, you can optimize your body’s natural
