Veterinary surgeries are a potential source of infection and cross-infection both toanimals and to humans, whether working in the practice or visiting as clients. Veterinary professionals have a responsibility to minimise the risk of transmission of infectiousagents and good infection control should be placed at the centre of clinical practice.

Ideally, all practices should have a written infection control plan (as a Standard Operating Procedure) which should be reviewed regularly, audited, and updated at least annually1. Cleaning and disinfection protocols are a necessary part of the plan and staff training and education are essential to ensure effectiveness*.

This guide is primarily aimed at helping you assess the risk of infection in various procedures and areas within your practice. It is based on normal day to day operations, but confirmation of the presence of an infectious disease will immediately change the risk profile since ‘high risk’ pathogens can potentially colonise areas normally considered as ‘low risk’.

Practices need to review risk factors for the spread of resistant micro-organisms. Animals at particularly highrisk of hospital acquired infections (HAIs) include those with indwelling medical devices (e.g. catheters4 andimplants), severe illness, a history of prolonged antibiotic usage, large wounds (traumatic or surgical) or patients who have had prolonged hospitalization. There is no debating the fact that the single most important way of reducing HAIs and zoonotic infections is attention to effective hand washing. Some veterinary laboratories can offer hygiene screening services which will monitor the effectiveness of the cleaning regimens in your practice.

A number of HAIs can affect human hospitals, of which meticillin resistant Staphylococcus aureus (MRSA) is one of the most high profile. Similarly, in veterinary practices, MRSA can have very serious implications if post operative infection occurs. Fortunately, MRSA is a surface organism which is easily destroyed by the correct use of antibacterial disinfectants.

Conversely canine parvovirus, a highly infectious viral disease, is very resistant to disinfectants and once it has occurred in a premises the virus can remain viable for up to a year5. As the virus is shed in high titres in faeces, close attention should be paid to reduce contamination in the environment. Also, the virus is resistant to many commonly used disinfectants. Thankfully, Vetaclean® Parvo Liquid and Tablets from the Animalcare range of veterinary exclusive disinfectant cleaners have proven efficacy against this extremely hardy and very contagious virus.

Product selection has traditionally been made on the basis of “what we’ve always used” or “because it smells clean”. This has led to a mixture of products being used throughout a practice, many duplicating or indeed negating the action of others. Once again, the intention of this guide is to simplify matters by recommending the minimum frequency of disinfection and the right product for the right job.

KEY POINTS

It is important to recognise that hand hygiene is the single most effective measure in preventing the spreadof infection. Hands must be cleansed before and after handling each patient, and the use of gloves should not reduce the frequency or diligence of hand disinfection, particularly as surveys have shown that veterinary staff appear to be at a greater risk of MRSA colonization6.

Vetasept® Chlorhexidine is a safe, effective biguanide disinfectant with broad spectrum antibacterial and antiviral activity. It is highly active against Gram+ve organisms, including MRSA, and at a concentration of 2-4% it is also active against yeasts, particularly Malassezia pachydermatis8. It is not inactivated by organic matter, and is persistent in action.

If rigorous hand disinfection is important to prevent the transmission of infection between sick and healthy patients, as well as preventing hospital borne post-operative infections, then rigorous pre-operative disinfection is critical. Surgeons in both human and animal medicine have come to rely on two compounds, Chlorhexidine gluconate and Povidone-Iodine, to ensure pre-operative asepsis. Both are highly effective in conjunction with good scrubbing techniques and the use of one or the other is a matter of personal choice. ROUTINE HAND WASHINGROUTINE HAND WASHING

When preparing surgical fields it is important to not only achieve asepsis prior to surgery but to maintainskin asepsis during the procedure by using an antiseptic with a spirit base to target micro-organisms in the sebum rich layers of the skin#. Animalcare supply three tinctures which provide rapid asepsis, are quickdrying and have residual activity*. Vetasept® Skin Scrub Blue and Vetasept® Povidone-Iodine Alcoholic Tincturetemporarily stain skin blue and yellow respectively on application. This ensures the entire area has been treatedand confirms to all theatre staff that the skin prep procedure has been completed minimising risk of breachesin aseptic procedures. If you prefer a non-staining tincture use Vetasept® Chlorhexidine Clear RTU Spray.

The following procedure is recommended for preparation of surgical sites:

1. Clip the hair from the surgical field using a surgical clipper blade in the prep room. 2. Remove all hair debris from the surgical field using a hand-held vacuum cleaner (specific for this purpose).For grossly dirty skin, proceed to step 3. For visually clean skin proceed to step 4.3. Wash grossly dirty skin using a 1:8 dilution of either Vetasept® Chlorhexidine or Vetasept® Povidone-Iodine surgical scrub. Thoroughly dry the skin with sterile towels or swabs.4. Apply preferred Vetasept® tincture (see above) wearing gloves if using staining products. It is essential that the tincture is applied to dry skin to maximise skin penetration and avoid dilution and ‘run-off’.5. Apply the solution to the edges of the operating site first using swabs (or cotton wool). Another swab should be used to apply the tincture to the centre of the site. Repeat this application process two or three times. 6. Apply sterile drapes once the skin is dry. It is important to allow the product to work for at least three minutes prior to surgery.

DISINFECTION OF OPERATION SITESDISINFECTION OF OPERATION SITES

A longer and more thorough handwash which includes forearms and elbows is required before surgical and invasive procedures. Maintain contact with antiseptic for 3-5 minutes. Scrub brushes are recommended aroundthe nails. Raise hands throughout to allow water to run off at the elbows. Use sterile towels to dry the skin.

PRE-OPERATIVE SKIN SCRUB PROCEDUREPRE-OPERATIVE SKIN SCRUB PROCEDURE

Follow these steps to ensure the soap is applied to all areas of your hands:

1. Wet hands under running water, apply antiseptic soap and rub palms together to ensure complete coverage.2. Spread the lather over the backs of the hands.3. Make sure the soap gets in between the fingers.4. Grip the fingers on each hand, paying particular attention to the thumbs.5. Press fingertips into the palm of each hand.6. Rinse and dry thoroughly with a paper towel and dispose of in a pedal bin. Do not use cloth towels.

Notes: # Traditional techniques using aqueous skin scrubs followed by pure surgical spirit should be avoided for several reasons; aqueous skin scrubs cannot target micro-organisms in the sebum (oil) rich layers of the skin, surgical spirit does not have any residual antiseptic activity once evaporated and its application rinses away any previously applied antiseptics such that recolonisation by airborne bacteria starts to occur almost immediately. * Shown to maintain asepsis for up to 60 minutes - studies available on request.

Vetaclean® (lemon or floral)Description: A liquid multi-use disinfectant with excellent detergent properties.

Composition: A quaternary ammonium compound (quat). Biocide is benzalkonium chloride.

Uses: • Surface disinfection of floors, work surfaces, door handles, walls etc.• Sanitising of utensils.

Effectiveness: • Broad spectrum bactericidal. • Some fungicidal and virucidal activity.

• pH 7-8.• Free rinsing.

• Calibrated reservoir at 20ml intervals for easy, accurate dilution.• General disinfectant / excellent cleaner for breaking down organic matter.

Areas such as the waiting room, where risks appear to be low, can be deceptive. Patients that have not yet been examined but are suspected carriers (despite being asymptomatic) should be accorded a high level of hygiene attention. Coughing, urinating, defecating or the patients shaking themselves can add to contamination of theenvironment. Invasive procedures on an infectious patient can release dangerous pathogens into the atmosphere.Routine, rigorous hard surface cleaning and disinfection is a critical contributor to practice hygiene, eliminating or reducing infectious agents to a safe level. Product selection should depend not only on efficacy againstthe suspected pathogen, but also on detergent activity. This activity is necessary to clean away organic matter which may compromise antimicrobial efficacy. The Vetaclean® hygiene range offers a choice of disinfectantswith excellent cleaning properties which have been optimised for the sanitising of hard surfaces.

In normal circumstances some hard surfaces are more likely to acquire microbial contamination - and thereby assist in the spread of infection - than others, depending on the degree of contact with humans and animals. For example, the surface of a consulting room table is far more likely to be colonised by bacteria than the walls of the room. Selection of a product offering both detergent and disinfectant properties will also ensure good physical cleaning.

The following pages seek to identify the areas in the various rooms of the practice which are high risk and, therefore, in need of frequent disinfection, and those of lower risk which can be disinfected less frequently.

• General traffic from clients waiting and collecting diets, drugs, accessories etc.

• Animals should be controlled or placed in pet carriers.

• Surfaces which come into contact with animals - such as front of reception desk being brushed, licked or scented by dogs - should be wiped liberally or disinfected with Vetasept® Chlorhexidine Clear RTU Spray.

• Clutter on the floor acts to discourage proper cleaning and can act as a reservoir of bacteria.

• Wounds should be properly dressed and injuries covered.

• Accidents should be cleaned up immediately using wipes or absorbent granules/powder such as Rapid Response. Use disposable towels/spatulas for solids and place in hazardous waste bag.

• Potential high risk patients should be segregated, reducing close contact.
