Waiver of Premium (WP) IIssue A~s 15 -55.

This rider when added to a Value Term policy will waivethe premiums for the duration of the disability shouldthe policyowner become disabled before age 60.

Misstatement of Age or Sex

The premiums for this policy are determined based on the Life Insured's insurance agewhich is the age attained at his/her birthday nearest the policy date, as shown in theSchedule of Benefits and Premiums.

We require satisfactory proof of the date of birth and sex of the Life Insured. If theinsurance age or sex has been misstated, any amount payable by us will be adjusted inaccordance with the correct age, sex and premium. I

Premiums and Renewal Provision .

All premiums after the first will be payable on premium due dates determined from thepolicy date and the payment frequency.

Until the Plan Termination Date, this policy, if in force, may be renewed for successive tenyear periods on payment of the applicable premium. The annual premium for each tenyear period subsequent to the first ten year period is equal to the sum insured shown inthe Schedule of Benefits and Premiums multiplied by the appropriate premium rate shownin Appendix A, divided by 1000. The appropriate premium rate depends on the suminsured and the Life Insured's sex and smoker status at the effective date of this coverageand insurance age at the renewal date.

i 8 i\ You may pay premiums annually, semi-annually or, monthly by an automatic payment~ system. The payment frequency may be changed only on an anniversary of the policy

date. A change may be made by paying the premium for a new payment period on orbefore the policy anniversary or within the grace period. When a new payment frequencyis selected, premiums will become payable on that basis.

If we introduce lower premium rates for this plan of insurance after this policy is in force,we will automatically reduce the premium rates on this policy, at the following renewaldate to the new rate based on the Life Insured's class of risk at issue and the insurance,age of the Life Insured at the renewal date.

The renewal premium rates will never exceed those shown in the Schedule of BenefitsI ' and Premiums. .

Disclosure ~ r,

You and each Life Insured must disclose in the application for this policy or in anyapplication for reinstatement, every fact which is material to the contract. Subject to theIncontestability provision, failure to do so or misrepresenting any material fact in theapplication will entitle us to cancel the coverage. Suicide and Self-Inflicted Injuries

If the Ufe Insured while sane or insane commits suicide or dies from self-inflicted injuries: .18)

a) within 2 years from the policy date, the death benefit is t1e return of premiums paidwithout interest,

b) within 2 years of the effective date of reinstatement and after the policy has been in) fo~ce longer than 2 years, the death benefit is the premiums paid since the, reinstatement,

Reinstatement I '.

This policy may be reinstated at any time within two years fro~ the date of lapse, on thefollowing conditions:

a) receipt by us of evidence satisfactory to us of the good health and insurability of thejUfe Insured determined on the same basis as when the coverage was issued;

b) .1~receiPt by us of payment of all overdue premiums with interest

c) issuance by us of a certificate of reinstatement.

1Grace Period .

.A grace period of 30 days will be allowed for the payment of e ch premium after the first.I.If the Life Insured dies during the grace period. this policy will be considered to be in force ,

as if the premium had been paid but any unpaid premium will be deducted from the deathbenefit.Lapse .This policy will lapse and our liability will cease at the expiry of the grace period of anunpaid premium.

Non-Smoker/Smoker Class .

If the Life Insured whose insurance age at the policy date is 20 r greater is stated to be anon-smoker, premiums will be based on non-smoker rates. If a misstatement is maderegarding the Life Insured's smoking status, we have the right to cancel coverage on thatperson as described in the Disclosure and Incontestability provisions.

Face Enhancement Rider

fIf Face Enhancement Rider coverage is included in this poli y it will be shown in theSchedule of Benefits and Premiums.

Under this rider. the sum insured will increase automatically on each second policyanniversary during the first ten policy years by 10% of the original amount of insuranceissued.

.Total Disability Premium Waiver ider

Definitions

"Total disability" and "totally disabled" mean a state of incapacity resulting from bodilyinjury or disease which during its first 24 months prevents the Life Insured from engagingin the Life Insured's occupation or employment. After 24 months of anyone continuousperiod of disability, the state of incapacity must prevent the Life Insured from engaging inany occupation or employment for which the Life Insured is qualified or may reasonablybecome qualified by reason of training, education or experience.

"Conversion Period" referred to in this rider is the conversion period stated in the policy.

Benefits I

If the Life Insured becomes totally disabled while this rider is in force and prior to thepolicy anniversary nearest the Life Insured's 60th birthday, and has been continuouslydisabled for at least 6 months, we will on receiving satisfactory proof of total disabilitygrant the following benefits subject to the terms and conditions of this rider:

Waiver of Premiums

We will waive payment of any premium falling due after the commencement and during theuninterrupted continuance of total disability. No premium which became due more than 12months prior to our receiving written notice of claim will be waived. The premium waivedwill be the premium according to the frequency of payment in effect when total disabilitycommenced and the premium frequency may not be changed during a period of disabilitywithout our consent.

.; If this policy and rider are in force at the end of the conversion period and the Life Insured; is then totally disabled and has been continuously disabled for at least 6 months, this

policy will be converted automatically to a new policy of an individual life non-participatingplan in accordance with the conversion option in the policy. The new policy will containthe total disability premium waiver rider which is included by us in individual life policieswith the same policy date as this policy. The premium for the new policy will be waivedduring the uninterrupted continuance of total disability.

Notice and Proof of Claim .'

.Satisfactory proof of age of the Life Insured must be received by us before a premium is .waived. .I-Proof of Continuance of Total Disability

We may at any time request satisfactory proof of the continuance of total disability. Onfailure to furnish proof, the Life Insured will be considered to have ce?sed to be totallydisabled immediately prior to the date of request and there will be no further benefits underthis rider. At any time, we may request a medical examination by a medical examinerappointed by us.

Recurrence of Total Disability : I-

If the Life Insured ceases to be totally disabled and again becomes totally disabled; aclaim will be allowed subject to the same terms and conditions as if there had been noprior disability.

Lapse While Totally Disabled

JI-: If this policy lapses while the Life Insured is totally disabled an

(a) proof of claim is received within 12 months after the date of lapse showing that as aresult of total disability the benefit under this rider would have been granted if thispolicy had remained in force, and

(b) we receive overdue premiums which would not have been waived under this rider with I.interest at a rate determined by us, then

this policy will be reinstated and the benefit will be the same as if this policy had notlapsed. Settlement Options.A payee is the person entitled to receive any death benefit payable by us under this policy.

During the lifetime of the Life Insured, you may elect to have the proceeds of the policypaid in accordance with one of the following options rather than in a single sum. .

If no election is made, the beneficiary may do so after the death of the Life Insured.

1. Trust Deposit -Proceeds may be left on deposit with us for no longer than the lifetimeof the payee with interest payable annually at a rate determined by us. The interestrate may change from time to time but will not be less than 3.5 percent per annum,compounded annually. When this option is chosen any amount on deposit at thedeath of the payee will be paid in one sum to the executors or administrators of the

, payee's estate.

2.1 I nstallment Payments -Proceeds may be applied to purchase any type of annuity that, is available from us at the date of the Life Insured's death. The rates for the annuity

will b.e rates then in use by us. In all cases, satisfactory proof of age and sex must besubmitted to us before annuity payments commence.

Conversion Option :'~

.This policy, if in force, may be converted within the conversion period to any eligible plan

issued by us at the conversion date.

If the conversion option is exercised between the second and tenth policy anniversaries,we will reduce the first annual premium of the new policy by 20%.

The sum insured of the new policy may not exceed the sum insured of this policy at theconversion date or be less than the minimum for the plan selected.

The conversion will be made without evidence of insurability. The new policy may includethe same riders as this policy. However, if the new plan is other than a life plan requiringpayment of premiums until at least age 85, the continuation of a total disability rider will besubject to satisfactory evidence of insurability.

The conversion will be subject to our receiving a satisfactory written request and thispolicy for cancellation. Any indebtedness to us under this policy will remain a chargeagainst the new policy.

The policy date of the new policy will be the conversion date. The premium for the new

policy will be determined from:

(1) the premium rates in use by us on the conversion date for the Life Insured's insurance

age.

(2) the plan and amount of the new policy. ,-

(3) the class of risk applicable to the Life Insured under this policy at the conversion date.

The new policy will contain the same provisions and be subject to the same

.endorsements regularly included in policies issued by us on the conversion date.

.) Children's Protection Ride

Definition .

"Child" includes a natural born child, step-child or legally adopted child of the LifeInsured. To be insured under this rider the child:

a} must be at least 15 days of age and less than an insurance age of 18.

b} must be named in the application for the rider, or become a "child" to the Life Insuredafter the date of application for the rider and while payment of premiums is required onthe policy to which this rider is attached. --

c} will be subject to approval of its good health and insurability if th-e child is other than an~tural born child of the Life Insured.

The amount of insurance on the life of each child will be $1,000 for each unit of this riderand will be payable on receiving satisfactory proof of the child's death on or before thepolicy an_niversary nearest the child's 25th birthday.

Waiver of Premiums .

8) On the death of the Life Insured we will waive all future premiums for this rider.

Conversion Options

Within 31 days prior to the policy anniversary nearest either the 21 st, 23rd or 25th birthday,or within 30 days following marriage prior to the policy anniversary nearest the 25thbirthday, any child insured under this rider may apply for a policy on his/her life withoutevidence of insurability. The total insurance coverage for any child under the new policy,any other policies issued under this option and any continuing coverage under this rider,must not exceed $10,000 for each unit of this rider. The new policy will be issued subjectto proof of age, to payment of the first premium and may be on any eligible plan ofinsurance issued by us at that time for the desired amount of insurance. The effectivedate and the policy date will be the option date selected and the premium will bedetermined from the premium rates in use by us at the option date for the then insuranceage.

IAutomatic Termination -

This rider will terminate automatically on the policy anniversary nearest the 25th birthdayof the last child insured under this rider to attain age 25 or on the termination of this policyor the endorsement of this policy as paid-up insurance. I-IncontestabilityStatements made in each of the application for this policy and for this rider, if later, or forreinstatement, other than fraudulent statements, will be considered incontestable after a.1\ period of 2 years from the effective date of this policy, the rider or reinstatement1

Suicide and Self-Inflicted Injuries

18If the life insured while sane or insane dies by suicide or self-inflicted injury within 2 years )from the policy date of this policy, or the effective date of this rider. if later. or in the caseof reinstatement, from the date of the certificate of reinstatement, this rider will terminateand all rider premiums paid will be forfeited to us. If any child insured under this rider,while sane or insane dies by suicide or self-inflicted injury within these periods, theinsurance under this rider on the life of the child will be invalid and no payment will bemade by us.

Misstatement of Age or Sex and Reinstatement Provisions I -

The misstatement of age or sex and reinstatement provisions of this policy will also applyto all .Of the persons insured under this rider.

we will pay, in addition to the sum insured, the amount of this rider as stated in theSchedule of Benefits and Premiums or in any application for change which requested theaddition or amendment of this rider and is part of the policy.

The benefit under this rider will be doubled if death was caused by an accident while theLife Insured was a fare-paying passenger and not an operator or a crew member on apublic conveyance regularly used for passenger service.

.(1) disease including heart attack and stroke, medical or surgical procedure,

(2) suicide or self-inflicted injuries while sane or insane,

(3) committing or attempting to commit a criminal offense, including being in care orcontrol of a motor vehicle while having a blood alcohol reading in excess of 80milligrams of alcohol per 100 millilitres of blood,

(4) civil disorder or war whether declared or not, .I-

(5) taking or inhaling any drug, poisonous substance or gas whether voluntary or not,

(6) travel in an aircraft if:

-the Life Insured was operating, learning to operate, or serving as a member of thecrew,

-the flight was for any training, testing, parachuting or experimental purpose,

-the flight was in any aircraft operated by, or under the direction of any military(land, sea or air) authorities.

Descent from any aircraft in flight is considered to be part of the flight.

Automatic Termination

This !rider will automatically terminate on the earliest of: ~

(1) the termination of the policy, d

Accidental Death and Dismemberment Rider.This benefit provides coverage only on a person who is shown as insured for this benefit

in the Schedule of Benefits and Premiums or in any application for change whichrequested the addition or amendment of this rider and is part of the policy.

Benefit

If the Life Insured suffers injury which results in a loss listed below, and when we receivesatisfactory proof that

(1) the loss resulted directly and independently of all other causes from bodily injurycaused solely by external, violent and accidental means, and

(2) the loss occurred while this policy and rider were in force and within 365 days of thedate of injury, and

(3) the injury was received before the policy anniversary nearest the Life Insured's 65thbirthday,

we will pay, in addition to any other benefits payable under this policy, the amountspecified for the loss to the individual specified, as follows:

(1) the amount of the rider to the beneficiary for loss of life.

(2) the amount of the rider to the owner for the loss of: '"c;,(i) both hands or both feet or the sight of both eyes, ,J

(ii:1 one hand and one foot,

(iii) either hand or foot and the sight of one eye.

8) (3) one-half of the amount of the rider to the owner for the loss of either hand or foot, or: the loss of sight of one eye.

(4) one-quarter of the amount of the rider to the owner for the loss of the thumb or indexfinger.

Loss will mean, with regards to hands, feet, thumb or index finger, total and irrevocableloss of use; with regard to eyes, entire and irrevocable loss of sight.
