Modern medicine uses a lot of resources and generates a lot of waste and pollution in the process. This pollution, in turn, has a negative effect on human health, and wasteful systems can increase costs and block access to care for those who need it. In order to design more efficient, more sustainable ophthalmic care, systems engineers and medical providers have been analyzing the environmental footprint of ophthalmic services. This masterclass will discuss this ongoing research in ophthalmology and the approaches various organizations have taken to incorporate sustainable practices in their services. Our overarching goal is for attendees to take home some of the principles shared through this masterclass, so that they, too, can reduce their footprint and improve costs and access to eye care. ObjectiveThis masterclass teaches attendees the basic concepts of Triple Bottom Line (TBL) sustainability and how TBL approaches have been applied to eye care to reduce costs, minimize environmental footprint, and increase access to eye health services worldwide.

Summary of ContentThe first third of this instructional course is an introduction to the Triple Bottom Line (TBL) of sustainability and how sustainability concepts have been applied to healthcare in general and ophthalmology, specifically. These introductions will be led by Ingeborg Steinbach of the Centre for Sustainable Healthcare in Oxford, UK. The core component of this instructional course is the Masterclass on Environmental Sustainability for Ophthalmology, led by Dr. Cassandra Thiel, the leading researcher on environmental sustainability for ophthalmology and an Assistant Professor at New York University. This Masterclass will focus on current research on the environmental footprint of cataract surgery at facilities in the USA, UK, and India, and ongoing projects to develop sustainability monitoring tools for ophthalmic services worldwide. Attendees will learn tools and techniques used to quantify environmental emissions, and hear about the results of these studies, which have been used to inform new sustainability interventions for ophthalmology. Through this masterclass, attendees will be able to see their services through a systems lens and be able to effectively apply TBL approaches to reduce the footprint of their services. The final third of this instructional course is devoted to sharing examples of sustainability in practice. Dr. R. D. Ravindran of Aravind Eye Care System will share the approaches Aravind has employed to redesign their patient flow and surgical processes in order to increase throughput and decrease environmental footprint. Tanya Harris, Global Procurement Manager, will demonstrate how choices matter and influence the complexity healthcare procurement teams face, how those impacts directly relate to some of the UN Sustainable Development Goals and ideas on where to start with sustainable procurement initiatives.

Dr. Neil Murray of the Royal Australian and New Zealand College of Ophthalmology (RANZCO) with input from Dr. Hannah Faal, Adjunct Professor of International Eye Health at the University of Calabar and Teaching Hospital and Chairman of the Board for the Africa Vision Research Institute, provide examples of eco-design renewable power solutions in healthcare in the Solomon Islands and in Africa.
