Backgroundthe increasing amount of sport active people, lead to an increase in sportrelated trauma injuries. Especially in the acute phase, fast and effective cooling as well as compression is essential to avoid excessive swelling in the effected area. Many procedures for cooling have been used, such as ice sprays, sponges soaked in ice water, bags full of ice or crushed ice and so called cold packs. The problem of these procedures is - besides others - that the cooling is mostly too short and the initial temperature is often too low, little or no compression during the cooling time, reduced mobility for the patient and limited possibility of use at remote location.Since th Liquid Ice™wraps have been developed, a self-cooling compression bandage is available. The cooling starts after just a few minutes. The lowering of the temprature of about 10° F (5°C) lasts up to 2 hours.The wrap can be reused immediately, simply by resoaking it in the Liquid Ice™ fluid. The design of the bandage allows for an even, gentle compression of the effected area.

HypothesisTreating sport trauma injuries with the Liquid Ice™ compression wrap will offer substantial advantages over treatments with crushed ice and cold packs.The superior cooling effect of Liquid Ice™ regarding the decrease of swelling and reducing of pain, as well as the practicality of the treatment was up to be tested in a prospective randomized clinical trial

Results and DiscussionA total of 50 patients, 27 of the LI-group and 23 of the CP-group, could be included into the analysis, their avarage age was 45 (17-82) and respectively 38.5 (13-79). The character of the injuries was similar, but regarding their numbers not homogeneous.The average reduction of swelling in the LI-group was 1,2 cm (2h) to 2,6 cm (72h), the CP-group reduced only 0,6 cm (2h) to1,5 cm (72h). The LI patients claimed the feeling of pain reduction according to NAS of 1,3 (2h) to 2,2 (12h) points average, whereas the CP-group-s pain reduction through cooling only was 0,3 (2h) to 1,2 (12h) points.The Liquid Ice™compression wrap was always well accepted and considered as comfortable treatment, no irritations to the skin. 3 patients of the CP-group complained about the Ice bags / cold packs as being too cold and reported an increase of pain rather than a decrease, along with local redness of skin.The easy and ready to use Ice Wrap was very positively evaluated by patients, nurses and doctors. However the costs of 1 EUR per treatment were above the cold packs and ice bags.

ConclusionLiquid Ice™ has the following advantages over cold packs(Except for the higher costs):1) faster and easier treatment2) reliable, large area compression without dislocation3) longlasting, even cooling (min. 2 hours)4) clearly more reduction of swelling during first 12 h of trauma5) substantial pain reduction (NAS 1-2 Points6) No irritation of the skin 7) faster optimization of soft tissue, with the chance of performing surgery sooner

Patients and Methodology2 groups of 30 patients each were formed. The assignation to each group was done randomly. The chosen patients had sport trauma injuries of the upper and lower extremities, especially distal radius fractures, metacarpus and ankle joint fractures, fractures of middle and forefoot, sprains and ligament ruptures.The first group (LI) was treated with the cooling Liquid Ice compression bandage, the second group (CP) with common ice bags and cold packs.The circumferences of the effected joints (in cm) were measured and documented several times during the treatment. The measurements were taken at the according points of expertise. Simultaneously the subjective feeling of pain was documented, using the numeric and visual analog scale (NAS/VAS) as well as the condition of the skin and soft tissue.The purpose was to survey the course up to the required surgery, or respectively at least 72 h with conservative treatment. Patients, who did not show up for measurement after 2 hours, were excluded from the study.

1. Estudo Experimental A amostra foi constituída por 25 estudantes dos quais 10 eram do sexo feminino

(40%) e 15 do sexo masculino (60%). A idade média da amostra era de 21,4 anos (SD

=2,3) de idade; o indivíduo mais novo tinha 19 anos de idade e o mais velho 29. Dos

indivíduos que participaram no estudo, 13 (52%) tinham como membro inferior

dominante o direito e 12 (48%) o membro inferior esquerdo. Na Tabela 1 encontram-se

os valores médios, mínimos e máximos relativos a características individuais dos

indivíduos, tais como a altura, o peso, a perimetria e a prega cutânea.

No primeiro dia do estudo, quando foi utilizado o gelo como forma de CT, a

temperatura ambiente (TAMBTE1) média era de 13,7ºC (SD = 0,5), sendo o valor

mínimo 13ºC e valor máximo 14,3ºC. A temperatura axilar (TAXILAR1) média dos

indivíduos era 36,5ºC (SD = 0,3), com um valor mínimo de 35,9ºC e um valor máximo

de 37,5ºC. Antes de iniciar o estudo foi a avaliada a temperatura da face anterior da

coxa (TGELO1) nos limites determinados. A temperatura média rondava os 28,6ºC (SD

= 1,3), com um valor mínimo de 25,8ºC e um valor máximo de 30,5ºC.

Passados 20 minutos da aplicação do gelo este foi retirado e foi avaliada a

temperatura (TGELO2). Em média os indivíduos apresentavam uma temperatura de

6,8ºC (SD = 1,8), com um valor mínimo de 3,5ºC e máximo de 11,7ºC. Vinte minutos

após retirar o gelo a temperatura foi avaliada novamente (TGELO 3) e a amostra

apresentava um valor médio de 21,8ºC (SD = 1,0), sendo o valor mínimo 18,6ºC e o

valor máximo 23,5ºC.

No segundo dia do estudo foi utilizado o Liquid IceTM como forma de CT. Neste

dia a temperatura ambiente média (TAMBT2) era de 15ºC (SD = 1), sendo o valor

mínimo 13,5ºC e o valor máximo 16,5ºC. A temperatura axilar média (TAXILAR2) dos

indivíduos era de 36,6ºC (SD = 0,4), sendo o valor mínimo de 36ºC e o valor máximo

de 37,6ºC.

Antes de ser colocado o Liquid IceTM (TLIQICE1) a temperatura média da face

anterior da coxa dos indivíduos era de 28,6ºC (SD = 1,61), sendo o valor mínimo de

24,9ºC e o valor máximo de 30,8ºC. Quando terminou a aplicação do Liquid IceTM, após

20 minutos deste ser colocado (TLIQICE2), a temperatura média era 25, 2ºC (SD =

1,6), sendo o valor mínimo 22,5ºC e o valor máximo 27,2ºC. Vinte minutos após a

retirada do Liquid IceTM (TLIQICE3) a temperatura média da coxa era de 27, 39ºC,

sendo o valor mínimo 24,3ºC e o valor máximo 29,9ºC.

Após se verificar a homogeneidade das variâncias (p> 0,05) com recurso ao

Teste estatístico de Levene, calculou-se o valor de p para verificar se existiam igualdade

entre os diferentes valores da temperatura recolhidos (TGELO1, TGELO2, TGELO3,

TLIQICE1, TLIQICE2,TLIQICE3) usando o teste paramétrico ANOVA. Uma vez que

o p – value da ANOVA é 0,00 <0,05, rejeita-se a hipótese de igualdade dos valores

médios das temperaturas nos diferentes momentos (α=5%). Para averiguar quais os

valores médios que apresentavam diferenças significativas foi utilizado o Teste de

Scheffé (teste de comparações múltiplas).

Pela análise dos resultados obtidos constatou-se existirem diferenças

significativas entre os diferentes momentos de avaliação (0,00 <0,05), com excepção

dos testes: “TGELO1”x “TLIQICE1”, (1> 0,05); “TGELO1” x “TLIQICE3”, (0,191>

0,05) e o teste “TLIQICE1” x “TLIQICE3” (0,158> 0,05), para um α= 5%.

Neste estudo, o aparelho de termografia utilizado demonstrou ser fidedigno uma

vez que não houveram diferenças significativas (p=1) quando se compararam os valores

University Institute of Body Motion Science (biomechanics) , Rome - IUSM

At the medical service of the IUSM we have used, for about 3 months, the Liquid Ice wraps.They have been used on 31 patients, 18 females and 18 males , aged 22+/- 1,5 years.

The pathologies were: - 12 instep sprains (8 right, 4 left)- 7 knee sprains (5 right, 2 left) - 2 sprains of the metacarpus-phalanx joint of the first finger of the

hand (2 right) - 5 elbow bruises (3 left, 2 right) - 2 bruises of the lumbar-sacral site of the vertebra - 3 bruises of the back of the foot (right)

To those patients affected by instep and knee sprains , considered the seriousness of the trauma, additional wraps were given away to be applied at home after 4 hours from the previous treatment.The followed up patients showed tangible appreciation of the product, both for the technical qualities ( materials, smell,, ease of use) and for the clinical outcomes obtained by using Liquid Ice ( reduction of pain and swelling, relief feeling).The site affected by the trauma showed a tangible swelling reduction . None of the patients was affected by skin reactions.

Because of the above mentioned results, I believe that Liquid Ice is surely useful , especially as a first aid, in sport and general traumatology , particularly when used within 24-48 hours replacing ice bags and similar cooling products . Moreover, I think that this product can be successfully used also for the treatment of chronic inflammatory pathologies.It is recommended the use of the of the longer version of the wrap when treating extended traumas of the lower limbs.

TMIm Sanitätswesen von IUSM haben wir ca. drei Monate lang die Liquid Ice Bandagen verwendet.Getestet wurden sie an 31 Patienten, 18 weiblich und 13 männlich im Alter von 20 - 24 Jahren.

Verletzungen: - 12 Handgelenkverstauchungen (8 rechts, 4 links)

- 7 Knieverletzungen (5 rechts, 2 links)- 2 Verstauchungen am metacarpus-phalanx Gelenk des ersten Fingers (2 rechts)- 5 Prellungen im Ellbogenbereich (3 rechts, 2 links)- 2 Prellungen des Lendenwirbels- 3 Verstauchungen am Knöchel (rechts)

Denjenigen Patienten, welche unter Handgelenk- und Knieverletzungen litten, haben wir, unter Berücksichtigung der Ernsthaftigkeit der Verletzung, weitere Bandagen mit nach Hause gegeben, damit sie diese 4 Stunden nach der ersten Behandlung erneuern können.Die weiterhin in Behandlung gebliebenen Patienten zeigten grosse Begeisterung über das Produkt. Sowohl über die technischen Eigenschaften (Material, Duft, Anwendung) als

TMauch am klinischen Ergebnis, der Wirkung von Liquid Ice erreicht wurde (Reduzierung von Schmerz und Schwellung, Entlastungsgefühl).Die von der Verletzung betroffene Körperparthie zeigte schnell eine Verminderung der

TM Schwellung. Bei keinem Patienten trat unter Anwendung von Liquid Ice eine Hautirriation auf.

TMAufgrund obgenannter Resultate stufe ich Liquid Ice als sicherlich nützlich ein, besonders für den Erste Hilfe - Gebrauch im Sport sowie für allgemeine Behandlungen, Stauchungen und Schwellungen, insbesondere wenn innerhalb der ersten 24 - 48 Stunden anstelle von Eisbeuteln oder ähnlichen Produkten verwendet wird. Ausserdem denke ich, dass dieses Produkt erfolgreich für die Behandlung von chronischen Entzündungen verwendet werden kann.Bei der Behandlung von erweiterten Verletzungen der unteren Gliedmassen empfiehlt es sich, die „längere“ Version der Bandagen zu verwenden.

I highly recommend the use of Ice wrap after orthopaedic surgery for reducing swelling and pain.

I use Liquid Ice™ every day after knee replacements , crucial ligament surgery, and Arthroscopic surgery.

I also use the Liquid Ice Wrap Professional after all my of foot surgeries as it cools comfortably and forms to the bones of the foot.
