SLC Registration Information

Registration Online Registration Opens February 1, 2017

Registration Deadline Registration closes at midnight Mountain Time (MST) on February 28,

2017 NO late registration accepted.

Registration Fee $60.00 per Student Delegate, Advisor, Chaperone, or Guest.

Each Chapter MUST collect their student fees and pay with a school

check. Idaho HOSA does not accept credit cards or personal checks.

Non-rooming Fee $30 per Student Delegate and Advisor that does not stay at the conference

hotel. Please indicate upon registration.

Refunds No refunds will be made for cancellations after midnight Mountain Time

(MST) on March 24, 2017.

National Affiliation Chapters must be affiliated with National HOSA by February 1st, 2017.

Important Notes Advisors need to make sure student names are spelled correctly in the

online system. Idaho HOSA will not reprint name badges onsite for

spelling errors.

Flag Bearers Identify two flag bearers to represent your chapter and tag them as “Flag

Bearers” on the online registration form. They will be presenting your

schools banner or sign at the beginning of opening ceremonies. Be sure to

bring your HOSA banner or sign to SLC.

Voting Delegates Identify two voting delegates from your chapter and two

alternates to serve at the Delegate Assembly to vote for State

Officers and By-Law Changes. Please tag them on your online

registration form.

Online Registration Instructions

1) Go to: www.hosa.org/hosaconf/

2) Log in as a Local Chapter Advisor using your affiliation login (charter #) and password

If you do not have your login or password please contact Holly Connley at

[email protected] or call 208-429-5526.

3) Select “Conference Registration”

4) On top of the left side of the screen:

a) Choose the dropdown “Select a Conference”

b) Choose the conference “State Leadership Conference 2017”

c) Click “Begin Registration”

5) Select the name of your first participant and select “Register”

NOTE: If your participant name does not show up then they have NOT been affiliated

with National HOSA and you must go back and complete the member affiliation.

Indicate whether the participant is Male/Female.

Indicate whether the participant is Secondary/Postsecondary/Advisor, etc.

6) Advance to activities/options and select activities for that student.

7) Go to events and select the competitive events the student will participate in

See the RULES in the “Events List 2017” to make sure your students are registered

correctly.

Students may participate in as many recognition events as they choose.

8) In the team number box, please indicate the team number for the team event:

Team numbers only apply to the specific events (example: Team #1 in CPR/First Aid and

another Team #1 in CERT Skills.)

9) After registration for each student is complete, click “Submit”.

10) Continue to register all SLC participants in the same manner.

11) Register all advisors, chaperones and guests (registration fees will also apply).

12) After completing your online registration, be sure to print your invoice.

Competitive Event Information

Conference Eligibility Students must be affiliated and paid members to compete at the State

Leadership Conference. Students must place at State Conference in the

top three (3) of their event and attend the Closing Awards Ceremony in order to qualify to compete at the International Leadership Conference

(ILC). Paid HOSA members are eligible to attend ILC and participate in

symposiums, etc.

Event Guidelines Student Delegates must have a copy of the event guidelines with them

upon checking in to their event. This CAN be an electronic copy on their

smart phone/electronic device.

Substitutions Substitutions may be done, provided the Student Delegate meets the

same requirements of the competitors, is entering the same event,

and the information is received by the state office by March 27,

2017.

HOSA Bowl Only one (1) team per chapter is allowed to participate in HOSA

Bowl.

Recognition Events All “Recognition Event” portfolios must be turned in at SLC

on April 5, 2017 at the Advisors Meeting. No Exceptions.

Event Finalist Process Certain competitive events consist of two rounds. Round One is a written,

multiple-choice test of knowledge and understanding, and Round two is a

performance/skill. Depending on the number of competitors, the Idaho

HOSA testing process may lead to only the top scoring competitors

proceeding to Round Two. Competitors are encouraged to be prepared for

both rounds, even though they may not participate in Round 2.

Policy on Cell Phone/

Electronic Devices

A student CAN show event guidelines to event judge on their electronic

device. Once this is done the phone must be turned off and placed face-

down on the table in front of them.

Registration Add-ons

T-shirts Conference souvenir t-shirts can be purchased at online registration for $10 apiece. This year’s t-shirts will be

light, Heather Gray, and will be offered in the sizes Extra-small through 3X-Large. HOSA t-shits will not be

available after online registration is closed.

HOSA Lapel Pins Each SLC registrant will receive a HOSA State Lapel Pin. Some students may want more than one pin to trade at

the International Conference. Additional pins may be ordered during online registration for $1.00 apiece.

Conference Logistics and Procedures

Registration Materials Conference packets will be available for pickup upon arrival at the Hotel

between 3:00 p.m. – 5:00 p.m. on Wednesday, April 5, 2017. Packets will

contain official programs, name badges and conference gifts.

State Officer

Applications

Applications will be available on February 1, 2017 on the Idaho HOSA

website at: https://cte.idaho.gov/students/student-organizations/hosa/

Applications are due to the CTSO Coordinator, Holly Connley, and must

be postmarked no later than March 3, 2017.

Advisor / Chaperones All Conference Delegates must have an advisor in attendance. It is

recommended to have one advisor or chaperone for every 10

Student Delegates.

Raffle Gift Baskets All chapters are requested to bring a gift basket to be raffled during

the conference. All money raised will go to the Idaho HOSA

Scholarship Fund.

Code of Conduct Signed Code of Conduct forms will be turned in at advisors meeting

on April 5, 2017. The advisor will keep a copy of the Student Delegate’s

Code of Conduct.

Medical Release Forms It is required that each Student Delegate complete the Medical Release

Form. It is the Advisor’s responsibility to keep these with them at all

times during the conference.

Dress Code Student Delegates must adhere to National HOSA’s dress code guidelines.

See following page.

Thursday Night Activity Wahooz Fun Zone, 7:00-10:00pm This activity is for registered conference participants ONLY and must have

their name badge to enter. Participants will receive a wristband upon

arrival. Bussing will be available to transport to and from the hotel between

6:30pm-11:00pm. Participants who leave the premises will not be allowed

to return to Wahooz.

HOSA is an organization for Future Health Professionals and professional appearance is paramount in branding itself as such. It is the responsibility of all HOSA members to follow dress requirements for SLC. For most skilled events, scrubs and clinical shoes or tennis shoes are acceptable clinical

attire. Other dress may be appropriate to the occupational area and acceptable for HOSA

competitive events. Shorts and flip flops are not appropriate clinical attire. Scrubs can be

any color/design, with or without a HOSA emblem.

Thursday Night

Activity - Wahooz

Casual slacks, shorts, jeans, polo, HOSA t-shirt, or tasteful/modest athletic wear. Dresses

and skirts not recommended during Wahooz.

Unacceptable Items

Online Testing Information

Online Testing Online testing must be completed March 1–17, 2017 at your home school.

See the attached Event List for these selected events.

Finding a Proctor Advisors cannot serve as a proctor for online testing. Each advisor is

responsible for selecting a proctor at their school for online testing. This

proctor must be a 3rd party entity, not connected to HOSA. A faculty

member or other administrator from your school is allowed.

Submission of

Proctor Information

Once a Proctor is identified, the Advisor must SUBMIT their Proctor’s

information online by following these steps:

1) Login to www.hosa.org

2) Under ‘Conference Registration’ click on “Identifying Proctors for

Online Testing” and fill in the Proctor information

3) The Proctor is required to sign the “Proctor’s Code of Ethics” form

and must agree to fulfill the role of Proctor during online testing.

4) The Code of Ethics form must be scanned and e-mailed to Holly

Connley at [email protected] by February 21, 2017.

Online Proctoring Instructions

Proctors: After the HOSA advisor has identified you on the website you will received an e-mail with

your username and password.

Follow these steps to proctor a test:

1) www.hosa.org

2) Click “Login” on the upper right corner

3) Go to “Online Testing Proctor” and Click “Proctor A Test”

4) Login using your username and password

5) You will find detailed testing instructions once you login

6) Your student’s names and passwords will show up if the

student is in multiple tests they will have separate passwords

for each test.

In the e-mail subject line please put “School Name, HOSA Rooming List”

Room lists are due to Darcy by March 14th. The reservations phone line

will NOT take your reservations.

Tax Exemption When paying through your school you can set up a tax exempt status with

your school’s tax exempt number. Request a hotel-specific tax exempt form

from Darcy at the above email. Turn-in the proper documentation

PRIOR to arrival. If you are paying individually, tax will be assessed.

Payment Options P.O.’s are due to the hotel prior to check-in. Please fax, scan, or mail P.O.’s

to Darcy at the address above. If paying by check, these must be turned in

at time of check-in. Credit Cards are accepted, see Credit Card

Authorization Form.

Late Checkout Request You can request 1-2 rooms for a late checkout for Friday afternoon until 4:00pm on a complimentary basis. Add check out time to this.

Current State Officers Current HOSA state officer’s rooms will be paid by Idaho HOSA and reservations will be made by the Officer Coordinator.

Bus Driver Rooms When reserving rooms for your bus drivers, do so as you normally would with your group. Please indicate “Bus Driver” next to their name on the rooming form. The hotel will make effort to place your drivers in a quieter location of the hotel. Please pay the conference hotel as normal.

Idaho HOSA Code of Conduct Form

Code of Conduct Form must be completed and given to the Chapter Advisors. All chapter Code of Conduct Forms will be turned in at SLC Headquarters upon arrival at SLC.

A good reputation enables members to take pride in their organization. IDAHO HOSA members have an excellent reputation. Your conduct at any IDAHO HOSA function should make a positive contribution to the reputation that has been established.

1) Your behavior should be such that it reflects credit to you, your school/college, your state and IDAHO

HOSA.

2) The general session should be enthusiastic, but we must be courteous to those in the audience or on

stage. It is important to remain seated and in attendance until the end of the session. Students who do

not adhere to general session protocol will be asked to leave the conference.

3) You are expected to attend all general sessions and other scheduled conference activities. Please be

prompt and show respect to audience and those on stage.

4) Student delegates are to abide by the SLC dress code policies at all business sessions, general sessions,

competitive events and other conference activities. Please refer to Dress Code Policy on Page 9. Student

Delegates not adhering to the dress policy during the conference will NOT be admitted.

5) Idaho HOSA Conference Name Badges shall be worn at all times, however school tags, state badges or

ID’s must be removed or completely covered during competition.

6) Student Delegates are to report any accidents, injuries or illness to their local or state advisor

immediately.

7) Students Delegates shall keep their advisors informed of their activities and whereabouts at all times.

8) Student Delegates are to observe the designated curfew. (Curfew means being in your assigned hotel

room by the designated hour)

9) If a Student Delegate is responsible for stealing, damage to property or vandalism, the student and

his/her parents will be expected to pay any and all damage costs.

10) Any participants at the HOSA State Leadership Conference may not purchase, consume or be under the

influence of alcohol or drugs at any time. Violators will be subject to disciplinary action.

11) No weapons of any sort will be allowed on the student’s person, room, transportation or luggage.

12) Smoking is permissible to those of legal age but only in designated areas defined by the facility.

13) Any long distant phone call or other charges to rooms, etc. will be the responsibility of the individual

students and/or parents.

Student Delegates who disregard the rules will be subject to disciplinary action and will be sent home at their own expense. Parents will be notified. By signing below, you confirm that you have read the above Code of Conduct for IDAHO HOSA Conferences and agree to abide by these rules.

Advisor Code of Conduct

1) Idaho HOSA Advisors project a positive and professional image of Health Professions Education

and Idaho HOSA to all those whom they interact.

2) Idaho HOSA Advisors promote Idaho HOSA as a positive student experience; therefore, will act

as a positive role model for students. This will be portrayed and not limited to the way they

dress, tone of voice, attitude, actions and demeanor.

3) Idaho HOSA Advisors are first in line to ensure that their Student Delegates are dressed in

appropriate HOSA attire (see page 9) and are practicing professional etiquette and behavior.

4) Idaho HOSA Advisors are accountable to and for their students during all Idaho HOSA related

activities.

5) Idaho HOSA Advisors will notify the State Advisor, Joanne Clovis, or CTSO Coordinator, Holly

Connley, of any accidents or injuries of Student Delegates, Advisors, Chaperones or Guests that

take place during Idaho HOSA activities or conference.

6) Idaho HOSA Advisors understand and follow established processes within the organization that

protect the rights of all members.

Idaho HOSA advisors are proud of the standards of excellence they maintain for themselves and their

students. Attendance of any Idaho HOSA function implies acceptance and practice of these standards.

Image Policy and Opt-Out Provision Form

Idaho HOSA, as part of the National HOSA Organization, asks that Idaho chapter advisors notify

the state HOSA office of photo/video release objections from their students. Idaho HOSA reserves the

right to take photo and video images at its public events when its members are engaged in related

activities held in public venues. Idaho HOSA reserves the right to own, alter, reproduce, and distribute

its images. Participants engaged in the activities of the Idaho HOSA assume that their image may be

taken at one of its many public events. Members and advisors that wish to opt out and decline their

image being released must fill out the bottom section of this form. Forms shall be turned in at the

advisors meeting at SLC. It is the responsibility of the chapter advisor to adhere to his/her local school

district policies and procedures related to image releases. If a local school district prohibits the

release of its students’ images, the chapter advisor must notify the state HOSA office. Local school

district policy supersedes state HOSA policy as related to image release.
