HealthAsyst partnered with the customer to build a care co-ordination platform with real -time bidirectional integration with the different subsystems involved. The solution included a workflow management component for planning and integrating care co-ordination activities, with checkpoints, alerts, and notification for timely interventions. Patient assessment and integration of care plans were also made possible. Care plans could be securely shared with the entire care team. It also provided user-friendly and highly actionable analytics that optimized user workflows.

The entire solution was developed in a distributed agile environment using industry best practices.

The intuitive interface, workflow management capabilities and real-time notifications resulted in high end-user adoption. By providing right information to the care givers and patients, the solution influenced their behavior, thereby reducing re-admissions.

Risk Analysis became more systematic with the new features. The solution helped the end users reduce redundancies and cost associated with care delivery.

While working with multiple care givers who were part of ACOs, the customer realized that achieving a coordinated workflow based on the defined care plan, was a big challenge. Also, integrating the data from multiple sources and reconciling it in a timely manner, at the point of care system, was difficult. Standard terminologies were not maintained even within the different systems of the customer. The care co-ordination platform would allow risk stratification and predictive analytics for patients participating in the co-ordinated care model.
