Thank you for applying for the above post with the Ayrshire Hospice. This information sheet summarises the terms and conditions related to the post. The Hospice has recently undertaken a Pay and Benefit Review and the following terms and conditions will be effective from 1 April 2016.
ABOUT THE POST:

Salary/Grade Scale: £15,512 - £18,284 pro rata, per annum
Your salary will be paid into your bank account on the last Friday of each month.
Hours of Work: 33.75 hours per week
Annual Leave: Leave year is from April – March
30 days (225 hours) on commencing employment32 days (240 hours) after 5 years’ service
For part time staff, this will be applied on a pro-rata basis and also allocated in hours.
Public Holidays: Entitlement is 37.5 hours based on the 5 designated Public Holidays per annum, and on a 5 day week working 7.5 hours per day. The 37.5 hours entitlement is due to all whole time staff irrespective of work pattern.

Pensions: The Company will comply with the employer pension duties in accordance with Part 1 of the Pensions Act 2008 by automatically enrolling you where eligibility criteria is met, in the Group Personal Pension Plan.
The Hospice provides a generous employer contribution of 7.5% of your salary per month provided a minimum 2.5% employee contribution is made.

2. JOB PURPOSE
Under the supervision of the IPU Charge Nurse and Staff Nurses and in collaboration with the multi-disciplinary team the post holder will actively participate in the delivery of evidence based person centred care.
3. DIMENSIONS
The IPU has 20 beds providing respite, symptom management, multi-disciplinary team assessment and last days of life care for people with life limiting illnesses. The post holder will assist in the assessments and care of a delegated case load.

4. ORGANISATIONAL POSITION
5. ROLE OF DEPARTMENTThe Ayrshire Hospice envisions a community where access to specialist palliative care services and support is available equitably throughout the community.
The Ayrshire Hospice is a specialist resource for those individuals receiving palliative care, who have particularly complex needs that cannot be fully met in their usual place of care. The hospice strives to meet physical, emotional, social and spiritual needs providing the highest standard of person centred care possible while ensuring choice, dignity, self-esteem and an enhanced quality of life. The hospice engages with family and friends offering them support and bereavement counselling.
Working in collaboration with NHS Ayrshire & Arran and other agencies, the hospice strives to promote excellence in palliative care through facilitation, enablement and support throughout the community.
The Ayrshire Hospice is widely acknowledged as a centre of specialist palliative care expertise. The organisation is committed to research and provides extensive education and training opportunities for both internal and external healthcare professionals. This professional development is essential to ensure that the hospice takes a leading role in facilitating the highest quality evidence based palliative care for patients.
6. KEY RESULT AREASCLINICAL
• Carry out individual patient care needs, ensuring safe, effective person centred care is carried out with dignity and respect under the supervision of Staff Nurses and IPU Charge Nurses and as prescribed by the registered nurse.
• Work collaboratively with all members of the MDT ensuring effective communication and the delivery of high standards of care.
• Actively participate in patient journey from admission to discharge or death, including after death.
• Ensure that own practice is safe and effective.
• Act as a role model for new staff and students to enhance their knowledge, communication skills.
• Support new staff and students in the provision of safe and effective care.
• Maintenance of accurate and relevant documentation through both electronic and paper based systems. Ensure all documentation is tailored for each individual and reflects person centred care.
• Demonstrate an active role in maintaining the necessary standard set by Healthcare Improvement Scotland.
• Escort patients to hospital appointments as directed by IPU Charge Nurse
• Handle patients’ belongings according to Ayrshire Hospice policy.
• Ability to prioritise workload to meet the needs of the service.
• Maintain patient confidentiality at all times.
• Maintain good relationships and an empathetic approach to patients, carers and relatives and refer them to a registered nurse for any questions they may have on a patient’s condition, or for any suggestions or complaints they wish to raise.
OPERATIONAL
• Promote and implement all Hospice policies and procedure at department level• Maintain knowledge of policies and national guidelines• Report all incidents through the incident reporting mechanisms and take responsibility for
own learning through this process in order to reduce risk of further incidents.• Participate in maintaining stock levels of nursing supplies by demonstrating an
awareness of stock control systems and how they should be used.• Ensure effective use of resources.• Ensure that equipment is clean and ready to use to support the safe running of the IPU.• Report any incidents or complaints and provide statement as requested by IPU Manager
or Charge Nurse.•
• Take responsibility for own health and safety and contribute to the management of this within the department undertaking risk assessments to ensure safety of self, patients and other members of staff.

EDUCATION
• Participate in clinical supervision and reflective practice and be responsible for integrating into own everyday practice.
• Participate in the orientation and induction of new staff and volunteers within IPU under the supervision of IPU Manager and Charge Nurses.
• Take active role in self-development and knowledge as opportunities arise.
• Provide support to patients, family members, students and volunteers.
• Attend annual mandatory training.
• Maintain responsibility for own personal development under the guidance of IPU Staff Nurses, Charge Nurses and Manager.

8. ASSIGNMENT AND REVIEW OF WORK
• The Care Assistant will report directly to the Staff Nurse.• Workload is generated by the clinical activity within the IPU.• The Care Assistant will be responsible to the IPU Charge Nurse for clinical guidance and
professional management, work review and formal appraisal of performance.• Work load will be assigned by the IPU Charge Nurse and Staff Nurse; however the Care
Assistant will have the responsibility for carrying out care safely and effectively.• Formal review and objective setting will take place with the IPU Charge Nurse annually.• Peer group review.
9. DECISIONS AND JUDGEMENTS
• Managing and prioritising workload relating to direct patient care on a shift by shift basis.• Ability to recognise a change in patient condition and know when to refer on to a Staff
Nurse• The post holder will be required to use own initiative and report back to Staff Nurse• Ability to quickly assess and respond to patients needs in emergency situations.• Ability to recognise own limitations and the requirement for appropriate referral.

12. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOBThe post holder will be involved in mainly clinical activity and may frequently be required to move between tasks at short notice to meet the needs of the service. Workload is unpredictable, with priorities constantly changing.
