A PUBLICATION OF THE HOSPICE & PALLIATIVE CARE GROUP, INC.

For over a decade, Palliative Home Care of Niagara has supplied durable medical equipment and licensed home care services to primarily hospice patients throughout Western New York. In an effort to help expand these services to more non-hospice patients, two very important initiatives have been launched.

First is the creation of Liberty Home Care as the new name and identity of the organization. This change was implemented to simplify marketing of the organization to the public, a challenge not easily overcome by the original name of Palliative Home Care of Niagara. The second initiative involves the opening of a second lo-

Introducing Liberty Home Care

Camp Hope is a weekend bereave-ment camp for children ages 7-13 with the goal of helping them cope with the recent death of someone close to them. It is offered at no charge to all grieving children in the Western New York area, whether or not their loved one ever ex-perienced hospice care. The Circle of Hope program of Niagara Hospice will hold its 12th Camp Hope for griev-ing children June 29 - July 1, 2012 at the YMCA’s Camp Kenan in Barker.

The camp is staffed by profession-al counselors, trained volunteers and a nurse who is on hand for the entire weekend. During the camp weekend, children learn to express their grief through many activities such as yoga, karate, arts and crafts, pet visits, and a memorial bonfire. Outdoor recreation like rock wall climbing, making and eat-ing S’mores around the campfire, swim-ming and nature walks are also enjoyed.

With a gener-ous gift recently given by the family of Roger A. Foote, Niagara Hospice has created the Roger A. Foote Camp Hope Endowment. This endowment

was established to provide the finan-cial ground on which Camp Hope will continue to serve the children of tomorrow. Mr. Foote’s love of chil-dren makes this gift a fitting tribute.

For a free camp application, call the Outreach Department at (716) 280-0777 or visit www.Niagara Hospice.org for printable forms. Message from the CEOWhen Bob Dylan released The Times they

are a Changin’ in 1964, it soon became a phrase that defined an entire generation. But little did he know that more than four decades later, this verse could also be used to describe today’s healthcare environment.

Despite the winds of change, we continue to provide the necessary services our families need. Undoubtedly, you will read in the future of many changes occurring at the State and Federal level. Change is good and we embrace the opportunity to help you while complying with the many new mandates on the horizon.

As the New Year approaches we recommit ourselves to making Niagara County a better place to live. For almost 25 years we have part-

nered with you during some of your most difficult times. We have traveled a journey with you that at times may have seemed hopeless. We have cried with you, smiled with you and most importantly – shared memories as you have invited us into your home and hearts. Theodore Roosevelt once wrote: “The best thing you can do is the right thing; the next best thing you can do is the wrong thing; the worst thing you can do is nothing.” Rest assured that we will always strive to do what’s right for you!

During this holiday season, all of us Niagara Hospice and Liberty Home Care wish you and yours a safe and joyous season.

Peace to all,

John L. Lomeo, President & CEO

The Hospice and Palliative Care Group, Inc. Mission Statement:

“ The Hospice and Palliative Care Group is dedicated to providing the right care, in the right place, at the right time.”

Niagara Hospice and Northgate Health Care Facility establish partnership

Healthcare statistics reveal that approximately one-third of all persons entering nursing facilities die within 12 months of their admission. But despite this well established fact, hospice services among nursing home residents is much lower than in other health care environments. According to a study sponsored by the National Hospice and Palliative Care Organization (NHPCO), only 6% of nursing home residents in the U.S. currently elect the hospice benefit, even though nearly one-in-four deaths in the U.S. occur in a nursing home.

There are a number of reasons for this. The territorial nature of health-care providers, conflicting regula-tions, and Medicare reimbursement all play a role in the under-utilization of hospice care in nursing facilities. But perhaps these and other challenges can be overcome with the right chemistry between two providers committed to delivering the very best to those individuals entrusted into their care.

After months of discussion and planning, Niagara Hospice and the McGuire Group’s Northgate Health Care Facility have found this synergy. Af-fectionately named Jeanne’s House, after a beloved Northgate employee who utilized Hospice services herself, a 22 bed unit at Northgate will be devoted to meeting the physical, emotional and spiritual needs

of residents requiring both 24 hour skilled nursing care as well as the unique services that only Niagara Hospice can provide. Ms. Terry Collins, RN and Ad-ministrator at Northgate, commented that this collab-oration could very well serve as a model for future relationships between nursing homes and hospices.

“Jeanne’s House is a natural extension of the culture that already exists here at Northgate,” stat-ed Collins. “By partnering with Niagara Hospice,

we have the ability to define our vision and offer one-of-a-kind services to those in our community.”

As a center of excellence for hospice care, staff on the unit will be specifically trained to meet the requirements for providing this spe-cialized level of care. Through this collabora-tion the unit will be dedicated entirely to hos-pice patients and feature beautifully decorated private and semi-private rooms; a lounge with

Total Hospice Experience is different for everyone

Since 1988 Niagara Hospice has developed a proud history of grateful patients and family members. Our staff and vol-unteers often go above and beyond to learn what “comfort” means to each individual patient, and what’s most important to them now.

Many people are surprised to learn what the Total Hospice Experience (THE) entails. As an organization that always strives to improve, the Niagara Hospice of today is not the Niagara Hospice of 1988. Thanks to an amazing and commit-ted workforce, hospice is so much more than what most people think. Many of you are familiar with the variety of services hospice provides that include nursing, social work, spiritual care, hospice aides, pharmacy, massage therapy, volunteers, and durable medical equipment (DME). But as we have grown, so has our commitment to providing individualized care to each patient.

Following are some of the things our staff and volunteers provide to patients who choose the hospice model of care:

• Comfort totes, each with a handmade quilt• Hand crafted birthday cards and balloons • Home makers clean the home, run errands and offer

companionship• PeaceofMind personal emergency response systems

(PERS) • MealstoGo for patients and caregivers with nutritional

needs• Spa day for Hospice homecare patients, with lunch

served at Hospice House• Ambulette transportation• WeHonorVeterans – specialized services for all our

veterans • Assist families with Advance Care Planning •AcrosstheMiles visits connect distant loved ones to pa-

tients utilizing Skype enabled netbooks. Through Skype, a video conference allows a patient to see and talk to an otherwise unreachable loved one.

Additionally, after a patient passes while under the care of Niagara Hospice, services do not stop for the family. Bereavement services are a crucial component of THE. In ad-dition to grief counseling, the outreach counselors at Niagara Hospice provide a broad spectrum of services for families you may not be aware of including re-connecting broken friend-ships, linking people with community resources, assisting in organizing financial records and re-organizing household belongings, and helping re-distribute loved one’s belongings. Services are provided to children as well.

The CircleofHope program, located at the Niagara Hospice Resource Center in the Ulrich City Centre in Lockport, also provides community outreach and grief support to Niagara County residents of all ages, whether or not they ever experi-enced hospice care.

THE is ever-changing. As we identify the needs and wants of those we serve, we will do our best to meet them.

Thank you, Hospice Heroes!Hospice Heroes are special every day heroes that give of themselves in the form of an event or activity to raise both funds and awareness for Niagara Hospice. Niagara Hospice thanks the following com-munity groups, organizations and individuals that have shown their support of the hospice mission:

Events2011

Bouquet of Love was held last sum-mer in remembrance of Kathy Waite who lost her 11 year battle with breast cancer. Her friends and family raised nearly $6,700 at the event complete with prize raffles, a bas-ket auction, and a great barbeque dinner to benefit Niagara Hospice. “We thank Niagara Hospice for their service to the community and for being there to help, any hour of any day,” said long-time friend of Kathy, Donna Berczynski. Pictured from left are Kathy’s nurse from Niagara Hospice, Judith Johnson, RN, and Kathy’s family – Emily Claude and Cheryl Bridges.

The Milherst Construction Charity Golf Classic was held last summer in support of Niagara Hospice. Held at Arrowhead Golf Club in Akron in re-membrance of Cindy O’day, the tour-nament raised $7,00. Jim Collins, Jr. of Milherst Construction, Inc. (pictured at left) presents a donation to Niagara Hospice President & CEO, John Lomeo.

Viatran Corporation, a Dynisco company based out of Wheatfield, NY host-ed a Chiavetta’s Barbeque in support of the programs and services of Niagara Hospice. With the support of the community and their vendors, employees raised a stagger-ing total of over $5,000 that was donated to Niagara Hospice. Pictured from left are Viatran employees Diane Kankiewicz and Candy Grabowski presenting a check to Niagara Hospice Alliance Development Coordinator, Thomas LaChiusa.

The Voyager is going GREEN!Many Americans are embracing a greener lifestyle that

can not only help preserve equatorial rain forests – it can also mean improving your health, padding your bank account, and ultimately improving your overall quality of life. The Hospice and Palliative Care Group has made the decision to take TheVoyager digital, recognizing that doing a little something, is better than doing nothing, and that a little something can have a big impact when many of us do it.

Consider the following information that may help inspire you to go green with at least one small action:

• Each American generates between 5 to 7 pounds of waste per day; over a ton per year; and over 175,000 lbs. in a lifetime.

• Although the U.S. makes up 5% of the world’s population, we produce over 40% of the world’s garbage – more than any other country.

• The rain forest is being cut down at 100 acres per minute.• Recycling one ton of paper saves between 15-17 trees,

7,000 gallons of water, and nearly half-a-year’s worth of electricity for the average household in the United States.

• Nearly 1 billion trees worth of paper are thrown away every year in the United States.

Available from the Resources page of our website at www.NiagaraHospice.org, TheVoyager will soon have the benefit of full color while becoming less costly – and saving trees! It is our hope that you visit the website often for news and updates, or sign up for an e-mail version of TheVoyager.
