Updated Online Employee Injury and Illness Incident Reporting Form On March 17, 2020, MIOSHA introduced a revised online form for use in reporting workplace injuries and illnesses. This form includes the ability to report any workplace injury or illness that resulted in an in-patient hospitalization, amputation, or the loss of an eye, as well as injuries caused by a mechanical power press even if the injury did not result in an in-patient hospitalization, amputation, or the loss of an eye. This form is now compatible with all of the most common web browsers and does not require any additional software. If you have any difficulties using the online reporting form, please call MIOSHA’s toll-free Severe Injury and Illness Reporting hotline at 844-464-6742. Associated General Contractors of Michigan Safety Day MIOSHA Construction Safety and Health Division (CSHD) representatives participated in the annual Associated General Contractors (AGC) of Michigan Safety Day on March 5, 2020. This year the event was held at VisTaTech Center, part of Schoolcraft College in Livonia, MI. Several construction vendors, construction employers, safety and health professionals, and tradespeople were present to learn about construction improvements on the job, products, and topics of concern. Several sessions were offered to the attendees to gain knowledge in specific areas of safety and health that ranged from crane operations to marihuana laws. Amongst the sessions offered, MIOSHA representatives spoke to help the industry improve their knowledge on MIOSHA laws, best practices, and enforcement issues. CSHD Supervisor, Tarah Kile presented on Construction Fatalities, alerting participants to the hazards surrounding various work operations in the hopes of preventing these incidents from occurring again. Appeals Coordinator, Katrina DeBose presented on the current status of the Appeals Division and how the MIOSHA appeals system works.

Michigan Worker Deaths of 2020 To date, there have been five work-related fatalities reported to MIOSHA in 2020. The information below shares preliminary details about the most recent fatalities reported to MIOSHA which are believed to be covered by the MIOSH Act. The description reflects information provided to MIOSHA at the initial report of the incident and is not the result of the official MIOSHA investigation. To report fatalities/catastrophes, call MIOSHA at 800-858-0397. Preliminary summary of incidents: On March 13, at approximately 8:30 a.m., a 42-year-old mechanic was changing the oil in a car that was a manual transmission. Another mechanic started the car to cycle the oil and released the clutch, causing the car to jump forward striking and pinning the mechanic against a toolbox. Emergency services were called and he was taken to hospital where he was pronounced deceased. On March 13, at approximately 10:45 p.m., a 23-year-old volunteer firefighter was performing firefighting operations on a tri-level residence. During the structure fire, the firefighter partially fell through floor joists on the upper level of the residence. A “mayday” was issued from other firefighters inside the residence. An emergency response team was assembled and entered the residence to extract the fallen firefighter. He was evacuated from the scene and pronounced dead en-route to the hospital.

On March 18, at approximately 9 a.m., a 35-year-old machine operator was loading and unloading parts into the cavity of a 200-ton horizontal hydraulic press used to form metal. He was positioned within the sensing field of the light curtains. The machine cycled and crushed the machine operator between the ram and the collet die. There were 35 MIOSHA-covered deaths in 2019. 2009 saw the lowest number with 24. Every life is precious. Our mutual goal must be that every employee goes home at the end of every shift unharmed.
