New AAP Recommendation
The American Academyof Pediatrics recentlyreleased newrecommendations aboutpreventing the mostcommon chronic illnessin childhood: dentalcaries. Therecommendationsstates that all childrenshould start using fluoridated toothpaste as soon as thefirst tooth appears. Previous recommendations called foruse of fluoride toothpaste after 24 months, but this newedict calls for maximizing the benefits of fluoride at anearlier age.
The document advises parents to use a tiny "smear" orgrain of rice-sized amount of fluoride toothpaste on the firsttooth and to then increase the amount of toothpaste to pea-sized after age 3.
This news is important because it changes parenting andprimary care practices that have been in place for years. The recommendations also call for the application offluoride varnish in a primary care setting every three to sixmonths after the firth tooth erupts. Many very youngchildren do not receive dental care but do routinely visit thepediatrician. With these recommendations, pediatricianshave the opportunity to help prevent tooth decay.

Facing the Consequences
Dental practitioners do a goodjob reminding patients about theimportant habits that can helpkeep them free of tooth decayand gum disease - brushingregularly with fluoride toothpaste, flossing, and so on. It isimportant to focus on the goodhabits, but also educate themabout some of the bad habits
that can damage teeth and gums. And smoking is one ofthose bad habits. Gum (periodontal) disease is an infection of the tissue andbones that support your teeth. In severe cases, the boneand tissue that hold your teeth in place can break down, leading people to lose many or all of their teeth. TheCenters for Disease Control and Prevention (CDC) hasproduced new TV ads and videos that use personal storiesto demonstrate the link between smoking and gumdisease. The National Institutes of Health calls smoking "one of themost significant risk factors" for gum disease in the UnitedStates. Tobacco use in any form - cigarettes, pipes, andsmokeless (spit) tobacco - raises your risk for gumdisease.
Smoking weakens a person's immune system, whichmakes it harder to fight off a gum infection. Smokers aretwice as likely as a nonsmoker to have gum disease. Thelonger someone smokes, the greater his or her risk forgum disease. Treatments for gum disease may not workas well for people who smoke. If you know a friend, co-worker or family member whosmokes, let them know it's never too late to quit.
