The accident happened as a result of being hit by another BGP vehicle from behind at high speed. Driver of rear vehicle was driving recklessly, over speeding and not obeying convoy procedures. Driver of the rolled over vehicle sustained left knee cut, scratch on his forehead and redness on stomach while rear vehicle driver escaped unharmed. Both drivers had their seat belts on. ROOT CAUSES: Speeding, reckless driving and ignoring convoy procedure. Lack of convoy supervision. Lateral Learning Always adhere to rules and procedures. Always wear your seatbelt. LIFE SAVING RULES VIOLATION: Speeding Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction Seatbelts SAVE lives. 30/8/2010 Actual = Asset damage 3A Potential = Fatality

OnAug30that08:29hour, a Toyota Lc pickup driven by BGP vibe pusher rolled overtwice and landed on its wheels. The accident happened as a result of being hit byanother BGP vehicle from behind at high speed. Driver of rear vehicle was drivingrecklessly, over speeding and not obeying convoy procedures.
