INCIDENT

OnAug30that08:29hour, a Toyota Lc pickup driven by BGP vibe pusher rolled overtwice and landed on its wheels. The accident happened as a result of being hit byanother BGP vehicle from behind at high speed. Driver of rear vehicle was drivingrecklessly, over speeding and not obeying convoy procedures. Driver of the rolled overvehicle sustained left knee cut, scratch on his forehead and redness on stomach whilerear vehicle driver escaped unharmed. Both drivers had their seat belts on.

ROOT CAUSES:• Speeding, reckless driving and ignoring convoy procedure. • Lack of convoy supervision.
