Infant Mortality Reduction Initiative• Goal: Achieve infant vitality by reducing infant deaths by at least 15% over a 10 year period

• Geographic focus: Bootheel and St. Louis

• Community-driven

St. Louis Effort

Vision: Achieve a substantial and sustained reduction in infant mortality throughout the community

Collaborative Grants

• Purpose– Support alignment with FLOURISH St. Louis by

promoting organizational and systems-level work to advance infant mortality reduction efforts in the region

•Who can apply?– Organizations meeting eligibility criteria who

serve women, infants and families in St. Louis

Levels of Change for Infant Mortality

Planning Template

• Applicants may decide to target one or more levels of change. While both levels of change may be addressed, applicants are expected to focus most of their attention and resources on one level.This table is meant to serve only as an example. Do not enter any information into this table.

Award Information

• $1.5 million across St. Louis and the Bootheel

• Priority given to proposals that:– Reach underserved populations– Articulate a clear need for the intended

outcome(s)– Align with the mission of your organization– Describe reasonable activities that accomplish

the intended outcome(s)– Reflect an interest in engaging in collaborative

work– Present a budget that aligns with activities and

intended outcome(s)

Application Narrative

• Abstract– Summarize the request; these will go on the

website if funded

• Need– Use data

• Organization– How does the proposed project align with your

mission?

Application Narrative

• Target population– What are the needs of the target population as

it relates to infant mortality?

• Rationale for 2-year request

• Statement of success– What does success look like at the end of the

project?

Other Required Documents

• Planning template

• Partnership table

• Letter(s) of commitment from all partners listed in the table

• Signed commitment to engage

• Financial documents:– Budget worksheet and narrative– Required documents as outlined in the RFA• Fiscal agents must also submit these

documents

Evaluation Plan

• State up to three evaluation questions

• Identify data you will collect to answer the questions

• Questions should be tied to the intended outcome(s) stated in the planning template

•What do you hope to learn in the course of this project?

Sharing Learning with the Collective• IMRI Learning Community– Purpose: Provide opportunities for learning,

relationship building and reflection with peers to advance our collective work.

• Budget for:– One half-day in-person meeting (early

December)– One overnight convening that may require

hotel and travel costs

• Two year proposals should build in costs for each year of the grant
