Note ID: 051110180790001 General Note for Bridge Referenced Photos: "15", "16" AASHTO fatigue category E' welds exist at the ends of the interior girder partial length cover plates. The 1997 Bridge Inspection Manual, Appendix G requires that these welds receive a 100% hands-on inspection. The fatigue analysis was last completed on 4/14/2009 by W. Wong with a remaining safe life of 18 years. However, the fatigue welds were 100% hands-on inspected and no cracks were identifeid. NOTE: The concrete haunches on this bridge are not the type described in the 1997 Bridge Inspection Manual, Appendix G: Special Emphasis Inspection Requirement. The detail identified in Appendix G and TA 96-001 is for concrete haunches that extend past the edge of the top flange with the bottom face of the haunch flush with the bottom surface of the top flange. On this bridge, the bottom face of the concrete haunch is flush with the top of the girder top flange. The haunches on this bridge would not experience the type of failure described in Appendix G. However, in the past, hollow haunch concrete was identified at this bridge in the area where a bus fire occurred below deck. The haunches and deck concrete were sounded as appropriate. In addition to the areas identified in Safety Flag #02090026 that remain, several hollow areas were identified and marked with bright orange spray paint or red kiel. All of these conditions were included in Safety Flag #05110016.

Note ID: 05111018079000B General Note for Bridge Referenced Photos: "15", "16" The BIN Plate on the End, Left Wingwall is faded and not easily legible. This BIN Plate should be replaced.

Note ID: 05111018079000C General Note for Bridge Referenced Photos: "15", "16" This bridge was last inspected on 5/21/2009. The bridge inspection was fully completed 33 days early on 4/18/2011. A Repair Request was sent to Bridge Maintenance at that time concerning the condition of the end joint, however, on 8/16/2011, an e-mail from Highway Maintenance alerted Structures that the condition had worsened, and now impacted safety. On 8/19/2011, the condition was re-examined and the inspection re-opened to write Safety Flag #05110057, causing the inspection date to become 90 days overdue.

Note ID: 051110180790005 Beg Abut -- Wingwalls: Walls -- Rated 4, Was 4 End Abut -- Wingwalls: Walls -- Rated 4, Was 4 Referenced Photos: "4", "5", "6", "7" The wingwalls typically contain map cracks, as well as, vertical and horizontal cracks at all four corners of this bridge. At the Left Side of the begin and end abutments, the cracks are tight with dampness and some efflorescence, but the concrete sounds solid. The Left Side wingwalls would Rate "5". At the Right Side of the begin abutment, the cracks in the wingwall are up to 1/8" wide (up from 1/16" during the previous inspection) with a few hollow sounding areas on the

On the end Right wingwall, the cracks exist on the entire surface and are also up to 1/8" wide (up from 1/16" during the previous inspection). The Right Side wingwalls control the Rating for this Item.

Note ID: 051110180790002 Approaches: Pavement -- Rated 5, Was 5 Referenced Photos: "8" On the Left side of the begin approach pavement, the curb box of a catch basin is pushed back a distance of 3" on its begin side. The open space for the curb box is now exposed to bicycle traffic on the W/B NY 24 shoulder. This condition is a hazard to bicycle traffic. Safety Flag # 05110017 was issued as a part of this inspection to have the misaligned curb and curb box re- aligned as soon as possible.

Note ID: 051110180790006 Span 001 -- Deck Elements: Sidewalks, Fascias -- Rated 5, Was 5 Referenced Photos: "9" The Left Side fascia contains horizontal cracks over its entire length up to 3/32" wide, but the concrete sounds solid. The Right Side fascia was repaired with shotcrete near the end abutment. This area also sounds solid. The sidewalks are in fair condition and also Rate "5".

Note ID: 051110180790008 Span 001 -- Superstructure: Structural Deck -- Rated 4, Was 4 Referenced Photos: "10", "11" The underside of the structural deck contains map cracks, transverse cracks with efflorescence, patched areas, spalls with exposed rebar, and a honey combed area which is leaking onto the Girder G-2 bottom flange. This deterioration is contained on approximately 90% of the structural deck. Hollow sounding areas were discovered on the underside of the structural deck and were marked with bright orange spray paint or red kiel and included in Safety Flag #05110016. This flag also includes areas found during the previous inspection, and supersedes Safety Flag #02090026.

Note ID: 051110180790009 Span 001 -- Superstructure: Paint -- Rated 6, Was 7 Referenced Photos: "12" The paint is peeling from the undersides of many secondary members. A few areas on the bottoms of the girders are also peeling. These areas constitute only approx. 1% of the total paint area. The rest of the paint remains in excellent condition.

Note ID: 051110180790000 Span 001 -- Utilities: Lighting -- Rated 5, Was 1 Referenced Photos: "13", "14" The underbirdge light in Bay 1, next to intermediate diaphragm ID1 has been repaired since the previous inspection. At the Left Side of the end approach, the hand hole a the light pole is missing its cover. Insulated wires and wire nuts are exposed. The light pole is located adjacent to the sidewalk in front of Belmont Racetrack. This condition is a potential shock hazard to pedestrians and should be repaired as soon as possible. This condition was first reported in Safety Flag 05070017 during the 2007 inspection. Safety Flag #05110015 was issued as a part of this inspection to have this condition repaired as soon as possible. It should be noted that the two light poles to the west of this pole are similarly missing their covers with wires exposed. These Items are not rateable, however, they should also be repaired, and will be mentioned in the flag.

Note ID: 05111018079000A Span 001 -- Utilities: Sign Structure -- Rated 3, Was 1 Referenced Photos: "9" The previous report indicated that the correct clearance, 9'-5". During this inspection, this reading could not be reproduced. The clearance measured 10'-6". The Vertical Clearance signs are 1" conservative and are in excellent condition, however, they are not posted in the correct location.

Note ID: 051110180790003 End Abut -- Abutment: Joint with Deck -- Rated 4, Was 4 Referenced Photos: "1" The Span 1 Side end joint steel armoring and joint header are detached in places, and move in the vertical direction as traffic travels over them. In the westbound Right shoulder, the header and armor remain attached to each other, and are elevated 3/4" higher than the End Side of the joint. The Span 1 adjacent pavement is spalled as deep as the bottom of the header concrete. In the westbound Right Lane, the header and joint armor move separately. The armor bounces up to 2" above the pavement surface. The header has sunken up to 1-1/2" below the adjacent pavement level. The Span 1 adjacent pavement is spalled in almost the entire lane width. The movement of the header and armoring in the Right lane is beginning to affect the Right Side of the Center Lane. The Span 1 adjacent pavement is beginning to spall, and movement of the header and armoring is minor, but detectable. The deep spalls, and the movement of the header and joint armor pose a hazard to traffic. The joint condition was not as serious during the original complete inspection in April. There was little spalling, and movement was much less. A Repair Request was sent to Bridge Maintenance on 4/26/11 to have these conditions repaired. An August e-mail from Highway Maintenance alerted Structures that the condition had worsened significantly. The area was re- inspected on 8/19/11. The inspection report was re-opened, and Safety Flag #05110057 was

Note ID: 051110180790003 - continued issued to address the worsened condition. The Span 1 deck contains patched spalls adjacent to the joint in the westbound Right and Center Lanes. Some patches have already spalled and were re-patched. In all eastbound lanes, the deck and approach pavement are spalled adjacent to the end joint header with the greatest spall is in the Center Lane that measures 3" x 6' long.

Note ID: 051110180790004 End Abut -- Abutment: Seats and Pedestals -- Rated 4, Was 4 Referenced Photos: "2", "3" End abutment pedestal P2 contains a 7" x 4" x 24" spall with exposed corroded rebar on the Right Side and a 4" x 4.5" x 3" spall on the Left Side. Pedestal P3 contains a 6" x 7" x 3" spall on the Right Side and a 12" x 6" x 5.5" spall with exposed rebar on the Left Side. Pedestal P9 contains a 7" x 5" x 3" spall on the Right Side with exposed rebar. Pedestal P13 contains an 18" x 1.5" x 4" spall on the Right Side with exposed rebar. There is no loss of bearing support. The above conditions remain essentially unchanged since the previous inspection. In addition, Pedestal P14 contains a 7" x 3" x 2.5" spall on the Right Side with exposed rebar. The bridge seat also exhibits a few scattered spalls between the pedestals. The remaining pedestals would Rate "5". �

Description of Flagged Condition: Span No. 1 Component Structural Deck (as per Tp349/350) Rating 4 There are several areas of hollow-sounding concrete on the underside of the deck. We removed all loose material with a masons hammer during the biennial inspection. The following hollow areas that were identified in Safety Flag #02090026 remain: 1) A 6" x 6" area of deck haunch at the end of girder G1 over the end (east) abutment. 2) Three areas at the longitudinal median joint in Bay 7; the largest measuring 36" long x 4" wide. 3) One 21" length of hollow haunch on the Left Side of G15 near the end abutment. 4) Four hollow areas on the underside of the deck overhang on the Right Side near the end abutment. In addition to the areas identified in Safety Flag #02090026 that remain, several hollow areas on the Right Side of Bay 13 were identified and marked with bright orange spray paint. Additional areas adjacent to the longitudinal median joint in Bay 7 were identified during this inspection and marked with bright orange spray paint or red kiel. All of the above areas should be removed as soon as possible. Description of Flagged Condition: Span No. None Component End Joint With Deck (as per Tp349/350) Rating 1 The Span 1 Side of the end joint steel armoring and joint header are detached in places, and move in the vertical direction as traffic travels over them. In the westbound Right shoulder, the header and armor remain attached to each other, and are elevated 3/4" higher than the End Side of the joint. The Span 1 adjacent pavement is spalled as deep as the bottom of the header concrete. In the westbound Right Lane, the header and joint armor move separately. The armor bounces up to 2" above the pavement surface. The header has sunken up to 1-1/2" below the adjacent pavement level. The Span 1 adjacent pavement is spalled in almost the entire lane width. The movement of the header and armoring in the Right lane is beginning to affect the Right Side of the Center Lane. The Span 1 adjacent pavement is beginning to spall, and movement of the header and armoring is minor, but detectable. The deep spalls, and the movement of the header and joint armor pose a hazard to traffic.
