1. INTRODUCTIONThe inspection report is prepared and submission in respect to the Term of Reference (TOR) of Agreement between NATIONAL HIGHWAYS AUTHORITY OF INDIA, represented by its Chairman and having its principal offices at G-5 & 6, Sector 10, Dawrka, New Delhi – 110 075 and Stanley Consultants India Pvt. Ltd. The report includes Project background, Salient Feature of civil works and scope of Consulting Services. The report also covered resources mobilized by Consultants as well as Concessionaire and the perception of the Management and Service of Independent Engineer (IE) of the Project.
2. THE PROJECT:2.1. Background:
2.1.1 Client: M/s National Highway Authority of India (NHAI), PIU - Hajipur
The project envisages Four-Laning of Chhapra - Hajipur section of NH-19 from Km.143+200 to Km.207+200 in the State of Bihar under NHDP-III on (DBFOT) Annuity basis. 2.2. The scope of works is broadly summarized as follows:
Independent Consultant for the project - Four laning of Chhapra – Hajipur Section of NH-19 from Km 143+200 to Km. 209+940 for a length of 66.740 Kms (Existing Kms) in the State of Bihar under NHDP III on Design, Build, Operate and Transfer (DBFOT) Annuity Basis. In the above section Chhapra Bypass will be only Two Lanes. The above scope also includes all design, engineering, financing, procurement, construction, completion & maintenance of the above stretch for the entire duration of concession period i.e. 15 years. 2.3. The Concessionaire and EPC Contractor:
The Project comprises Construction of 4 lane road, two lane & service roads at four locations has been entrusted by M/s National Highway Authority of India (NHAI) under a Concessionary Agreement to M/s Chhapra - Hajipur Expressways limited.M/s Chhapra - Hajipur Expressways limited have appointed M/s Madhucon Project limited (MPL) as the EPC Contractor who is planning and executing the work partially through contractors and the balance themselves.
3.1. Right of Way (ROW):
The proposed Right of Way from Km.143+200 to Km.209+940 is 60 meter
3.2. Carriageway:
The paved carriageway shall be 17.50 meter wide excluding the median except Chhapra Bypass which is 10m in width.

IC’S COMMENTS
A) IC’s Comments on the MPR of June 2014 submitted by the Concessionaire.
1) The S-curve show that the cumulative physical progress is 74.56% .It is also not substantiated with facts and figures. In the month of June-14 the progress was only 0.2 % has been done by the concessionaire.
2) As per MPR June-2014 concessionaire has submitted 15 km of Hindrance/Obstructed length, but by the joint verification it is only 14 Km.
3) In the manpower section the concessionaire has reported as follows-:

B) IC’s overview on quality of work and material used and their sources.1) WMM Plant is not at all working and WMM mix is produced manually. This is not in conformity with the procedure laid down is the QAM Para no-5.3 submitted by the concessionaire HM plant at Nayagaon and Batching Plant at Dighwara are working without printer. So it is difficult to keep a check on the quality.
2) This is no summery sheet for test report from QC department.F C) IC’s overview on safety on construction.
The NHAI’s Safety Audit team namely Moti MacDonald had visited the site on Last week of March- 2014 and had given their comments which the concessionaire is still to adopt.
D) IC’s view on lapses, defects or deficiency.
The Construction of R.E wall being carried out at CH -207+000 to 207+200 and 208+739 to 209+000 is more than 6 meters high which is not in conformity with CL .no. 7.15.1 of SP-84.

The Concessionaire, CHHAPRA - HAJIPUR EXPRESSWAYS LIMITED, has appointed Madhucon Projects Limited to execute the Chhapra – Hajipur Section of NH-19 Starting from Km. 143+200 to Km. 209+940 in the State of Bihar. Lab & Equipments:One lab is established near Gandak Bridge, Sonpur site camp (Ch: 205+650) LHS and mobilization of equipments is in progress.
