›› Login using your User Name and Password
You will momentarily see a screen that says:
Leaving CareFirst Administrators
You are now leaving the CareFirst
Administrators web site. The information you
are accessing is provided by our vendor.
›› Select “Health Ticket” on the left-hand side of
the screen
›› Enter the following information:
Member Identification Number
Member Last Name
Member Zip Code
›› Click “Sign In”

›› Select “Create an In-Network Health Ticket” ›› A Health Ticket will be created. You may print
or save the Health Ticket.

Getting Started ›› Please have the following information handy:
Provider Tax ID Number, City, State & Zip,
Contact Name.
›› Go to: www.cfablue.com. Be sure to enable
your browser to allow pop-ups.
›› Select “Register Now” located under MY CFA
Account on the right hand side of the page.
You will momentarily see a screen that says:

Administrators web site. The information you
are accessing is provided by our vendor.
›› Enter your chosen user name and the email
address you would like to have on file.
›› Select “I am a provider” and then click “Next”
›› You will be asked to verify your identity.
Use UPPERCASE letters when entering this
information
›› To confirm your registration, please send an
email to [email protected].
›› Include your chosen user name, Tax
ID Number and subject line “Provider
Registration Confirmation Request”
›› You will receive a temporary password within
48 hours (excluding weekends).
