This banner is repeated on each subsequent page.Banner at bottom of page is a timeline of the patientʼs life, scaled to show more recent events in greater detail than earlier ones. The present day is kept in considerable detail, clear enough to list events down to the half-hour if necessary. Clicking on any time period will adjust the scaling to show that period more clearly.Crossing the timeline horizontally is a series of lines representing various possibilities A green line would represent the patients history of prescribed medicines with small dots indicating each instance of a regime starting or ending. Clicking a line will bring up the full details of the related history. Clicking a dot will bring up the details of that encounter and subsequent adjustments or completion of treatment.Centered on the form are images of the upper and lower teeth. On these will appear patients missing teeth, dentures, braces, etc. Clicking on any tooth will bring up the page associated with the mouth, if a holistic issue, or the tooth, if local.

On the sides of the form are four vertically oriented banners giving cursory information on the patients history of test/lab results, medications, diseases, alternative treatments and consumptions pertinent to medical analysis. When a patient has a disease or any other ailment those items affected will be highlighted on the body and surrounding indicators; so a trained professional can see at a glance the general medical picture of this particular human being. For example a person missing a limb will have that limb replaced by a dotted line.Clicking on any highlighted part of the body will bring up a page specific to that body part, excellent for emergency workers (see pg. 3).Front of Vitruvian man image will show driverʼs license or other photo-identification, updated as the patient updates their identifications. Clicking on the photo shows all previous photos. Overlaying the whole is the outline of a hand. To access the record the patient places their hand within the outline. In emergency situations, a professional may input an identity code or professional license to override the patient security. This action will be immediately reported to the local police department.

When these reports start at birth, a baby footprint is used until hand matures enough. Each health encounter will have its own record and all associated information will be stored. All accessible through this design. Each medication, immunization, treatment, surgery, check-up, and life event available information. Eventually the patient will have a chip implanted at birth recording their full history in a universal format. All actions affecting the body will be recorded automatically; every calorie consumed and expelled, every cigarette smoked, every ejaculation, every heartbeat. Primary Care Physicians will log on remotely and return to the record a new entry with their diagnoses and reports, immediately transmitting to the patient all pertinent information. Eventually, the system outlined will be integrated to immediately and constantly run tests internally and manufacture treatments and repair damages entirely within the body itself.
