30 years prevention in private dental office – a win-win situation
Prevention is cause-related therapy.No filling, no crown, no implant can cure the diseases caries and periodontitis.only successful biofilm fighting can make or maintain our patients healthy.This is provided by instructing our patients in efficient home care and professional tooth cleaning by a dental hygienist in needs-related intervals.calculation of the economic outcome shows that the dentist can make his living on an organized preventive office.
Prevention turns out to be a true win-win situation. it gives benefit to the patient first, the dentist is happy and even the social systems profit from it.a review after 30 years!

Hidden caries – the new challenge / How to detect – how to treat
in recent decades epidemiological studies have shown a general drop in the caries prevalence together with a concentration of lesions in the pits and fissures of permanent molars of children and young adults in many industrialized countries. While progression of caries lesions generally appears to slow down with increasing age, the paediatric dentist will diagnose young patients and adolescents with non-cavitated caries lesions more frequently. Therefore, early detection, correct diagnosis and monitoring of those lesions are key targets in the overall effort to move away from operative towards non-
operative preventive dentistry. reflecting basic requirements of additional caries detection and diagnostic methods, e.g. validity, reproducibility and clinical practicability, the presentation will provide an update about the clinical usefulness of available tools. Beside this general overview the presentation will give further a short introduction to the new DiagNocam technology. The main advantage of this method has to be seen in the possibility to capture diagnostic images from proximal spaces without ionising radiation. The device is using near-infrared light (>750nm) to illuminate the tooth crown through gingiva and alveolar bone. Such images provide detailed information about the caries extension especially on proximal sites and could help to reduce the need of bitewing radiographs.

Traumatic dental injuries are relatively frequent accidents that typically involve teeth in the maxillary anterior segment. The aim of this lecture is to present current treatment guidelines of the various injuries in the permanent dentition. The clinical approaches and their rationale as well as possible (long term) complications will be presented.

Traumatic influence to the facial skull causes in most cases loss of teeth and more or less also severe loss of bony structures of the alveolar ridge.even in the aesthetic zone the goal is not only to rehabilitate the chewing function, the aesthetic restoration of an ideal appearance
of the “red and white” structures is of high importance.our concept for restoration of bony structures is only based on the use of autologous bone material, receiving a biological ideal bone bed. The mouth area presents several locations in order to harvest cancellous bone or bone blocks for sufficient augmentations in defect areas. after augmentative procedures in one or two steps it is possible to integrate adequate implants for fixed prosthetic structures. The new aspects using implant bodies made of circondioxid shall be pointed out with the potency of perfect aesthetic outcome.

Erosion and more. New technologies and materials in the restoration of the worn dentition
erosion-induced loss of tooth structure in young patients represent an international ever increasing problem and is often associated with complex rehabilitations. These rehabili-tations represent a particular challenge for the restorative team, since the vertical dimen-sion of occlusion (VDo) needs to be reconstructed or redefined, as well as a minimum-invasive approach is of primary interest for the longevity of the abutment teeth.
The use of temporary acrylic or composite materials allows to evaluate the treatment objective over a certain period of time and therefore generates a high predictability of the final rehabilitation in terms of aesthetics and function. caD/cam technology enables the use of prefabricated polymer materials, which are fabricated under industrial conditions to form a highly homogeneous structure compared to those of direct fabrication. This increases long-term stability, biocompatibility and resistance to wear. Furthermore, they offer more suitable caD/cam processing characteristics and can be used in thinner thicknesses compared to ceramic restorative materials. also, based on the improved long-term stability, the transfer into the final resto-ration can be divided into multiple treatment steps. The lecture presents different clinical cases with minimal-invasive indications for caD/cam-fabricated temporary restorations for the pre-treatment of complex cases.

Primary oral health care is not of major priority in non-established market economy (non-eme) countries due to lack of infrastructure and professional manpower, and the most common dental treatment is tooth extraction. established dental restorative treat-ment options appear out of reach in most cases. By the introduction of the atraumatic restorative Treatment (arT), dental caries treatment became manageable for non-eme countries by delegation of arT to specially trained nurses. The speech reviews pre-con-
ditions and the development of a primary oral health care concept for rural areas of the gambia as a potential example for other West african areas.
