The North East Independent School District Incentive Program, Healthy Living Rewards, pays dividends. We teamed up with Blue Cross and Blue Shield of Texas (BCBSTX) to provide healthy rewards.

Who can join and how much can be earned?Anyone covered under the North East Independent School District plan administered by BCBSTX can join the program.

• Employees only are eligible for the Health Assessment reward.

• Members must download and sign up for the Ovia Pregnancy app during the first or second trimester of pregnancy to be eligible for the reward.

• Each incentive can only be earned once in 2020 and rewards will be distributed quarterly.

Check your health status!Find out how your health measures up by taking a Health Assessment.2 Answer a few questions about your health and lifestyle. Then, get suggested programs that can help you get and stay healthier.

How do I complete a Health Assessment?• Gather as many of the following personal details as possible:

- Current height and weight

- Systolic blood pressure (top number) and diastolic blood pressure (bottom number)

- Total cholesterol level

- HDL cholesterol level

- Triglyceride level

- Blood sugar level

- Waist measurement in inches

• Go to wellontarget.com. • Log in using your Blue Access for MembersSM username

and password or click “Register Now” to create an account.

• Click “Start” in the “Health Assessment” box at the top of your dashboard.

Get help managing your diabetesLiving with Diabetes, through Well onTarget®, is an interactive six-week, online self-management program to help you develop and maintain healthy habits. The program is based on the Center for Disease Control’s Diabetes Prevention curriculum and addresses lifestyle factors related to diet, physical activity, sleep and regular medical care. You get a certificate of completion for successfully finishing the program. To register, go to wellontarget.com or the Well onTarget app, AlwaysOn®.3

Get ready for baby If you plan to add to your family, the Ovia Pregnancy app from Ovia Health® can guide you step-by-step through pregnancy.

With the Ovia Pregnancy app, you can:

• Watch your baby grow week by week and learn about their development• Read personalized articles about your pregnancy and relevant health plan benefits• Receive instant feedback and get support from a registered nurse coachDownload the Ovia Pregnancy app. Select BCBSTX as your health plan and North East Independent School District as your employer for full access to your full maternity benefits. After delivery, download the Ovia Parenting app and complete the Edinburgh assessment to earn the reward.

Get the support you need with wellness coaching Well onTarget’s wellness coaching gives you the support you need to be your best. Work with a wellness coach over the phone or by exchanging secure messages online at wellontarget.com or through the AlwaysOn app. Wellness coaching uses evidence-based guidelines and proven techniques of motivation and goal setting. Complete one of the three programs below in order to receive the incentive:
