    TRANSITION PLANStudent Name: Doug Smith

    I. IDENTIFY THE STUDENTS POSTSCHOOL VISIONDoug is currently ending his sophomore year at Berkeley High School. He will besoon acquiring the life skills necessary for him to be living with a roommate oncehe graduates high school and obtains a job. These skills include paying forgroceries and rent, working the public transportation system, as well as laundryand cooking skills. Doug is also looking into several different fields for jobs,including places such as the local Pet Grooming Shop, Subway, at the local fitnesscenter, and WalMart. Depending on the job he chooses, Doug may look intotaking some courses at the community college based around his job choice. Thesemay include courses in the following subjects: animal science, business, kinesiologyand communication.II. IDENTIFY THE STUDENTS TRANSITION GOALS

    1. Career and Economic Self Sufficiency

    1.1 Employment Goal:1. Doug will job shadow at the following sites: a local Pet Grooming Shop,

    Subway, the local Fitness Center, and WalMart. Doug will do this byspending three two-hour sessions for two weeks for each site. Doug will thenrank his preference of career opportunities after the completion of the jobshadowing experience.

    2. Doug will participate in an internship for the job site of his choice at eitherthe local Pet Grooming Shop, Subway, the local fitness center, or WalMart.

    3. Upon graduating high school, Doug will enter the competitive employmentfield for the job of his choosing in which he has gained experience inthroughout high school.

    4. Doug will enroll in an animal studies class at his high school for onesemester to learn more about different animals and their behaviors. Thiswill help prepare him for working at the Pet Grooming Shop.

    5. Doug will enroll in a nutrition class at the high school level for onesemester. This will help him learn about a healthy diet and how to make ameal that tastes good while it is still healthy for you. This will prepare himfor working at Subway.

    6. Doug will enroll in a fitness class at the high school level for two semesters.This will help him learn about different fitness activities and how to use theequipment that can be found in most fitness centers. He will also learn howto teach others how to use this equipment. This will further prepare himfor working at a local fitness center.

    7. Doug will enrolled in a communications course where he will learn thebasics of communication (social skills): such as when it is and is notappropriate to interrupt a conversation, how to effectively express himself,and when it is and is not appropriate to be communicating on the job aswell as appropriate topics of conversation. This will help Doug by teachingproper socialization techniques that will assist him in his job.

    1.2 Postsecondary Education Goal:1. After graduating high school, Doug will enroll in at least one course related

    to his work field. This could be animal science/psychology (Pet Grooming),cooking/nutrition class (Subway), Communication course (WalMart), or afitness course (working at a fitness center).

    2. Doug will enroll in a course at the community college to refresh his memoryon time and money management. This will help him balance his dailyschedule and the money he earns for groceries and personal spending.

    1.3 Financial/Income Needs Goal:1. Doug will be enrolled in an economics course in his junior year of high

    school that will assist him in financial responsibility, such as how to balancea checkbook as well as write checks, pay bills, and how to live on a budget.

    2. Upon graduating high school, Doug will enter the competitive employmentfield for the job of his choosing in which he has gained experience inthroughout high school. By obtaining this job, Doug will be earning anincome and will be responsible for paying his bills as well as for groceriesand other living essentials.

    2. Community Integration and Participation

    2.1 Independent Living Goal:1. Doug will learn how to use a washing machine and dryer by doing the towel

    laundry for the school pool and fitness center.2. Doug will enroll in a cooking course at the high school level in order to

    learn basic recipes and how to utilize kitchen appliances.3. Doug will enroll in a community living course at the high school level that

    will focus on living within the community and what to do in specifichousehold situations such as power outages, tornadoes, fires, being lockedout of the house, burglar, dealing with roommate situations, etc.

    4. Doug will enrolled in a communications course where he will learn thebasics of communication (social skills): such as when it is and is notappropriate to interrupt a conversation, how to effectively express himself,and when it is and is not appropriate to be communicating on the job aswell as appropriate topics of conversation. This will help him when livingwith a roommate and dealing with different types of social situations.

    5. Doug will enroll in a course at the community college to refresh hismemory on time and money management. This will help him balance hisdaily schedule and the money he earns for groceries and personal spending.This will help him when living on his own.

    6. After graduating from high school, Doug will seek to live in the location of his choice with or without a roommate (again, his choice). He will be ableto complete all daily living skills independently with a specialist coming into check on him once a day.

    3.2 Self-determination Goal:1. Doug will advocate for himself by ranking the jobs he has shadowed in

    preference order. After doing so, he will explore internships in those jobs.2. Doug will attend the remaining of his high school IEP meetings in order to

    advocate for himself by stating his wants and desires for post-schooloutcomes as well as any concerns he has thus far with his job and post-school preparation.

    III. STUDENTS COURSE OF STUDYDoug will remain in his life skills curriculum classes where he will learn basicsocial communication skills such as when it is and is not appropriate to interrupt aconversation, how to effectively express himself, and when it is and is notappropriate to be communicating on the job as well as appropriate topics of conversation. This will help Doug by teaching proper socialization techniques thatwill assist him in his job. Doug will also learn how to practice self-hygiene andcare, such as brushing his own teeth, using dental floss, showering, shaving, andother self-preparation activities. Doug will also learn how to sort, wash, and dryhis own laundry as well as how to read tags on clothing to know how he should bewashing them. In these life skills courses he will also learn how to wash dishes anddo basic cleaning skills such as cleaning windows, sweeping, washing dishes,vacuuming, and wiping down surfaces. In these life skills courses Doug will alsoenroll in a community living course at the high school level that will focus on livingwithin the community and what to do in specific household situations such aspower outages, tornadoes, fires, being locked out of the house, burglar, dealingwith roommate situations, etc. This will all better prepare Doug for living on hisown and within the community.

    Doug will enroll in an animal studies class at his high school for one semester tolearn more about different animals and their behaviors. This will help preparehim for working at the Pet Grooming Shop. He will also enroll in a nutrition classat the high school level for one semester. This will help him learn about a healthydiet and how to make a meal that tastes good while it is still healthy for you. Thiswill prepare him for working at Subway and making healthy choices when livingon his own. In addition to this, Doug will be enrolled in a cooking course with hissame-aged peers to learn basic recipes and how to operate different kitchenappliances. Doug will also enroll in a fitness class at the high school level for twosemesters. This will help him learn about different fitness activities and how to usethe equipment that can be found in most fitness centers. He will also learn how toteach others how to use this equipment. This will further prepare him for workingat a local fitness center and help maintain a healthy lifestyle.     2. Related Services

    Doug will be receiving related services from his jobcoach, who will be implementing much of his jobshadowing instruction and supplementing anyinstruction he learns in class related to his job.

    Dougs case manager will also be in charge of related services for him to ensure that he isreceiving all of the services that he should be andchecking in on his progress within all areas.

    3. Community ExperiencesJob Shadowing

    Doug will be shadowing jobs at the followinglocations two hours, three days a week for a total of two weeks:

    a) Working a pet grooming shop where he willbe in charge of scheduling appointments,calling customers to state their pet is ready,taking payments, and playing with theanimals.

    b) Working at WalMart as a greeter forcustomers who walk in. He may also be

    If Doug is not satisfied with any of theselocations, further job shadowing will occur tofind the right fit for him.4. Employment

    Doug will participate in an internship with thecareer of his choice, ideally at the pet groomingshop, WalMart, the fitness center, or at Subway.He will work at the location of his choice for oneschool year.

    5. Adult Living and Community Participation

    The special education teacher will teach Doug howto wash clothes as well as basic daily living andself-hygiene and cleaning skills within the life skillsclass setting. In this setting he will also learn howto effectively communicate with others within thecommunity.

    Doug will also learn how to properly use the publictransportation system as well as teach him how toapply to different recreational programs in thecommunity. Dougs job coach will also instruct himon using the public transportation system to takethe bus to and from each of his work sites.

    Doug will also receive instruction for living in thecommunity through his nutrition, cooking, fitness,economics, resume-building courses.
