– Has been useful in many cases

– Recent experience is leading us away from this approach and into a different paradigm

– Assay development is more complicated than the three broad buckets

Why?• Because broad buckets do not tend to work for us in the

translational and early development space (one size does not fit all)

• Hinders the scientific process of decision making and accountability and inadvertently disengages the scientist’s brain

• Exploratory is a catch all for biomarkers whatever their purpose, from as broad as potentially useful development support with no clear decisions to making clear internal decisions including dose decisions.

– A: Type of Biomarker (Target engagement, distal/proximal pharmacodynamics, safety, disease, patient selection etc) and stage of development

– B: How data will be used and who will be using it. (making dose decisions, exploring potential treatment effects, support for hypothesis driven questions, regulatory submission)

– C: What type of data is expected (mass units, change from baseline, percent control, cut-off assessment, large/small delta etc)

– D: Basic assay challenges (normal/disease levels, matrix, endogenous available, assay available etc)

How much risk is the programme team willing to take?

– Typically in early development: Preclinical to FTIH– Internal decisions (often described as exploratory)

– Essential data to drive programme progression (candidate selection)– Data used to make dose decisions/recommendations– Actual mass units used to develop models – May be used to make dose escalation decisions– Needs to be sufficiently robust to give relatively accurate measurements that are consistent through

preclinical and dose escalation (FTIH) in humans

And the final word

– Case study steering us towards a CoU driven approach and greater stakeholder involvement prior to development

– A great learning experience

– Rewarding interactions with the biologists and the modeller: More satisfactory understanding of data, encouraging a no blame culture and be true scientists

– Still evolving dev/validation strategies

– Sometimes the unexpected happens
