ผปวยหญงไทย อาย 84 ป Underlying DM, HT, Bed Ridden มาดวยไข หานใจเหนอย แพทยม order ให Antibiotic Vancomycin drip ทก 12 ช วโมง พยาบาล Ward ไมสามารถทางเสนไดหลงจากพยายามแลว 2 ครง จงขอความชวยเหลอจาก ICU Nurse หลงจากพยายาม 2 ครง สามารถ access ได บรเวณ หนาแขงขวา ….. 2 วนผานไปพบ phlebitis grade 2 จง off IV site ผปวยกลบบานได 2 วน โทรศพทมาปรกษาแพทยวา บรเวณทแทง IV บรเวณหนาแขงเปนหนอง

Stage 1 Skin blanched, Edema less than 1 inch in any direction, Cool to touch, with

or withoutpain

Stage 2 Skin blanched, Edema 1 to 6 inches in any direction, Cool to touch, with or

without Pain

Stage 3 Skin blanched, translucent, gross edema greater than 6 inches in any

direction, Cool to touch mild to moderate pain, possible numbness

Stage 4 Skin blanched, translucent, skin tight, breaking, skin discolored, bruised,

swollen, gross edema greater than 6 inches in any direction, Deep pitting tissue edema, Circulatory impairment, Moderate to severe pain.

Remark: Stage 4 also includes the infiltration of any amount of blood product,

irritant, or vesicant

Vesicant drug should be given first with slow IV push via the site tube of running infusion

Irritant drug should be sufficient diluted

Inform patient to report any sensation of burning or pain at the infusion site.
