Hospital only = can only be prescribed within the hospital. NICE guidance should be followed,

where available. A maximum of 3 months supply will be issued on an outpatient prescription

Specialist initiation required = medication initiated by the hospital. Prescribing and monitoring may

be transferred to the GP in accordance with approved Specialist initiation required guidelines. A

maximum of 3 months supply will be issued on an outpatient prescription.

Blank status = can be prescribed both in hospital and by GPs. A maximum of 1 month supply will

be issued on an outpatient prescription. For non urgent and repeat items complete a 'Request for

Non-Urgent Prescription' form and give to the patient to take to their GP who will write a

prescription.

Items not listed in the hospital formulary should not be prescribed. Requests for new items should

be made to the Drugs and Therapeutics Committee.
