COMMUNICATION & RELATIONSHIPS

The post holder will:

• Provide and receive complex difficult and sensitive information, which may be in a challenging and emotive atmosphere.

• Communicate and collaborate with clinical nurse lead, ward manager, peers,

medical staff, patients, families and carers, general public, external agencies, voluntary agencies, GP's, other regional NHS Trusts.

The post holder will: • Complete 5 day basic Prevention and Management of violence and aggression

training • Have and demonstrate standard basic keyboard skills

• Implement safe moving and handling techniques

• Meet the physical requirements of the post subject to any requirements for the

Trust to make reasonable adjustments for the post holder.

PATIENT CARE AND FAMILY INVOLVEMENT

The post holder will:

• Deliver and maintain high standards of clinical care and contribute to achieving the Care Quality Commission Essential Standards. They will identify and act on any circumstances or practices which may compromise standards, quality of care and/or patient safety and report to the ward manager.

• Advocate and maintain a philosophy of care which is person/carer centred,

maintaining dignity and independence and consistently right for that person at that time

• Implement care delivery systems, ensuring contemporary clinical practice is

outcome focussed and of the highest professional standard and consistency

• Devise, implement and evaluate individual plans of care based on a person centred assessment.

The post holder will:

• Undertake surveys and audits in own work area.

Effort and Environment

Physical: • Push/ pull trolleys/ wheelchairs • Assist service users with personal care (i.e. dressing, hygiene, toilet needs) involving bending, kneeling, and twisting • Facilitate patient movement around the ward within Trust moving and handling guidelines. Transferring and maneuvering service users into positions for personal care and treatments several times each duty • Use physical intervention skills including restraint of service users following the

Trust training guidelines and policies Mental:

� Concentrate on service user treatment and care. • Concentration required when checking documents/ service user notes and administering medication, whilst subject to frequent interruptions from service user / relatives/ team members/ phone calls • Concentrate whilst maintaining service user observation levels according to Trust policy • Service user behaviour can be unpredictable requiring immediate change to activity • Coordinate and chair Care Program Approach reviews with a multi disciplinary team setting up to 4 times per week which can last up to 3 hours • Prepare reports for and attend other meetings as required e.g. Mental Health Act Review Tribunal or Managers Hearings

Emotional: • Deal with distressed/ anxious/ worried/ upset/ angry service users and relatives • Occasionally have to care for terminally ill service users • Due to nature of service user group, can be expected to deal with severely challenging behaviour • Will be expected to impart unwelcome news � Provide emotional support to service users and carers.

Working Conditions:

• Regular exposure to body fluids • Verbal aggression from service users can occur most days, several times per shift, depending on service user group • Physical aggression from service users is a potential hazard because of unpredictable nature of service user group. • The post holder may be required to work within different areas across the Trust. • Exposure to constant and excessive noise.

• To provide, under the direction of a Care Coordinator and or Pathway Clinical Lead, effective treatment and intervention to service users with complex and substantial needs.

PHYSICAL SKILLS

The post holder will:

• Demonstrate basic IT skills. • Sometimes be required to transport equipment to various sites in the locality. • Be required to carry out physical health care tasks, for example administration of

intramuscular injections, checking blood pressure, glucose monitoring. • Be appropriately trained and capable of using authorised breakaway techniques as

required.

PATIENT CARE AND FAMILY INVOLVMENT

The post holder will:

• Provide clinical evidenced based interventions, as detailed in the Care Plan and determined by the Care Coordinator, to service users with complex and substantial needs, under the direct supervision of the Pathway Clinical Lead or Care Coordinator.

• Offer clinical advice and information to service users, carers, other professional disciplines, voluntary organisations and the public.

• To engage in active relapse prevention planning with service users and carers, contributing to their recovery and discharge planning.

• Contribute to the service user’s transition across all services provided by the Trust and adhere to standard work procedures.

• Administer medications (oral and intramuscular) when required and will also monitor for therapeutic responses to medications and potential side effects in a variety of environments, e.g. service user’s home, and liaise with the prescriber (Consultant Psychiatrist or GP as appropriate) to ensure optimum outcome for service users.

• Recognise and anticipate situations that may be detrimental to the Health and wellbeing of service users and their carers and advise and treat on the promotion of health and the prevention of physical/mental and behavioral deterioration.

• Support service users to contribute to their own individualised Care Plans, and Wellness Recovery Action Plans (WRAP) / other Recovery Plans in relation to their Recovery and discharge planning.

• Use Digital Recording Equipment and mobile technology as per Trust Policy • To ensure Carer needs are considered and Carers are involved in Care Planning

as appropriate.

The post holder will:

• Be responsible, via paper or electronic systems, for maintaining accurate and contemporaneous records and securely storing and retrieving necessary information/documentation as per Trust Policy.

• Devise, evaluating, and maintain Care Plans, Risk Assessments and CPA Reviews as the Lead Nurse, within recorded time frames, as per Trust Quality Priorities and Policy.

The post holder will be:

• Expected to deal with frequent interruptions due to the unpredictability of the work and the service user group.

• Frequently required to observe service users for long periods of time during interviews, assessments and treatments of service users.

The post holder will be:

• Required to frequently work in areas not subject to health and safety regulations e.g. service user’s homes with exposure to unpleasant working conditions such as dirt, dust, smells or bodily fluids.

• Exposed to potential verbal and physical aggression from both service users and carer.

• Be required to work at various locations throughout the Trust. To use mobile technology provided by the Trust to enable mobile working, including from home as per Trust Policy.
