• Hasmoderatetosevere“TransitionNeeds”identi-fiedontheirIEP(IndividualizedEducationPlan).

• Hascompletedcourseworkattheirhighschool.

• Isinterestedininteractingwithinthecommunity.

Community Connections students ...• AreeligiblefortransportationtoandfromCommunityConnectionsthroughPSDTransportationServices.

• Attendtheprogram2½dayseachweek,butmayvarydependingonstudentneeds.

• Receiveadaptiveandlifemanagementtoolswhichmayincludeplanner,résuméandotherlifetools.

• Pay$25forabuspassand$5foraParks&Recre-ationpass.Inaddition,incidentalmoneyisneededforrecreationandleisureactivities.

• Uponcompletionoftheprogram,receiveaCertifi-cate of CompletionoraPSDgraduationdiplomaifgraduationrequirementshavebeenmet.

• Musthaveastate-issuedID.

Studentslearnskillsthroughparticipatinginavarietyofvolunteeropportunitiesthroughoutthecommunity.

VolunteeropportunitiesincludeMealsonWheels,PoudreRiverLibraries,HabitatforHumanity,theFoodBank,HELPInternational,AnimalHouse,etc.

Inaddition,internshipsareavailableatColoradoStateUniversityincludingthemailroomandgardens,andthroughlocalbusi-nessessuchasAlbertsons,Jax,Marriott,TacoBell,etc.

Wehelpstudentslearnaboutindependentlife-stylechoicesbasedontheirstrengths,interestsandpreferences.

Whethertheygoontoliveontheirownornot,studentslearnskillstoincreaseindependence,makefriends,andbeactiveinthecommunity.Inadditiontoallofthis,studentsandstaffatCommunityConnectionslovetohavefun!

Transition is a process of gradually increasing independence and becoming a member of our local community!

Transition ServicesStudentsintheCommunityConnectionsprogramtransitionintotheirnextphaseoflifebygraduallyin-creasingresponsibilityinordertogainindependence.Servicesstudentsreceiveinclude:

• Buildingdailylivingskillssuchasfoodplanning,shoppingandmoneymanagement

• Ridingpublictransportation

• Developingorganizationalskills,includingkeepingaweeklyschedule

• Traininginwork-relatedskillsandjobcoaching

• Developingfriendships

UponcompletionoftheCommunityConnectionsProgram,studentsreceiveaCertificate of Comple-tionandcelebratetheirtransitionthroughaSend-off GraduationfromCommunityConnections.
