Affordable Housing Referral Form

• Purpose

– Initial project review and screening for entitlement need

– Coordination and referral (communication) between City Planning and LAHD, CRA, LA County, and other agencies for funding purposes

– Referral to Planning Public Counter(s) for entitlement filing purposes

• Review and signature

– Applicant to complete form as much as possible

– Planning staff to review form and assess need for any entitlements (based on information provided)

– Planning staff to sign and date form as verification of review

– Applicant to provide form with Planner’s signature to LAHD (or other City/County agency) as required for application purposes.

• In order for Planning to review and sign AHRF and any other form(s) required for funding purposes, applicant must:

– Complete form(s) to the extent possible

– Provide a hard copy of all forms requiring signature as well as email an electronic version of all forms to be completed.

– Provide hard copy of reduced set (11”x17”) project plans (site plan, floor plans, elevations, and sections as necessary)

– Provide copies of all relevant zoning information including: B&S Zoning Verification letters, all previous entitlements/ decisions, status of currently filed applications, Q-conditions, Proof of exemption from SPR, certificate of occupancy for existing units, etc.

– Appointments will be scheduled on an as needed basis only

• Planning review and signature timeline

– All applicants shall provide to Planning all information necessary for project review (as discussed earlier) no later than 4:00 pm 4/30/09. Drop-off in City Hall Room 701

– 5/1/09-5/8/09 Planning staff to review all packages submitted for completeness and do follow-ups or schedule appointments if necessary.

– Planning staff to complete review and sign requested forms by 5/11/09 9:00am
