    Center. Since his acility became pa

    providing patients an easy-to-ollowlie-changing protocol, his acility

    has also improved documentation

    and tracking. And hes improved

    his cholesterol by ollowing the

    preventive protocol himsel.

    When Joni had a cardiacincident she was immediately able

    to get specialty care at her Kaiser

    Permanente acility. This incident

    made her even more dedicated to

    the work shes doing as part o the

    Community Beneft-unded Specialty

    Care Initiative to get access to

    outstanding cardiologists or the

    poor and uninsured.

    As the mother o our children,

    Raquel didnt have time to besidelined with an umbilical hernia.

    She had to keep going, in spite o

    the pain she experienced, which

    was orcing her to take lots o

    painkillers. In April 2008, she took

    part in a surgery day at KaiserPermanente, coordinated by Access

    OC. Her hernia was repaired at no

    cost and now shes enjoying living

    her lie pain ree.

    careWe know how important having a medical home is to maintaining optimal health. In all o our care-relatedeorts, this is our goal to make sure the uninsured, underinsured, and underserved in our communities haveaccess to consistent, quality care that a stable medical home oers.

    In 2008, our medical fnancial assistance and charitable coverage programs

    served more than 200,000 people in Southern Caliornia.

    In addition to the direct care we provide to people in need through Kaiser Permanente emergency rooms, hospitals, and medical oces,we also work closely with a saety net o community clinics, public health departments, public hospitals, and other organizations to und

    Our primary objective through these assistance programs is to broaden

    access to those who need a medical home. We want to provide a placewhere they can consistently get preventive care and where they will

    have access to the critical specialty care that they need. Because Kaiser

    Permanente is an integrated health care system, we have an advantage

    in making this happen everything is available in one place.

    Strengthening the

    saety netUninsured and underinsured patientsdont have many options when itcomes to medical care. Kaiser Permanente Community Beneft is working

    to change that in Southern Caliornia. We do this in three key ways:

    Facilitating dialogue and improvements in the system

    Funding programs that provide the necessary care

    Sharing experience and expertise

    Whether we help community medical acilities,

    put proven protocols in place with programs

    like Vital Signs, or enable homeless

    care by unding recuperative beds in

    Los Angeles and much-needed stang

    at the Bakersfeld Homeless Center,

    we provide fnancial and other supportto a wide array o programs that help

    strengthen the saety net in many ways.

    While Community Beneft has historically helped increase access

    to primary care, we discovered that access to specialists has been

    limited. Te Specialty Care Initiative launched in 2008 with theissuance o planning grants unded by three partners, including

    Kaiser Permanente. Our goal is to acilitate collaboration between

    public hospitals, community clinics, and others so patients get the

    specialty care they need.

    In the ools or Quality eort, we have helped establish a

    collaborative o six unding partners to assist 33 community clinics

    across the state to acquire and implement comprehensive diseaseregistry and chronic disease sotware. By putting these tools in

    place, clinics can improve their quality o care by extracting and

    analyzing data about patient treatment and using it or more

    proactive patient outreach.

    Getting aordable oral

    health care is not easy or

    the poor. Thats why

    Community Beneft unded

    requests or $2.5 million

    in grants or oral healthprograms ocused on

    prevention, direct access

    to care, and provider education.

    In Los Angeles County, one

    grantee used these unds

    to include oral health care

    in its medical hubs serving

    oster youth.

    Sharing our experience and

    resourcesAs one o the nations largest healthcare organizations, Kaiser Permanenteis home to a great deal o expertise. Sharing this knowledge and broadening

    its use has become an important unction o Community Beneft. For many

    years, our physicians and health care proessionals have donated their time

    and talents to organizations throughout the community.

    Te University Muslim Medical Association (UMMA) community clinic, the

    frst ederally-qualifed Muslim operated clinic in the U.S., was co-ounded

    by Alta Kazi, MD, a Kaiser Permanente pediatrician. UMMA continues toreceive unding through Community Beneft.

    At the Venice Family Clinic, Kaiser Permanente medical residents serve as

    volunteers under the guidance o Jimmy Hara, MD, chairman o the clinics

    board and director o amily medicine residency at Kaiser Permanente Los

    Angeles Medical Center.

    In addition to unding, Community Beneft also coordinated the donation o

    in-kind construction project management to help build a new 20,000-square-

    oot health acility with 25 examination rooms, a new dental acility, teaching

    spaces, and administrative oces. With this expansion, the clinic can now

    handle 50 percent more medical visits annually.

    Kaiser Permanente Community Beneft has helped the San Diego Council

    o Community Clinics and Riverside County Regional Medical Center

    implement an evidence-based intervention to prevent cardiovascular disease

    in diabetic patients. Developed at Kaiser Permanente by R. James Dudl, MD,

    the aspirin, lisinopril, and lovastatin (ALL) protocol replaces a complicated

    regimen requiring multiple visits with three cost-eective pills taken once a

    day, improving health outcomes and quality o lie or patients with diabetes.

    brightestEducating tomorrows medical proessionalsis a core ocus o our Community Benefteorts. More than just traditional schooling, we emphasize programs that

    oer real-world experience. Our Community Medicine Fellowships allow

    Kaiser Permanente residents the opportunity to provide care in community

    clinic settings in Los Angeles, Orange County, and Woodland Hills under the

    direction o our physicians. Tis program is scheduled to expand in the near

    uture, with new locations in Fontana and Riverside.

    Were also doing everything we can to minimize the impact o the ongoing

    nursing shortage. We established the School o Anesthesia in 1972 when we

    saw the valuable care that advanced practice nurses could oer. Te school is

    ranked among the top-ten anesthesia programs byU.S. News & World Report

    and is the number one trainer o Certifed Registered Nurse Anesthetists in

    the state o Caliornia. Community Beneft unding helped secure an additional

    aculty position or the school and supported its eorts in distance learning and

    the new associates degree program with Pasadena City College.

    Playing an integral part in our

    communitiesA number o KaiserPermanente SouthernCaliornia Community Beneft programs ocus on the importance o outreach

    and establishing an ongoing presence in our communities. Since 1967, the

    Watts Counseling and Learning Center has provided quality counseling and

    educational services to the surrounding community. Te center oers:

    Pre-employment training classes

    All o the unding or the program and its 30-person sta is provided by

    Community Beneft. Te Watts Center also served as the model or our

    Baldwin Park Educational Outreach Program. In 2008, this center in the

    San Gabriel Valley ocused on helping the community residents advocate or

    themselves. While the program continued many o its established outreach

    eorts, one notable success last year was the adoption o the City Lactation

    and Accommodation olerance Policy to ensure creation o a child-riendly

    municipal environment that supports breasteeding.

    At Kaiser Permanente Southern Caliornia Community Beneft, were always

    looking or ways to increase the impact o our most valuable assets our people.

    More than just writing a check or unding the purchase o equipment, the

    opportunity to take our medical expertise into the community can have a ar

    reaching eect. Tat is the motivation behind our Raymond Kay Fellowships

    program. In 2008, the program enabled fve primary care nurse practitioners

    and physicians assistants to take a paid sabbatical and serve their community.

    Not only did these individuals help their assigned clinics with patient

    workload, they also established outreach programs and enabled access to

    Kaiser Permanente educational resources to improve the quality o care.

    community parks, the Trustor Public Land

    has introduced more people

    Bringing our communities

    back to basicsEating and exerciseare two o the mostundamental ways we, as human beings, take care o ourselves. But, making

    the right choices isnt always easy, especially in communities where produce

    is not readily available, junk ood is prevalent, and where open spaces

    and sae, exercise-riendly areas arent easily accessed. Tats why Kaiser

    Permanente Southern Caliornia Community Beneft has made this a

    critical area o unding.

    More than $2 million was investedin encouraging healthy eating

    and active living in ourcommunities in 2008.

    Making healthy ood choices

    more appealingMore than justcore curriculum,our schools are a place or learning about lie. What would happen i we treated

    the caeteria as a dierent kind o classroom? Tats what the Healthy School

    Food Coalition has done in the Los Angeles Unifed School District. As a result

    o this groups eorts and the $50,000 it received in 2008 rom Community

    Beneft, parents and students have seen important changes in what the second

    largest school district in the country is teaching our kids about eating.

    Tis collaborative o parents, students, and administrators has succeeded in

    having an impact throughout the district. From banning on-campus soda and

    junk ood to obesity intervention and caeteria improvements, changes are

    happening on every level. By encouraging the district to hire an executive

    che to improve the menus, getting students involved in menu design, and an

    ongoing eort to eliminate the stigma o eating in the caeteria, the collaborative

    has increased meal program participation and has ensured that our kids have

    healthy options and better ood education.

    Helping communities exercise

    their optionsIn areas where gyms are notavailable or economically viableoptions, how can we encourage exercise and make it a community priority?

    Tat is a question Community Beneft works closely with our grant partners

    to eectively answer. WalkSanDiego has been addressing these issues by creating

    a saer pedestrian environment in west Chula Vista, working with schools,

    citizens, promotoras, and local youth to improve the local neighborhoods

    environment or walking and biking and make areas like Lauderbach Park saer

    and more inviting. By clearing out overgrown plants and enhancing planned

    city improvements such as new restrooms, drinking ountains, and playground

    equipment, the WalkSanDiego grant has helped make these areas more secure

    and useable.

    In parks in Los Angeles County, the rust or Public Land Partnership has

    transormed open, unused space into outdoor gyms. In the 11 ftness zone

    sites installed so ar, the trust has ound that expanding a communitys exercise

    options is very well received. From senior citizens working out together to

    parents using the equipment while they wait or their kids to fnish baseball or

    soccer practice, the acilities are almost always busy. Te easy-to-use equipment

    is ideal or any ftness level. And most importantly, the ftness zones have

    become hubs o community activity by creating a group atmosphere that eels

    sae and social and helps people be more active.

    Access to Care

    Community Medicine FellowshipFellows provide direct patient care and mentor residents and

    medical students in the provision o care in a variety o settings

    including community health clinics, homeless shelters, and local

    schools. Participants collaborate with local health department

    physician leaders to develop programs that address community

    health concerns.

    Grants to Hospitals and Medical Service AreaGrant unds are allocated annually to each o the 13 Kaiser

    Permanente hospitals and Kern County medical service area

    to support community beneft activities and projects outlinedin the local community beneft plans and local community

    needs assessments. Most unding is in the orm o charitable

    contributions to community coalitions, local government

    agencies, nonproft 501(c)(3) organizations, and public schools.

    Health Insurance Outreach and CoverageKaiser Permanente supports activities related to health coverage

    or all uninsured children in Caliornia by partnering with

    organizations to support ongoing outreach to enroll children

    at no cost to the applicants. Available health programs include

    Healthy Families, Medi-Cal, Healthy Kids, Caliornia Kids and

    Kaiser Permanente Child Health Plan.

    Healthy FamiliesHealthy Families is a ederal- and state-unded insurance

    program that provides low- and moderate-income amilies with

    comprehensive health insurance or children under age 19. o

    qualiy, amilies must meet the ederal income guidelines, and

    the children must be ineligible or Medi-Cal coverage.

    HIV/AIDS GrantsHIV/AIDS grants support community agencies throughout

    Southern Caliornia that serve the health care needs o people

    with HIV/AIDS. Te unding categories or the grant includeprevention, awareness, and/or treatment o individuals with

    human immunodefciency virus.

    Kaiser Permanente Child Health PlanKaiser Permanente Child Health Plan provides comprehensive

    benefts to children up to age 18 with amily income up to 300

    percent o the ederal poverty level, who lack access to employer-

    subsidized coverage, and do not qualiy or public programs.

    Premiums are $8 or $15 per child per month, depending on

    amily income, with low copayments or some services.

    Medi-Cal Complex Case Management Program

    Te Medi-Cal Complex Case Management Program providescase managed services to high-risk Medi-Cal clients who have

    complex chronic conditions or special needs such as seniors and

    persons with disabilities. Te program integrates and coordinates

    care and services across the continuum, including linkages

    between internal and external resources.

    Medi-Cal Fee-or-ServiceSouthern Caliornia Region hospitals provide health care on

    a ee-or-service basis or Medi-Cal benefciaries not enrolled

    as Kaiser Permanente members. Reimbursement or some

    services usually is signifcantly below the cost o care.

    Medi-Cal Managed CareTe Medi-Cal Managed Care program provides comprehensive

    inpatient and outpatient care to Medi-Cal managed care

    members through various local and state government entities.

    Kaiser Permanente subcontracts with local initiatives in Los

    Angeles, Riverside, San Bernardino, and Orange Counties

    and contracts directly with the state through their Geographic

    percent o the ederal poverty line.

    Public Hospital ProgramTis eort provides support or the Caliornia Association

    o Public Hospitals and Health Systems (CAPH) to develop

    a long-term partnership with public hospitals. Te Caliornia

    Health Care Saety Net Institute, CAPHs research and

    educational aliate, works collaboratively with Kaiser

    Permanente to implement chronic conditions management,health and clinical education, community health, cultural

    competence, and inorming policy.

    Raymond Kay Community Clinic Fellowship ProgramTe ellowship is a one-year assignment or registered nurse

    practitioners or physicians assistants to practice medicine at a

    community clinic in Southern Caliornia. Fellows provide care

    to underserved patients in community settings.

    In 2008 Kaiser Permanente Southern Caliornia Community Beneft provided unding and

    Tis eort provides support to community clinics to improveaccess and quality care in rastructure, and extend partnerships

    to health departments and public hospitals. Focus areas include

    chronic disease management, specialty care, homeless services,

    quality improvement, nurse practitioner ellowship, access to

    primary care, HIV/AIDS, and senior health.

    Steps PlanKaiser Permanente Steps Plan oers members the opportunity

    to continue their health care coverage at reduced cost when

    experiencing fnancial diculty due to job loss, involuntary

    reduction in work hours, legal separation, divorce, or death o

    a spouse.

    Healthy Eating Active Living

    Grants to Hospitals and Medical Service AreaGrant unds are allocated annually to each o the 13 KP hospitals

    and Kern County medical service area to support community

    beneft activities and projects outlined in the local community

    beneft plans and local community needs assessments. Most

    unding is in the orm o charitable contributions to community

    coalitions, local government agencies, nonproft 501(c)(3)

    organizations, and public schools.

    Healthy Eating Active Living (HEAL)HEAL promotes place-based healthy eating, active living

    programs, and other interventions to combat increased obesity

    rates. Te initiative supports eorts by community providers

    and coalitions to bring community-wide medical, environmental,

    and social changes that can help decrease obesity.

    Sharing our Knowledge and Resources

    Adjunct Faculty Support Program

    Clinical instruction is provided or nursing programs utilizing

    a qualifed Kaiser Permanente registered nurse sta to cohortso 10-12 nursing students while they are on clinical rotation in

    a Kaiser Foundation Hospital acility.

    Advanced Practice and Allied Health CareEducational Programs

    Te Southern Caliornia Department o Proessional Education

    (DPE) oers advanced practice and allied health care providers

    programs designed to meet many o the primary and continuing

    educational needs o allied health care workers. For every

    symposium that is organized by DPE, the community is invited.

    Scholarships are provided or any provider in community clinics

    or hospitals that are in partnership with Kaiser Permanente.

    Allied Health Care Education Scholarship Program

    Scholarships o up to $4,500 help Caliornia students pursueallied health care degrees. Caliornia Health Proessions

    Education Foundation manages the program or Kaiser

    Permanente. Preerence is given to students rom under-

    represented groups. Scholarship recipients must work or

    volunteer in a medically underserved area in Caliornia

    ater graduation or a designated period.

    Board o Registered Nurses Work Study Program

    Tis program gives nursing students direct clinical experience

    beore graduation, allowing them to enter the workplace with

    additional confdence and competence. Tis program helps

    educate the uture nursing workorce and enables students toassume a nursing role more rapidly and saely.

    Clinical Psychology Internship raining ProgramConducted under the Department o Psychiatry and Addiction

    Medicine, the program transitions interns rom students to

    proessionals by providing training in the roles and unctions

    o clinical psychologists. Pre-doctoral students augment their

    educational experience by working in a high-quality educational

    environment with a varied patient base, and by working with a

    multidisciplinary sta.

    Community Health PromotionRegional Health Education (RHE) provides health education

    programs, materials, and services, and also oers training sessions.

    RHE participated in specifc community-ocused projects and

    partnered with several nonproft community agencies to provide

    additional services or several critical priority needs, including

    obesity, diabetes, asthma, and healthy eating and active living.

    Community Medicine FellowshipFellows provide direct patient care and mentor residents and

    medical students in the provision o care in a variety o settings

    including community health clinics, homeless shelters, and local

    schools. Participants collaborate with local health departmentphysician leaders to develop programs that address community

    health concerns.

    Deloras Jones Nursing Scholarship ProgramTe scholarship provides fnancial assistance or students enrolled

    in any Caliornia nursing program to encourage and support

    advanced nursing training. Scholarships are based primarily on

    fnancial need and are awarded or underrepresented minorities,

    academic excellence, nursing as a second career, and graduate

    nursing degrees.

    Distance Learning, Advanced Nursing Degree Programs

    Kaiser Permanentes Distance Learning Program and CaliorniaState University (CSU), Fullerton, entered into a partnership

    to graduate registered nurses to the bachelor o science in

    nursing. Courses are open to both Kaiser Permanente and

    community registered nurses using a combination o interactive

    videoconerence and the Internet.

    Te master o science in nursing partnership began its frst

    semester in August 2008 and concentrates on nursing leadership.

    In addition, these students may elect to take three additional

    courses oered by CSU, Fullerton, and earn a certifcate in

    instructional design and technology.

    Educational Outreach ProgramTe dropout prevention program provides education and support

    services, primarily to Latino youth, ages 10 to 14, in the San

    Gabriel Valley area o Los Angeles County. Nine programs help

    students and parents with homework, study skills, and reading

    improvement and provide assessment o mental health needs in

    the community and summer enrichment sessions.

    Educational Teatre ProgramsTe programs use live theater, music, comedy, and drama to

    inspire children, teens, and adults to make healthier choices

    and better decisions about their well-being. All perormancesare delivered by proessional actors who are also trained health

    educators and are ree to schools and the general community.

    Extremity/Sports Rehabilitation Fellowship ProgramTe ellowship provides education in the specialty area o sports

    physical therapy and rehabilitation, with a ocus on advanced

    training in examination and treatment procedures o extremity

    injuries in an active and post-surgical patient population with

    musculoskeletal conditions.

    Graduate Medical Education

    raining and education are provided to medical residents inamily medicine, internal medicine, obstetrics/gynecology,

    pediatrics, and psychiatry. As part o their training, several

    o the independent residents have rotations at school-based

    health centers, community clinics, and homeless shelters. Tese

    ambulatory settings provide primary medical care services to

    low-income children and adolescents, the homeless, and other

    vulnerable populations.

    INROADSKaiser Permanente partners with INROADS, an organization

    ocused on developing underrepresented college students o color

    or leadership roles in the community. Student interns typically

    work two to fve summers with the goal o permanent placement

    upon graduation rom college, or continued education in the

    health care feld. Tis developmental program enhances the

    diversity o the health care workorce.

    Los Angeles Orthopedic Manual Terapy FellowshipProgram

    Te ellowship provides education in the specialty area o

    manual therapy ocusing on advanced training in manual

    examination and treatment procedures benefcial to patients with

    musculoskeletal conditions.

    Movement Science Fellowship

    Tis ellowship provides education in the specialty area omovement science, with a ocus on advanced training in

    movement analysis, therapeutic exercise, and ergonomic

    Nursing Practices Research Program

    Te program oers nurses the opportunity to engage in researchon nursing practice, patient care, and patient outcomes to

    improve clinical practices. Tis program provides administrative

    and technical support or nurses to conduct, publish, and

    disseminate studies to improve patient care and practices as well

    as contribute to the knowledge base on nursing.

    Oliver Goldsmith, MD, Scholarship ProgramTe scholarship honors Oliver Goldsmith, MD, retired medical

    director o the Southern Caliornia Permanente Medical Group,

    and champion o culturally responsive care. Scholarships are

    awarded annually to medical students entering the third or ourth

    year o study who have demonstrated commitment to diversitythrough community service, clinical volunteerism, leadership, or

    research.

    Pharmacy Resident Post-graduate raining ProgramsTe program provides one- or two-year training or licensed

    pharmacists to gain additional experience in pharmaceutical

    care and administrative pharmacy services in a ully integrated,

    nonproft managed care organization. Pharmacy residents

    and their preceptors participate in several community health

    initiatives, and develop and disseminate medical knowledge to

    health care providers and community members.

    Physical Terapy Orthopedic Residency ProgramTe residency provides education in the specialty area o

    orthopedic physical therapy. Each year, the program oers

    physical therapy residency slots at Kaiser Permanente hospitals in

    Southern Caliornia. Graduates are eligible to sit or their board

    certifcation examination in orthopedic physical therapy and

    apply to participate in a physical therapy ellowship.

    Radiology echnology rainingStudents enrolled in community college radiology technology

    programs can complete a ull year clinical rotation required

    or certifcation as a radiology technologist at one o nineKaiser Foundation Hospital acilities in Southern Caliornia.

    Summer Youth Employment ProgramKaiser Permanentes L.A.U.N.C.H. (Learn About Unlimited

    New Careers in Healthcare) Summer Youth Employment

    Program provides underserved high school students with

    supportive and meaningul employment experiences in the

    health care feld. Many ormer students are now employed

    with the organization as nurses, department administrators, labtechnicians, opticians, and engineers.

    Watts Counseling and Learning CenterTe Watts Counseling and Learning Center has been a valuable

    community resource or low-income, inner-city amilies in South

    Los Angeles. Te center provides mental health and counseling

    services, assistance to children with learning disabilities, and

    pre-employment training or high school youth. In addition, it

    operates several programs including a state-licensed preschool

    program, a summer day camp, and scholarships or high school

    and post-secondary students.
