•Tiger Tails Animal Hospital Wellness Plan Agreements are good for one year and renew annually unless client provides request to cancel. Purchaser has the option to pay in full or spread the cost over 12 monthly payments. Sign up fee waived if paid in full.• Two forms of payment required at sign-up. Major credit cards and/or automatic bank draft accepted.• Payment date changes are subject to a $15 administrative fee.• Failed/declined credit card transactions will be subject to a $15 administrative fee and 1.33% interest rate per month until paid. If client does not respond to requests for updated credit card information within 10 business days, the Wellness Plan Agreement will be terminated and the account balance will be due immediately. Any balance due over 90 days is subject to be turned over to collections.• A Wellness Plan may be cancelled due to the following circumstances: Relocation, rehoming of pet or death of pet.• Upon cancellation, the cost of services received, less installment payments made will be due immediately.* Prescription diet and flea/tick medications excluded.
