Dear Students,

Greeting from KLE VK Institute of Dental Sciences, Belagavi. Hope you and your family

are doing well.

As per the Dental Council India advisory (reference no. No.DE-14-ACADEMIC(MDS)-

2020/344, dt. 2nd June, 2020) the conduct of BDS and MDS examinations should be as per

standard norms and the students will have to appear for the examination in person for boththeory and practical / clinical examination. The MDS Part I and Part Ill examinations are

scheduled from 7th July 2020 upto 21St July 2020 as per the notification circulated. The final

year BDS (RS1 & RS2) theory examination is scheduled from 3rd to 12th August 2020 followed byclinical examination.

In view of this the KLE VK IDS and the KLE Academy of Higher Education and Research,

Belagavi has come-up with the following guidelines for MDS Part I and Part Ill and lv BDS (RS1 &

RS2) :

1. All students should report minimum 21 days before the scheduled date of examination.

2. Students who are arriving to Belagavi by any mode of transport will be stamped at theborder / Airport / Railway station and will have to compulsorily report to the CorporationOffice in Kumar Gandharva Hall, Subhash Nagar (next to SP Office), Belagavi, timing 10:30

am to 5 : 30 pin.

3. Those who are not stamped are strictly not allowed to enter the KLE Campus.

4. All the students who are coming from any state other than Karnataka will have to strictlyundergone home quarantine for minimum 14 days in the hostel rooms or at their place ofresidence.

5. The students coming from any districts of Karnataka are EXEMPTED from Institutional andHome Quarantine.

6. Students of Maharashtra State to note the following : -

a. The students having negative COVID-19 test from lcMR approved lab 48 hours before

entry into Karnataka state (Belagavi City) will be home quarantined mandatorily for 14

days in the hostel or in place where they stay in Belagavi.

b. If the students failed to produce COVID-19 negative test report on entry into state

(Belagavi City), they will compulsorily undergo Institutional quarantined for seven daysat hotels approved by District administration on payment of required charges. Aftercompleting the above procedure, the students should produce the registrationacknowledgement received from Belagavi city corporation to the respective hostelsupervisors and thereafter, they will be home quarantined for 14 days in theirrespective hostel rooms or their place of residence.

7. All the students will have to fill up the undertaking in a specific format which shall be mailed

to them from the college.

8. The home quarantine in the campus hostels will be managed by the KLE University and all

care will be taken of the students by the respective hostel staff.

9. The students shall be coming to the campus in a phased manner. Crowding of the rooms orother institutional spaces will be prevented. All students to note that the examination willbe conducted by the KAHER (university) following standard procedures.

UNDERTAKING FOR ENTRY INTO KARNATAKA STATE

(BELAGAVI / HUBBALLI-DHARWAD / BENGALURU CITY)

KLE Academy of Higher Education and Research (KAHER) bearing KAHER Registration No.

hereby undertake to complete the procedural formalities with regard to Institutional

and / or Home Quarantine keeping in view of the latest directions of the Government of India and

the Government of Karnataka in order to appear for the University Examinations / attending the

academic classes in the constituent units of KAHER:

a) I have completed the On-line Registration through Seva Sindhu Portal of the

Government of Karnataka and possess e-pass for entry into the Karnataka State

(copy of the downloaded version of Seva Sindhu Registration dated

enclosed - Annexure-I).

b) I have been tested / not tested in the Lab approved by lcMR for COVID-19.

The date of test shall be minimum 48 hours prior to the entry of the Karnataka State

and the same should be produced at the entry of Karnataka State (Belagavi / Hubballi-

Dharwad / Bengaluru).

c) My journey schedule for entry into Karnataka will be governed by the following rules:

i) Students of Maharashtra:- lf the COVID-19 test negative, minimum period of 14 days, Home Quarantine

is mandatory either in the hostel or in the place where they stay.

- lf the students fail to produce COVID-19 negative test report on entry into the

State (Belagavi City / Hubballi-Dharvad / Bengaluru), they are required to

undergo 7 days of Institutional Quarantine in the Hotels approved and

charges fixed by the concerned District Administration (Belagavi /

Hubballi- Dharwad / Bengaluru), followed by 14 days of Home Quarantine

either in the Hostel or in the place where they stay.

ii) Students from other states (except Maharashtra):- For th6 remaining students, they are required to undergo 14 days of Home

Quarantine compulsorily either in their hostels in the campus or in the place of

their stay.

The stuclents of Karnataka are exempted from Institutional and

Home Quarantine.

I hereby undertake that I shall strictly adhere to the amendments / revised guidelines

issued by Government of India and the Government of Karnataka from time to time.

ln pursuance of the above guidelines for entry into Karnataka State (Belagavi /

Hubballi-Dharwad / Bengaluru city), the stipulation of minimum period of Institutional Quarantine

and / or Home Quarantine have to be planned for their itinerary and make reservations well-in-

advance for entry into Belagavi / Hubballi-Dharwad / Bengaluru city.

I hereby undertake to inform the concerned Head of the Institution my itinerary including

date and time of arrival and mode of journey well-in-advance (one week in advance).

I am aware that any violation of Isolation / Quarantine guidelines by me in and outside the

KAHER (JNMC) campus will attract penalties of NDMA Act / Epidemics Diseases Act as per the

guidelines of the Government of India including filing of FIR and shift to the Government

Institutional Quarantine facility by the District Administration.
