Triple Play, sponsored by The Coca-Cola Company, is a dynamic wellness program currently being offered in Boys & Girls Clubs that demonstrates how eating right, keeping fit and forming positive relationships add up to a healthy lifestyle. The goal of the Triple Play program is to improve Club members’ knowledge of healthy habits; increase the number of hours per day they participate in physical activities; and strengthen their ability to interact positively with others and engage in positive relationships. 1. REGISTRATION : $20 per team Guaranteed two game minimum Form must be turned in by 2/20 for t-shirt Form must be turned in by 3/7 for play in tournament (no call-ins) Return completed registration forms in person to: Lacey Boys & Girls Club 1105 Tracey Lane SE 2. Public Schools, and the district assumes no responsibility for the

All registered teams must check-in no later than 9:45am. Games start at 10am sharp. Late check in may result in forfeiture of your first game (at the discretion of the Tournament Director).

GAME DAY RULES

1. All teams will consist of three or four players: three starters and an optional substitute. No changes to your roster once the tourney starts.

2. The event is for boys & girls in 5th-12th

grades.

3. Teams will be grouped into grade and gender

divisions.

Team divisions are as follows:

5th & 6th grade boys/5th & 6th grade girls

7th & 8th grade boys/7th & 8th grade girls

High school boys/High school girls

The grade of the team’s oldest player will deter-

mine their division. Co-ed teams will be placed in the boys division.

4. Players may play up in a division, but may not play in a lower division

5. Scoring:

One point per basket; two points for shots from

behind the arc. First team to 15 points (win by two) or the team ahead after 20 minutes wins. (For ties: next basket wins.)

One free throw shot awarded if fouled in act

of shooting.

Starting possession for each game is

determined by a coin toss by the official.

Possession alternates after each basket.

6. Held/Jump balls: possession awarded to defensive

team

Unsportsmanlike conduct by players or spectators will result in ejection or disqualification at the

discretion of the officials or tournament director. This includes yelling at or criticizing any player/

ref.

*A complete set of rules will be available at each

scorer’s table during the games.

Waiver Form and Parent’s Signatures: In consideration of the opportunity offered

to my child to participate in this activity, I, as evidenced by my signature below, do hereby hold harmless, release and waive all claims I may have or my child may have against the Boys & Girls

Clubs of Thurston County, North Thurston Public Schools, their officials, employees, agents, and any other person involved in this activity for any and all injuries, losses or damages suffered by me or my child as a result of my or my child’s participation. I accept full responsibility for the cost of treatment for any injury and understand and agree that the Boys & Girls Clubs of Thurston County, and North

Thurston Public Schools doesn’t offer insurance of any type or kind for injuries suffered as a result of participating in the 2013 Triple Play Hoop Jam. I further give consent to him/her being given a physical exam or emergency treatment by a physician or a hospital in case of an emergency. I hereby authorize each of the entities listed above to depict in perpetuity my likeness, image,

name, signature and other indicia of my rights of publicity (“Images”) in photographic or other works appearing in any and all media for purposes of promoting, advertising, or marketing current or future

events or programs and I agree that such images may be used for that purpose without compensation. Furthermore I understand, and agree to abide by, the rules and guidelines of this

event as determined by the Tournament Director.
