Welcome to the spring issue of CareLines.

First things first: Caldwell Hospice and PalliativeCare homecare nurse Dottie Metzger’s retirement isour “cover story.” She retired at the end ofFebruary, as our longest-tenured employee with 27years of service.

If you look for “hospice nurse” in the dictionary,you might see a picture of Dottie Metzger. We haveshared some “Dottie stories” on page three, andtheir common theme is her respect for the dignityof her patients and families. She combined profes-sionalism with extraordinary people skills to serveas a role model for all of us.

Dottie’s contributions to hospice care were proven,when Lenoir’s News-Topic readers voted herCaldwell County’s best nurse in 2005 and 2008. We cannot measure Dottie’s value to our patientsor our staff. We will miss her!

Next, our entire staff deserves thanks for helpingus maintain our status as one of the top hospicesin the country. Each quarter, the National Hospiceand Palliative Care Organization (NHPCO) conductssurveys with patient family members and care-givers. Caldwell Hospice is consistently among thenational leaders, and the numbers show why.

Fourth-quarter 2011 survey results place CaldwellHospice among the top 10 percent of all hospicesnationwide in 19 of 22 quality indicator results. In14 of those indicators, we recorded a perfect score.

The overall quality of care given to our patientsduring the fourth quarter of 2011 was 93.9%,compared to 81.1% for hospices in North Carolina

and 75.6% forhospices nationwide.

The survey reports ahospice’s performancein four key areas:coordination of care,attending to family’sneeds, providing infor-mation aboutsymptoms, and informing and communicating withpatients. Performance is presented as a “problemscore,” with zero noting no problems. Below areCaldwell Hospice’s performance numbers, followedby North Carolina and national figures:

• Coordination of care – Caldwell Hospice (2.0), N.C. (6.3), U.S. (7.3)

• Attend to family needs – Caldwell Hospice (0.0), N.C. (4.2), U.S. (5.1)

• Provide information about symptoms – Caldwell Hospice (0.0), N.C. (4.0), U.S. (4.9)

• Inform and communicate with patients – Caldwell Hospice (0.0), N.C. (13.2), U.S. (16.0)

Dottie Metzger retired from Caldwell Hospice andPalliative Care at the end of February as itslongest-tenured employee, concluding a 27-yearhospice homecare nursing career.

Dottie’s home visits with patients go far beyond“duty”; she cares about them. “I will miss mypatients and their families,” Dottie said.

Dottie’s patients and their families will tell you thatthe feeling is mutual, and people throughoutCaldwell County agree. Lenoir’s News-Topic readersvoted her Caldwell County’s best nurse, in 2005and again in 2008. Her philosophy of hospice careis “to keep patients comfortable, give them thedignity they deserve, and give them the opportu-nity to die at home.”

During a recent home visit, Dottie pulled out herstethoscope and listened to Alley Brooks breathe;she read medicine labels and reassured his wifethat the store-brand cough medicine was okay; sheasked Mr. Brooks if he was feeling better and thengot a second opinion from his wife to be sure.

The Brooks’ cousin who lives next door came in,followed by a friend who was dropping by to visit.The couple’s German Shepherd Charlie sat to theside of the couch, close by the patient. Laughterrose and fell, as jokes and teasing were matchedwith questions and answers about Mr. Brooks’health.

This is the atmosphere that Dottie cultivates: youmight think she’s a friend dropping by for a weeklyvisit, except that Dottie gathers necessary medicalinformation and learns essential details during thevisit. This is her gift: she combines medical skill, aprofessional focus on patients’ well-being, and thespecial qualities that make her visits feel personal.

“Dottie carries skill, respect, and her stethoscopeon each patient visit,” said Executive Director Cathy

Swanson. “There is no way we can measure hervalue to our patients or to our staff. We will missher.”

Dottie didn’t plan on a career as a Caldwell Hospicenurse. She worked in Baltimore’s Lutheran Hospital

emergency roomfor five yearsbefore she movedto Lenoir. It tooktwo requests toentice Dottie tojoin the CaldwellHospice staff. Thefirst time, Dottieexplained that shewanted to helppeople get better.When the secondrequest came afterher father’s death,Dottie welcomedthe chance to“repay for servicesrendered.”

It turns out that Dottie was a natural for providinghospice care. “I believed in what I was doing,”Dottie said. “I’ve always loved being here.”

“Here” is Caldwell Hospice, generally, and althoughDottie worked in the Stevens Patient Care Unit foralmost a year, after it first opened in 1989, shewas relieved when she returned to making homevisits. Dottie likes getting to know families, buildingrelationships with them, and teaching them thatpatients can die at home, comfortably.

Co-workers can recite the important lessons theylearned from Dottie: be prepared for any situation,ensure that patients will be pain-free, and maintaina sense of humor. Dottie’s parting words to them,at the retirement lunch, were: “I’ll genuinely missyou all, but it’s time to go.”

Dottie plans to limit her travel to visiting her fourchildren and their families, including three grand-children. She has been to Europe and on a cruiseto the Bahamas, Dottie explained, but “my wander-lust has left.” She will bake cakes—another of hertalents—and crochet and return to Caldwell Hospiceperiodically to teach CPR classes.—Pam Hildebran

The Fall 2011 Above and Beyond recipient, BrendaMonahan (photo, right), is Camelot Manor’s nursing unit secretary. Even in a “noisy environment, sheremains calm and steady,” wrote the staff member who nominated her.

Caldwell Hospice employees and volunteers cannominate staff members of area long-term-care facili-ties who encourage quality end-of-life care for resident-patients and their families and who go “above andbeyond” to support resident-patients, their families, and Caldwell Hospice staff members. The award ispresented each quarter.

You can purchase a personalized brick in memoryor in honor of your loved one to be placed on thewalkway that leads from the Kirkwood front porchto our Stevens Patient Care Unit.

Each four-by-eight brick can be purchased for a$200 donation; it comes with a certificate thatrecognizes your gift. For an additional $25, you can receive a duplicate brick with felt backing and a display stand.

Suicide Grant Put into Practice. We applied for aHogan Family Foundation grant in 2011, based onour awareness that suicide rates in this communityand our resources for responding to them were notin sync:

Ashewood…provides bereavement services foranyone in the community who experiences theloss of a loved one. In addition to providingwhat might be identified as “normal” bereave-ment support, we have averaged 10 to 15suicide-related cases per year over the past fouryears. The gravity of the issue led two membersof our staff to participate in a workshop led byUNC-Chapel Hill School of Social Work ClinicalInstructor Jodi Flick, ACSW, LCSW. The staffmembers were determined to strengthen thecommunity’s ability to respond to the compli-cated grief that accompanies deaths bysuicide....

Our idea was to bring together, as a mutuallysupportive group, the people who are charged withresolving this issue:

Caldwell Hospice and Palliative Care proposes tohost (1) a day-long suicide-prevention workshopto educate and train mental health profes-sionals, nurses, physicians, teachers, counselors,social workers, youth workers, police and correc-tional officers, school support staff, clergy, andvolunteers and (2) a half-day program for areaclergy. Ms. Flick will teach both workshops....

Using the Hogan Family Foundation’s generousfinancial support, we held two workshops in March.The first, Suicide: Prevention, Risk Assessment,Intervention, Postvention, was a day-long presen-tation to professionals. The second, CommunityClergy: Caring for Families in the Aftermath ofSuicide, was a shorter presentation to a differentaudience whose members are also called upon incrisis situations. Both events were co-sponsoredwith Catawba Valley Behavioral HealthCare.

We are grateful for the opportunity to explore,explain, and offer practical answers that allowprofessionals to provide support, with confidence,for coping with the complicated grief that followsdeaths by suicide.

What Color Is Your Grief? Parents and guardiansmight wonder how to help children who have lost aparent, grandparent, aunt or uncle—the childrenmight be sleeping too much or too little; theymight be too rowdy or too quiet; schoolwork orgrades might suggest a problem.

At Grief Camp, children meet others who’ve knownsimilar losses. The staff, adult volunteers, andvolunTEENS use every activity to help the childrenunderstand that it’s okay to feel sadness and loss.As they talk, draw, write, listen, and laugh, theycan find the healing tools they need.

Children may not know how to express theirfeelings. Who would know that COLORS would beso helpful to this process? Well, Dr. Seuss, for one,as these lines from My Many Colored Days illus-trate:

"Some days are yellow. / Some are blue. / Ondifferent days / I'm different too. / You'd be /surprised / how many ways / I change onDifferent / Colored / Days."

Last year’s Grief Campcommittee used Dr.Seuss’s book andexplored “the Colorsof Grief.” Emotion-connected colors werein every activity.Colors gave childrenfreedom to choosetheir expressions ofgrief; colors gaveparents and guardiansa vocabulary fortalking with them.

One parent said,afterward, “He was

able to express the anger, and now the sadness iscatching up.”

“We saw the anger he had, following his terribleloss,” said another parent. “He filled the [sand art]jar with his anger.”

At Grief Camp, staff members talk with the adults,too, about how to continue the learning experienceonce they return home, how to respond to anger,tears, and other emotions; they offer support tothe grown-ups who are still grieving, too.

Unifour Volunteer Conference: “Dementia Is More than Just Forgetfulness”

Caldwell Hospice and Palliative Care sponsored the2012 Unifour Hospice Volunteer Conference,welcoming approximately 170 volunteers fromHospice and Home Care of Alexander (County),Burke Hospice and Palliative Care, PalliativeCareCenter and Hospice of Catawba Valley, as wellas our own volunteers.

Heather McKay, MS, OTR/L (pictured above), anoccupational therapist and dementia care specialist,was the day’s featured speaker. Attendees ravedabout the quality and depth of information shepresented, including:

More and more people, including hospice staff andvolunteers, are facing the hard realities connectedwith dementia. Heather McKay delivered practi-cal and necessary information. Her compassionate

and conversational presentation brought thesecomments from volunteers:

Caldwell Hospice needs licensed hairdressersand barbers to serve as “special volunteers.”They take their skills to the homes of our hospicepatients who aren’t well enough to go to a salon.It’s an opportunity to use your special skills and“give back” to the community. Did you hear the one about thebanker who became aPresbyterian pastor? If you’reunfamiliar with the story, this ishow it goes:

Rob Hinman went into bankingafter earning his bachelor’sdegree from Wake ForestUniversity. He was in the profes-sion for 10 very good years; thenhe went dove hunting, and every-thing changed. No earthly birdswere flying that day, but Rob’slife changed forever. It was “notthe first time I heard the call,”Rob said, “but this time I knew.” He went home,talked with his wife, and they moved to Virginia,where he attended Union Theological Seminary.

After graduating with a Master of Divinity degree in1990, Rob returned to North Carolina and pastoredthe Presbyterian Church in Lowell for eight years andBethel Presbyterian Church near Lake Norman forseven years. Then he came to the First PresbyterianChurch in Lenoir and soon, thereafter, he was invitedto join Caldwell Hospice’s board of directors.

Rob’s father is his model for the ministry; the elderHinman, a medical doctor, was available 24 hours aday, seven days a week. He was one of seven physi-cians called to Washington, DC, by the late PresidentJohn F. Kennedy to serve on a mental retardationcommittee. His influence is clear in the youngerHinman’s community service, as well.

Rob was asked to serve on the board of directors forHoly Angels in Belmont. He calls his ongoing involve-ment “one of the greatest joys of my life.” Hebelieves in the Rotary Club International, too,although he is an inactive member, currently. He

realized how “international”its membership is when hetraveled to Ghana while inthe seminary—and attendeda local meeting. They werecollecting money to fund theeradication of polio, just asAmerican Rotarians do.

Rob has been a CaldwellHospice board member forsix years, and he serves onthe finance committee. Heviews hospice care, as aboard member and as aminister: “The help our

Caldwell Hospice staff and volunteers give makes abig difference, but the heart of Christ that they shareis life-giving to the families we serve.”

Community work and ministerial duties offer similaropportunities: to respond to needs and bring groupstogether, to appreciate and respect both differencesand similarities in people.

Even in the Hinman household, similarities anddifferences are honored—the Wake Forest graduate’swife Ginger graduated from NC State, and their sonsattend UNC-Chapel Hill, where one is a freshman andthe other is a junior.

Rob enjoys hunting and backpacking, and he countshaving hiked in Israel—where the terrain climbs fromthe Dead Sea at 1,349 feet below sea level to Mt.Hermon at an elevation of 7,330 feet—among hisaccomplishments.

The core of Rob Hinman can be stated in thesewords: “I absolutely love Jesus Christ because heloves me, and I look out for the new person who iswondering if he or she will be welcome.”
