For every workplace fatality there are at least 3,00,000 at risk behaviours and 3000 near

misses. Disasters in the past 35 years have taken an estimated 2.5 million lives and cost more

than US$1.5 billion, mainly in developing countries.

India tops the list and studies reveal that in 87% of accident cases the control

room was within the hazardous corridor. Out of 234 recent major accidents investigated by

UK HSE , in 44.1 % of them control systems failed due to incorrect specification

during Design and 14.7% due to Design and implementation errors highlighting the need

for better focus on Process & Functional Safety .

Less than 3 per cent of the world’s water is fresh (drinkable), of which 2.5 per cent is

frozen in the Antarctica, Arctic and glaciers. Humanity must therefore rely on 0.5 per

cent for all of man’s ecosystem’s and fresh water needs.

By the end of this century, the increase in global temperature will exceed 1.5°C & the

emissions of carbon dioxide has increased almost 50 per cent since 1990 . Average

sea level rise is predicted as 24 – 30cm by 2065 due to global warming.
