IntroductionThe Lee County Sheriff’s Office provides a comprehensive compensation package including group insurance benefits, Star Care employee health clinic and numerous other services. The Employee Benefit Highlights Booklet provides a general summary of these benefit options as a convenient reference. Please refer to the Sheriff’s Office Personnel Policies and/or Certificates of Coverage for detailed descriptions of all available employee benefit programs and stipulations therein. If you require further explanation or need assistance regarding claims processing, please refer to the customer service phone numbers under each benefit description heading or contact the Personnel Services Division for further information.

Group Insurance EligibilityThe Sheriff's Office group insurance plan year is January 1 through December 31.

Employee EligibilityEmployees are eligible to participate in the Sheriff’s Office insurance plans if they are full-time employees working a minimum of 25 hours per week. Coverage will be effective the first of the month following 60 days of employment. For example, if an employee is hired on April 11, then the effective date of coverage will be July 1.

TerminationIf an employee separates employment from the Sheriff’s Office, insurance coverage will terminate on the date in which the separation occurred. COBRA continuation of coverage is available as applicable by law.

Dependent EligibilityA dependent is defined as the legal spouse and/or dependent child(ren) of the participant or the spouse. The term “child” includes any of the following:

• A natural child• A stepchild• A legally adopted child• A newborn (up to age 18 months) of a covered dependent (Florida)• A child for whom legal guardianship has been awarded to the

participant or the participant’s spouse

Taxable Dependents Employees covering adult children under the medical, dental and vision insurance plans will be required to pay applicable premiums. Contact the Personnel Services Division for further details.

Dependent Age RequirementsMedical Coverage: A dependent child may be covered through the end of the month in which the child turns age 26. An over-age dependent may continue to be covered on the medical plan to the end of the month in which the child reaches age 30, if the dependent meets the following requirements:

• Unmarried with no dependents; and• A Florida resident, or full-time or part-time student; and• Otherwise uninsured; and• Not entitled to Medicare benefits under Title XVIII of the

Social Security Act, unless the child is disabled.

Dental Coverage: A dependent child may be covered through the end of the month in which the child turns age 30.

Vision Coverage: A dependent child may be covered through the end of the month in which the child turns age 30.

Please see Taxable Dependents if covering eligible over-age dependents over age 26.

Disabled DependentsCoverage for an unmarried dependent child may be continued beyond age 26 if:

• The dependent is physically or mentally disabled and incapable of self-sustaining employment (prior to age 26); and

• The dependent is otherwise eligible for coverage under the group medical plan; and

• Primarily dependent upon the employee for support; and• The dependent has been continuously insured; and• Coverage began prior to the age of 26.

Proof of disability will be required upon request. Please contact the Personnel Services Division if further clarification is required.

Qualifying Events and IRS Code Section 125IRS Code Section 125Premiums for certain supplemental insurance policies such as Aflac (excluding Life insurance), Colonial (excluding Life insurance) and the Deferred Compensation plans are deducted through a Cafeteria Plan established under Section 125 of the Internal Revenue Service (IRS) Code and are pre-tax to the extent permitted. Under Section 125, changes to employee's pre-tax benefits can be made ONLY during the Open Enrollment period unless employee or qualified dependent(s) experience a qualifying event and the request to make a change is made within 60 days of the qualifying event.

Under certain circumstances, employee may be allowed to make changes to benefit elections during the plan year, if the event affects employee, spouse, or dependent(s) coverage eligibility. An “eligible” qualifying event is determined by the Internal Revenue Service (IRS) Code, Section 125. Any requested changes must be consistent with and due to the qualifying event.

Examples of Qualifying Events:

• Employee gets married or divorced• Birth of a child• Employee gains legal custody or adopts a child• Employee's spouse and/or other dependent(s) die(s)• Employee, employee's spouse or dependent(s) terminate or start

employment• An increase or decrease in employee's work hours causes eligibility

or ineligibility• A covered dependent no longer meets eligibility criteria for coverage• A child gains or loses coverage with an ex-spouse• Change of coverage under an employer’s plan• Gain or loss of Medicare coverage• Losing eligibility for coverage under a State Medicaid or CHIP

(including Florida Kid Care) program (60 day notification period)• Becoming eligible for State premium assistance under Medicaid or

CHIP (60 day notification period)• Enrollment in a qualified health plan offered through an Exchange

during a special enrollment period

IMPORTANT NOTES

Employee who experiences a qualifying event must contact the Personnel Services Division within 60 days to make the appropriate changes to coverage. Beyond 60 days, requests will be denied and the employee may be responsible, both legally and financially, for any claim and/or expense incurred as a result of the employee or dependent who continues to be enrolled but no longer meets eligibility requirements. If approved, changes are effective on the first of the month following the qualifying event. Newborns are effective on the date of birth and marriage is effective on the date of occurrence. Cancellations will be processed on the date in which separation occurred. In the event of death, coverage will terminate the date following the death. Employee will be required to furnish valid documentation supporting a change in status or “Qualifying Event.”

TelehealthCigna provides access to two telehealth services as part of the medical plan – AmWell and MDLIVE. Telehealth is a convenient phone and video consultation company that provides immediate medical assistance for many conditions.

This benefit is provided to all enrolled members subject to an applicable copay. This program allows members 24/7 on-demand access to affordable medical care via phone and online video consultations when needing immediate care for non-emergent medical issues. Telehealth should be considered when your primary care doctor is unavailable, after-hours or on holidays for non-emergency needs. Many urgent care ailments can be treated with Telehealth, such as:

Dental InsuranceCigna Dental PPO PlanThe Sheriff's Office provides dental insurance through Cigna at no cost to benefit-eligible employees and dependent(s). A brief summary of benefits is provided on the following page. For more detailed information about the dental plan, please refer to Cigna’s summary plan document or contact Cigna’s customer service.

In-Network BenefitsThe PPO plan provides benefits for services received from in-network and out-of-network providers. It is also an open access plan which allows for services to be received from any dental provider without having to select a Primary Dental Provider (PDP) or obtain a referral to a specialist. The network of participating dental providers can be found in Cigna’s Total DPPO network. These participating dental providers have contractually agreed to accept Cigna’s contracted fee or “allowed amount”. This fee is the maximum amount a Cigna dental provider can charge a member for a service. The member is responsible for a Calendar Year Deductible (CYD) and then coinsurance based on the plan’s charge limitations.

Please Note: Members have the option to utilize a dentist that participates in Cigna’s Total DPPO Network. However, members who use the Cigna Advantage Network will see additional cost savings from the added discount that is allowed for using an Advantage network provider. Members are responsible for verifying whether the treating dentist is an Advantage Dentist or a DPPO Dentist.

Out-of-Network Benefits Out-of-network benefits are used when members receive services by a non-participating Cigna provider. Cigna reimburses out-of-network services based on what it determines is the Maximum Reimbursable Charge (MRC). The MRC is defined as the most common charge for a particular dental procedure performed in a specific geographic area. If services are received from an out-of-network dentist, the member will pay the out-of-network benefit plus the difference between the amount that Cigna reimburses (MRC) for such services and the amount charged by the dentist. This is known as balance billing. Balance billing is in addition to any applicable plan deductible or coinsurance responsibility.

Calendar Year DeductibleThe Cigna DPPO plan requires a $50 individual or a $150 family deductible to be met for in-network or out-of-network services before most benefits will begin. The deductible is waived for diagnostic, preventive and orthodontic services.

Calendar Year Benefit MaximumThe maximum benefit (coinsurance) the dental PPO plan will pay for each covered member is $3,000 for in-network or out-of-network services combined. Diagnostic and preventive services accumulate towards the benefit maximum.

IMPORTANT NOTES

• Each covered family member may receive up to two (2) routine cleanings per calendar year.

• Waiting periods and age limitations may apply for certain services.

• Teeth missing prior to coverage effective date are not covered.

Cigna Vision PlanThe Sheriff's Office provides vision insurance through Cigna at no cost to benefit-eligible employees and dependent(s). For more information about the vision plan, including exclusions and stipulations, please refer to the carrier’s benefit summary or contact Cigna’s customer service.

In-Network BenefitsThe vision plan offers employee and covered dependent(s) coverage for routine eye exams. To schedule an appointment, covered employee and dependent(s) can select any network provider who participates in the Cigna Vision network. At the time of service, routine vision examinations will be covered as shown on the plan’s schedule of benefits.

Out-of-Network BenefitsEmployee and covered dependent(s) may also choose to receive services from vision providers who do not participate in the Cigna Vision Plan. When going out of network, the provider will require payment at the time of appointment. Cigna will then reimburse based on the plan’s out-of-network reimbursement schedule upon receipt of proof of services rendered.

Star Care services are FREE. Some examples of services offered are listed below:

Primary Care and Urgent Care• Services are limited for children ages 2 and up and include only

acute care and school/sports physicals.• Services are also limited for all Medicare recipients. Care provided

within Star Care is not billed to Medicare; therefore, any services that will need to be billed to Medicare require the use of an outside Medicare Provider/PCP.

Medications Dispensed On Site• Providers may prescribe 6-12 months of non-controlled

maintenance medications for stable conditions.

Star Care does allow walk-in patients, however, you will have a longer than normal wait time as they accommodate you between already scheduled patients, and only for an illness. It is ALWAYS best to schedule an appointment when possible; there are same day sick appointments held open in the schedule; once those are taken it is possible a next day appointment will need to be made. If Star Care is your medical home you can still receive medical care outside of the clinic, however, it will be subject to the health plan schedule of benefits.

Star Care has a 10 minute late policy. Your time is important and anyone 10 minutes or more late will be rescheduled. This is to avoid making others wait.

Star Care is staffed by a Board Certified Physician, Experienced Nurse Practitioners, and a Skilled Team of Medical Assistants.

Employee Assistance ProgramThe Sheriff's Office provides at no cost a comprehensive Employee Assistance Program (EAP) available through Synergy EAP Solutions. Synergy EAP Solutions offers access to licensed mental health professionals through a confidential program protected by state and federal laws. The EAP program is available to help employee gain a better understanding of problems that affect them, locate the best professional help for a particular problem, and decide upon a plan of action. The Sheriff's Office employees are entitled to three (3) free counseling sessions. All EAP counselors are professionally trained and certified and licensed in their fields. Master-level counselors are available 24 hours a day, 7 days a week.

What is Dynamic Worklife Solutions?The Sheriff's Office recognizes that employees’ personal responsibilities may, at times, spill over into the workplace. To help ensure employee is able to address these concerns with minimal disruption, the program provides employee and family member(s) assistance for a variety of concerns – including child care, elder care, daily-living issues, and other issues they may encounter.

Are Services Confidential?Yes. Receipt of EAP services is completely confidential. If, however, participation in the EAP is the direct result of a Management Referral (a referral initiated by a supervisor or manager), we will ask permission to communicate certain aspects of the employee’s care (attendance at sessions, adherence to treatment plans, etc.) to the referring supervisor/manager. The referring supervisor will not, however, receive specific information regarding the referred employee’s case. The supervisor will only receive reports on whether the referred employee is complying with the prescribed treatment plan.

Basic Life and AD&D InsuranceThe Sheriff's Office provides all benefit-eligible employees a Basic Term Life Insurance benefit through Symetra, at no cost to the employee. The coverage amount will be determined by the employee's annual base salary, excluding overtime, as illustrated in the benefit table below.

AD&D pays in addition to the Basic Term Life benefit when death occurs as a result of an accident. The AD&D benefit amount equals the Basic Term Life benefit, based on employee classification, partial benefits may also be payable.

Florida Retirement SystemEmployees have a choice of two FRS retirement plans: the FRS Investment Plan and the FRS Pension Plan. Each Plan offers different benefits. Both plans require employee to contribute 3% of salary, beginning with employee’s first paycheck. Employee cannot change the amount contributed. The Sheriff’s Office also contributes a fixed percentage of employee’s gross salary to the plan elected. Contribution rates are set by the Florida Legislature. Investment Plan Pension PlanThis is a … 401(k)-type investment plan.

It is designed primarily for employees who want greater control over their retirement plan and who want flexibility in how their benefit is paid at retirement.

Traditional retirement pension plan.It is designed for employees who are not comfortable with choosing investments and managing their own portfolio, and who want a guaranteed monthly retirement benefit.

You will have to buy into the Pension Plan, using the money in your Investment Plan account. If your balance doesn’t cover the cost, you will have to make up the difference out of your own pocket.

Supplemental Retirement Plans Deferred Compensation Retirement PlansDeferred compensation retirement plans are governed by Section 457 of the Internal Revenue Service (IRS) Code. Sheriff's Office employees have the option of selecting a wide variety of market-responsive investment options for retirement planning and asset allocation strategizing.
