Healthcare Transition of Children and Young People Funding from Burdett Trust for Nursing has enabled Leeds Teaching Hospitals NHS Trust (LTHT) to appoint a team to support the running of a three year project designed to develop a model of care for Healthcare Transition for Children and Young People (CYP). Healthcare Transition is the planed process of supporting young people with long term health conditions when moving from Children’s services to Adult services. Louise Porter, Project Manager Healthcare Transition of Children and Young People, Leeds Children’s Hospital, in collaboration with the Yorkshire and Humber SCN Produce a model of care that is flexible and adaptable to meet the needs of the individual young person and their families in each service. Develop a gold standard model of healthcare transition for all services in LTHT. To improve age appropriate care and services through integration and collaboration with network partners. To be a leading Centre for Transition. To assist and support multidisciplinary teams to develop, implement and audit best practice transition patient pathways. We will use tools and methods such as master schedules (to help us break down what needs to be done, by who and by when), process maps (to help visualise each process now and in the future) and standardisation (so that we agree the best way and all follow the same steps). With each of the objectives we have identified measures that will help us monitor as we change and confirm where we are improving. Improve patient and family experience Improve staff experience and satisfaction Reduce aspects of Length of Stay Improve uptake of OPA Reduce all aspects of transition DNA rates Improve patient health outcomes We will be piloting the benchmarks developed by Professor Faith Gibson. The documentation we will be using is the ‘Ready Steady Go’ and ‘Hello’ documentation developed by Southampton Children’s Hospital.
