How To Get Started In 5 Easy Steps

Step 1: Contact Your Local Department of Social Services
In order to begin, the initial paperwork will need to be obtained from your local
Department of Social Services.
This paperwork will need to be completed by your doctor.
Step 2: See Your Doctor to Complete Paperwork
Your doctor will complete paperwork determining diagnosis, tasks needed, and self-directing ability.
Your doctor will submit your
paperwork to the Local Department of Social Services who
will determine if you are eligible for the
program.
Step 3: Department of Social Services Determines Your Eligibility
Step 4: Home Visit With Consumer
A home visit is conducted in collaboration with Cortland Community Action Program (CAPCO), the Local Department of Social Services, and Professional Nurses to assess the consumer’s needs.
Step 5: The Care Plan Is Made
After assessment, a care plan will made
which specifies:
The hours of care needed
What tasks will need to be performed
Step 5: Your Responsibilities BeginAlthough an assigned nurse will oversee your ability to make informed decisions, it is YOU who is in charge.
Duties include: Hiring and training aide to meet the
care plan developed.

Overseeing that you receive care.
