PURPOSE OF TRANSITIONTo ensure that the student begins the new year with all the necessary supports in place to allow him/her to be successful.

WHO IS ELIGIBLE FOR KINDERGARTEN

All students who are five by September 1st

Early Entrance to kindergarten Students that turn five between September 1st

and December 31st. Have to be screened

Communication is critical Home and school Preschool and Kindergarten teams-including

related service providers Complete the “What Works” forms and pass on to

home school

Participate in meetings (if there are any) for your child

your child Sign up for your home school’s and the district

listserve Attend Kindergarten Orientation nights January

PROCESS AND MEETINGS Preschool notifies home school of incoming

students RED (review of existing data meeting) Evaluation (if needed) MET (multidisciplinary-educational team

meeting) IEP (if needed)

Best setting for meeting a child’s needs (LRE) is determined at this meeting.

• Decisions are made as a team-parents are a critical part of this team.
