As this issue goes to print she is on her honeymoon, no doubt enjoying the break after many months of planning for her spectacular wedding. We wish her and her husband Rowan all the very best! As well as seeing Edwina get married, we also had the privilege of interviewing Elle Macpherson for this issue’s cover story. From local girl to global inspiration, Elle shared some fond memories from the North Shore and also about business and family, the interview shows her as a very grounded superstar. This edition I was delighted to interview Dr Michael Mosley, the world’s most famous human guinea pig. You may know him from his countless documentaries or as the creator of the 5:2 diet. Michael has just released his new book, The Blood Sugar Diet. It’s a worthwhile read especially if you or someone you know has diabetes.

I would also like to give a shout out to Sue who is no longer working with us. On behalf of our team, we would like to say a very big thank you to her for all of the great work she has contributed to Living Local over the last few years!

Christmas seems so long ago now. We could not have been happier with our hams and turkeys last year. The feedback was fantastic, in fact, the best ever. The Bangalow Sweet hams are in a class of their own. Again, we had the organic free range turkeys from a small farm in Victoria, these are really great birds and ethically raised too. It seems to happen quickly, the weather is changing from summer to autumn. A good time to think about slow cook-ing again: chuck steak, pork belly or lovely slow roast lamb from the farm.

FARM NEWSThe farms are going ahead well. We now have the two farms spilt. The new farm at Blayney is positioned in a cool climate and damp area, ideal for growing that lovely grass feed beef. The Dunedoo farm is well suited to our sheep production. The white Dorper sheep thrive in the warmer, drier climate

and the lambs finish really well on the pastures of dry land lucerne, Rhoades grass and digit pastures. It is amazing how well they do. We have won the Best Gourmet Lamb product at the Sydney Royal Fine Food Show two years in a row. They are just superb! We have our cattle numbers up to around 180 now and this current lot of vealers are the best yet. Mostly from our new Murray Grey bull, the same that won the Royal Easter Show Carcass competition last year. Even a lot of the herbs that we use, we grow at the farm. Fresh and organic.

SHOP SMALLAs times change, small business seems to be less and less. The large multi nationals get bigger. We don’t try to ever trade on price, we have strong views on ethical treatment of animals, as we practice on our farms. We love the fact that we know our customers and care about them. Our staff are

An amazing obstetrician whovolunteers to travel to Nepal as a Gynaecological Surgeon. There he operates on women every day with the most severe forms of prolapse. This is a debilitating and socially isolating condition for these women. W.H.O. estimates that over 600,000 women in Nepal suffer with this condition. LAURA DIVE - WINNER

Crosslands Reserve is situated on the fringe of Hornsby Heights in the Berowra Valley National Park and is well worth the drive.

The beautiful picnic spot is named after Burton Crossland who purchased the area in the late 1800s. He established a guesthouse to accommodate recreational visitors arriving by boat and also built the land access route now known as Somerville Road. After Cross-land’s death in 1920 Hornsby Shire Council took over the reserve’s management.

Berowra Creek snakes its way through Crosslands, all the way to the Hawkesbury River and out to sea at Broken Bay near Palm Beach.

The Hawkesbury River is actually classified as ‘drowned river valley’ because it was flooded by rising sea levels about 12,000 years ago. As such Berowra Creek is a mixture of fresh and salt water.

Shell middens indicate that the creek provided the Darug people with a rich seafood diet for thousands of years.

With a potential threat of Japanese invasion during the Second World War, about 2000 small boats were moored at Crosslands for safety. Unfortunately, a heavy storm caused a flash flood that surged downstream, destroying hundreds of these watercraft.

At certain points in the reserve walking tracks disappear into the bush as part of the Great North Walk (which runs an impressive 250km from Sydney to Newcastle).

The reserve is equipped with a children’s playground, flying fox, picnic benches, toilets, bubblers and electric barbecues. Be aware that the vehicle entry gate to Crosslands Reserve is locked in the evening. The 2.5km winding road from the gate to the reserve is quite narrow with many blind corners, so drive slowly.

Lovely camping areas are also available for hire and could be a good option to consider if you are thinking of introducing young children to camping and canoeing. Campsites can be booked through the Hornsby Shire Council website.

As a makeup artist, I work with my hands, so great nails are an essential. I’ve been a DIY manicure woman for many years, but it’s often time consuming to fix chips on the run. Michael is in Sydney for a short period to promote his new book, "The 8 Week Blood Sugar Diet". Maree and I were thrilled to have had the recent opportunity to meet Michael and his lovely wife, Clare Bailey, at Sydney’s Shangri La Hotel for a chat and photo session. Clare and Michael share the same passion for health – they met at medical school and both continue to inspire each other. Clare is a GP, parenting expert and founder of parentingmatters.co.uk. They live in Buckinghamshire with their four children, aged 14 to 23.Four years ago, Michael found out he was a type 2 diabetic and didn’t want to go on medication (which is what lead him to invent the 5:2 diet). He soon discovered that type 2 diabetes could be reversed with intermittent fasting.

HOW IS YOUR HEALTH GOING NOW, FOUR YEARS DOWN THE TRACK?Extremely well, thank you. My blood glucose is fine, I am 12 stone (76kg) and I am in very good shape. I am on 6:1 now because I don’t need to

lose any more weight and research shows you can maintain weight loss with 6:1. I have just had a small handful of nuts and plan to roam around outside later for more food. For breakfast I had smoked salmon and scrambled eggs which was quite nice and filling.

HOW IS AUSTRALIA LOOKING? Research, I have seen, suggests obesity and overweightness tends to be more focused in the inner suburbs rather than in places like Sydney. Obviously, similar problems there but it is part of a worldwide phenomenon.Funnily enough, the research in the UK, and I suspect true of Australia as well, is that in higher income groups, the women are slimmer and the men are fatter. Being overweight seems to be associated with poverty and ignorance. That certainly isn’t the case. When you simply look at the statistics, women from higher income groups tend to be slimmer. There is a very clear relationship between income and slimness. In men, it’s not at all clear, in fact the group in the UK and Australia who are at the lowest rates of being overweight and obese are those on the lowest income. Probably because they do more physically demanding jobs.

HOW DOES SYDNEY’S LIFESTYLE COMPARE TO THE UK? A lot more outdoors. There is the most incredibly, wonderful harbour out there. I took a ferry out to Manly the other evening. To have the ability to get on a ferry and only 20 minutes later be on a beach, and to be able to swim is incredible. I was thinking about surfing, realising I haven’t surfed for a long time but watching other people do so is still [to me] experiencing the great outdoors. I just love being by the sea and I wish London was by the sea. Equally, you have fabulous museums! The only thing (from my observations) Sydney lacks is a good public transport system.

IS NUTRITION FOCUSED ENOUGH ON IN MEDICAL SCHOOLS?Absolutely not. I was talking to a couple of Australian doctors the other evening. One of them said he had to look up the word 'carbohydrate' as he knew so little about it. During my years of training at medical school I was required to attend just one class on nutrition. My son is also at medical school and learning nothing on the topic. You learn some of the more exotic diseases and the deficiency diseases but what you don’t ever learn is what carbs do, what fats do, what protein does, what to say to an overweight patient who comes in and asks, what can I do about this?

There is an interesting GP in North England who works in a deprived area. He has used many of the principles in this book. And his patients are largely working class but they understand how to change their diet, you’ve got some very simple principles. Just by putting these principles into practice he’s dropped the drug budget and the diabetes budget by $50,000 in one single year and he’s cut obesity rates. My wife, Clare, works in a fairly deprived area as well and again, her patients do not have any problems with grasping the principles. She starts off by telling them that pretty well everything we’ve been telling you for the past 20 years is wrong – and at that point they listen.

Clare: “Their attitude is quite moving, because when you say this to them, they often end up in tears as they’ve been struggling for years.”

Clare does the practical stuff, she engages with her patients.

WHAT PART DOES MINDFULNESS PLAY IN THE BLOOD SUGAR DIET?Mindfulness is strongly associated with reaching for comfort food. When you’re stressed what happens

QUITTING SUGAR (AND PROCESSED FOOD) HAS BEEN A HOT TOPIC IN AUSTRALIA, DO YOU AGREE WITH THIS?Yes, but I think it has to go wider. You have to find alternatives. First of all, giving up sugar is really hard, speaking from personal experience. I love sugar. I love chocolate. But I am quite happy to not add sugar to anything. I don’t add sugar to my coffee but I do love sweet things. I have to leave them out of my house. If there is chocolate, I will eat it. But then again, you have to find other things to eat. It is not enough to say, give this thing up, you also have to find alternatives and that’s one of the reasons why in my book I bang on about the Mediterranean diet. If you give everything up you will drive your-self mad, or collapse. You have to find other things to keep you satisfied. That’s why I am a fan of eggs for breakfast because of the egg’s protein. Lots of olive oil, lots of lovely salads and occasionally give in to temptation and having a dessert. My general rule is to share my dessert with my wife because you don’t really need it, you just want that taste at the end of the meal.We have a website thebloodsugardiet.com. Lots of Australians have recently joined and they’re all submitting lovely, healthy recipes. We have asked people to drop in recipes to help people and apparently they’re posting away and they’re delicious recipes.

GENERALLY SPEAKING, GP’S LOVE TO PRESCRIBE MEDICATION, DO YOU AGREE? I think it is sad but completely true. In fact, in the UK you are paid as a doctor to prescribe drugs – not to help patients lose weight. If you help them lose weight then you’re practically losing money. Now, I don’t think many family doctors are motivated by this and I have spoken to Australian doctors who have said they would never do that and always offer their advice.This is more complex with patients who have a high blood sugar and nobody is doing anything about it at all. It’s an unintended consequence of a well-intended policy. That’s often the way in medicine, you start off wanting to do something and it turns into something else.

is your cortisol levels go up and what you want to eat are muffins, etc. Things like carrots, they really don’t hit it. Most of the case studies in the book during stressful events have all responded to stress in that way. There are three pillars to this diet. One of them is calorie cutting, the second is stress management (which is a kind of mindful-ness) and the third is exercise. Three elements you have to lose the weight, then you have to find the programme to keep the weight off. I have tried all kinds of things but I found mindfulness by far the most effective.Being optimistic can add up to seven years to your life, four years more than if a cure for cancer was found. The interesting thing about it is you can actually measure what happens after doing a mindfulness programme after six weeks. During my time filming, 'Don’t Worry, Be Happy,' they could actually see the changes in my brain. I became a happier optimist, it worked! And with no drugs or therapy I felt happier! It’s how you incorporate it into your life. Breathing exercises are the beginning of it. The principle sounds really easy but it’s hard. If you sit down and you think about your breathing and not your thoughts, you immediately start thinking in 20 or 30 seconds about your love, your life, what you are going to have for breakfast, things that

went wrong yesterday - your internal dialogue. A good exercise can involve your daily cup of coffee, instead of being on your email, or anything like that, you just feel the warmth of the cup of coffee in your hand, listen to the sounds going on around you. Disengage from the electronic world and your internal thoughts, and engage in being in the present. That’s what it is all about, it’s like when you go for a walk, instead of looking at your shoes or being on your phone, you actually stop and look at the view. Sydney is brilliant! Live in the moment, it is surprisingly hard to do. Try doing it for 10 minutes each morning. Have a go and see what you think.

EMERGENCY FOCUS HEALTH CARE AND ITS COMMUNICATION COMPLEXITY

Emergency departments (ED) are high stress environments – for health care providers, patients and their families. There has been an increasing realisation of the crucial role that communication between clinician and patient plays. Research has shown that com-munication failures in Emergency departments are a major cause of incidents contributing to avoid-able patient harm. I interviewed Dr Marie Manidis, a postdoctoral research fellow at the University of Technology Sydney, about some of the important research in this area.

WHAT HAS YOUR RESEARCH ENTAILED?We were awarded a research grant through the Australian Research Council. With a team of researchers (see book reference) we researched clinician patient communication in five emergency departments, four in NSW and one in Canberra. We audio recorded 82 patients from triage to disposition in the emergency department, we did over 1000 hours of observation and we interviewed 150 clinicians in all the hospitals. It was an extensive four-year project.

WHAT IS THE IMPACT OF INEFFECTIVE/POOR COMMUNICATION ON PATIENT OUTCOMES AND EXPERIENCES?Poor and inaccurate communication has a significant impact on patient experience, safety and healing. Inadequate communication is identified as to contributing

up to 70% of hospital sentinel events [an unanticipated event in a health care setting resulting in the death or serious physical or psychological injury to a patient, not related to the natural course of their illness] – for example a communication failure such as delays, forgotten or misplaced results, inaccurate or inadequate hand-over of clinical information resulting in safety issues, poor experience issues and incorrect treatment.

DO CLINICIANS NEED TO BE MORE MINDFUL WHEN PROVIDING THEIR PROGNOSIS?You can learn more about how communication works but when you’re under pressure people often resort to old patterns. It’s helpful to be able to put yourself in the patient’s shoes and empathise and check your information. Doctors today are stressed and time poor but they absolutely do their best.

HOW CAN PATIENTS IMPROVE?In non-urgent health settings, patients can help themselves by being more proactive, asking questions, asking for explanations, they can do prior research. The Internet has helped patients but it doesn’t always help clinicians as patients can think they have all the answers. In the ED if the patient is sick, it is important to have someone there who is vigilant and can hear what is being said because it is complex, people do forget and knowledge works in a different way to remembering.

IS AUSTRALIA GETTING THIS RIGHT AND WHY?Here, we have extensive research and interest in effective communication; for example, the NSW Garling enquiry was set up

after a young girl was hit by a golf ball and died in Royal North Shore. There was a huge state wide enquiry which looked at safety and a lot of new initiatives were then brought in including training.

WHAT WOULD YOU LIKE TO IMPROVE IN AUSTRALIA THIS YEAR?I would like a better under-standing of the complexities of health care rather than thinking about simplistic approaches to problems. People tend to sensationalise when there is a mistake but I think it is important for everybody to understand how difficult it is to operate in those departments, how communication actually works, as opposed to how we think it works, and to recognise it involves multiple people, systems and time frames. There isn’t one formula, each and every patient and clinician interaction is unique.

THE UTS PROJECT’S RECOMMENDATIONS:• Achieve a balance between

medical and interpersonal communication

• Provide explicit explanations to patients about processes and procedures in the ED

• Develop effective interdisciplinary teamwork

• Further develop cross-cultural communication awareness and strategies

• Provide training with authentic materials

• Examine communication in clinical handovers

This is a particularly leafy part of the North Shore, where you and your little ones will be spoilt for choice with parks or play spaces. Unique to St Ives is the Ku-ring-gai Wildflower Garden. I can remember enjoying it as a child, and my little one loves it too. It has many picnic and play areas, walking tracks for all abilities, and the chance to see flowers and animals which you don’t normally see around the suburbs. The best playground is probably Lambert’s Clearing, featuring play equipment, BBQ, picnic tables and toilets. But the best thing is the little rainforest house which is just perfect on a hot day. Bring your bikes or scooters and enjoy an hour or two around the Wildflower Gardens.

Not too far away from the St Ives Shopping Village is the Memorial Avenue Reserve. From the cute little pencils at the entrance to the brightly coloured equipment, this inviting park has much to offer. There are trees to climb, plenty of space for running around, but no toilets.

Finally, I’d recommend having a search for some of St Ives’ tiny little parks. Some can feel so hidden and all have their own character. Check out Melaleuca Park which falls literally in the middle of Melaleuca Drive; enjoy the relaxed Athena Avenue Reserve; spend a few minutes in the pleasant Yarrabung Reserve; or play hide and seek amongst the trees in Bede’s Forest. These little parks are a great treat for any young ones!

The nature of retirement is changing and so are the rules. If you want to reduce your working hours or even pay less tax with-out sacrificing your income, a transition to retirement pension could be the answer.

Longer life expectancies mean many Australians are spending more time in retirement than ever before, increasing the burden on our social security system and emphasising the importance of accumulating superannuation.As a consequence, the Government is encouraging us to remain in the workforce beyond the traditional retirement age.

DID YOU KNOW THAT YOU CAN ACCESS YOUR SUPERANNUATION WHILE YOU ARE STILL WORKING?Using a transition to retirement pension you can generally access between 4-10 per cent of your super balance, as long as you’ve reached your preservation age (between 56 and 60 depending on the year you were born) and you are still working, up to age 65.

A BETTER LIFESTYLE ON THE SAME INCOMEA transition to retirement strategy allows you to supplement your income by drawing a regular pension payment from your super fund. There are a few ways you can benefit:• continue to work full-time but

I am so excited. The facilities and the phenomenal acoustics of the Concert Hall are really something to look forward to. The last time I performed a solo concert in Sydney was at the Sydney Opera House. I decided the Concourse would be perfect for my new tour ‘This Time Tomorrow.’

HOW DID YOU MEET ANDRE RIEU?My Aunt sent an email to him to let him know that she had a niece in Australia who would be a perfect addition to his concerts. He immediately called me at home, invited me to Maastricht to meet him. So I flew to Maastricht the next day, auditioned for him in his studio and two weeks later I was on stage with him in Boston, in the

USA! The rest is history, really! I toured extensively with Andre for many years and still, to this day, perform with him when I am not touring solo.

WHAT ADVICE WOULD YOU GIVE TO ANY LOCAL NORTH SHORE YOUNG PEOPLE KEEN TO DEVELOP THEIR SINGING TALENT?The most important thing is to believe in yourself and your talent, and don’t let anyone tell you that you are not good enough. Listen to criticism and do with it what you can but never give up on your dreams. I had been told so many times that I would never make it and that I was too ugly. Luckily I never listened and kept on doing what I loved!

A comfortable three hours’ drive from Sydney’s North Shore is Moby’s Beachside Retreat located across the road from Boomerang Beach, a picturesque beach with postcard views. Moby’s was the perfect place for a weekend escape for my friend Rachel and me.

After arriving late afternoon, the staff at check-in suggested we book a table for dinner at Moby’s on Redgum, so after unpacking the car and enjoying the quiet of the Beach Hut we wandered back down the resort road towards the restaurant positioned right beside the pool. It’s a pleasant atmosphere and the servings were large enough that we could have shared theentrees. The food was delicious and it was lovely not to have to drive anywhere.

A short 20-minute drive later we were wandering around the shopping centre. We spent lunch at Beach Bums Café, a relaxed place to eat with tables directly on the beach edge. We both enjoyed a pulled lamb roll (we left most of the bread on the plate as we’re enjoying the benefits of a paleo diet).

With a couple of DVDs in hand we opened a bottle of red and basked in having no little people to manage for the usual afternoon/evening routines. Thankful for capable husbands at home being Dads, we cooked a simple dinner and enjoyed the night. Sunday morning, after another sleep-in and a morning cuppa, we checked out and headed back to Foster for a beautiful breakfast at Tartt Café

This is a story of hope, compassion, community, love and hard work.

Emily came into the world after a long, arduous labour, during which she was affected by hypoxic-ischemic encephalopathy – brain damage caused by low oxygen. Emily suffers from vision impairment, cerebral palsy and uncontrolled epilepsy. Together Claire (mum), Ben (dad) and Emily work with a physiotherapist and attend hydrotherapy to keep her muscles developing. Early intervention is key.

WHEN DID YOU FIRST REALISE THERE WERE COMPLICATIONS?When they checked me during labour and couldn’t detect a heartbeat. I knew things weren’t right but you just deal with it. You have a beautiful little girl who is relying on you and trying her damnedest so you just do your part and be there.

CAN YOU DESCRIBE A TYPICAL WEEK?Our week fluctuates but on a Monday we have either paediatrician, neurology, rehab clinic, doctors, eye clinic or feeding clinic. Then Tuesdays tend to be hydrotherapy. On Wednesday we often have RIDBC (Royal institute for Deaf and Blind Children) come and do vision therapy. Thursdays is occupational therapy and physiotherapy at Cerebral Palsy Alliance. Once a month on Friday I attend a special needs mothers group. Emily handles this relent-less schedule with grace and patience.

WHAT HAS BEING A MOTHER TAUGHT YOU?I never knew the full capacity and variety of love before Emily. I have no doubts when it comes to her. I appreciate life and loved ones like never before. She made me fall in love with my husband all over again and on a deeper level which I didn’t think was possible. I was scared at the beginning our situation would ruin my family and tear us apart but Emily did the opposite, she made us stronger. I have never felt more love, strength, support or purpose than I do now and she taught me to hang on to hope and I plan to spend the rest of my life giving it to her.

WHAT HAS BEING A FATHER TAUGHT YOU?It has taught me patience, persistence, hope, and unconditional love. Patience, with the screaming that we receive every day from Emily, and who can blame her, if I had multiple seizures every day, I’d say I would be pretty upset too, and I’m 35, she’s not even two years old. Persistence and hope, that maybe with assisted help she’ll possibly walk one day or, even more importantly, maybe communicate with us. Lastly, love. It’s taught me love like I’ve never experienced before. I love my wife and understand what love is but this is differ-ent, not greater, more or less, just different. She’s a part of me, I helped create her. She’s my responsibility. My unconditional love.
