Lockout/Tagout (LOTO)Control of Hazardous Energy

Maintenance of Equipment

Where maintenance, repairing, cleaning, ser- vicing, adjusting, or set-up operations can’t beaccomplished with the energy source discon-nected, such operations may only be per-formed under the following conditions:

The operating station where the machinemay be activated must be under the controlof a qualified operator.

All participants must be in clear view of theoperator or in communication with eachother.

All participants must be beyond the reachof machine elements that may move rapidlyand present a hazard.

Where machine configuration or size re-quires that the operator leave the controlstation to install tools, and where there aremachine elements that may move rapidly, ifactivated, such elements must be separatelylocked out. During repair procedures where mechanicalcomponents are being adjusted or replaced,the machine shall be de-energized or dis-connected from it’s power source.

LOTO Inspections

An authorized employee must perform in-spections of the energy control proceduresannually. The inspections must reviewLOTO procedures and correct any issues.

Removal of LOTO

A system shall be in place that addresses de- vice removal if the authorized employee whoapplied the LOTO device is unavailable to re-move it. The employee’s direct supervisor mayonly remove the employee’s LOTO device,after necessity has been established. The pro-cedure shall include at least the following:

The employee’s direct supervisor shall inves-tigate the situation and verify that the au-thorized employee who applied the device isnot at the facility.

All reasonable efforts shall be made to con-tact the authorized employee to informhim/her that his/her LOTO device will beremoved.

The authorized employee’s direct supervisoris certain that removal of the LOTO device will not endanger employees. The authorized employee’s direct supervisor will complete the removal of the LOTOform.Prior to resuming work at the facility, theauthorized employee shall be notified thathis/her LOTO device was removed inhis/her absence.

Introduction

Whenever performing service or maintenanceon any machine where an unexpected start-upor release of stored energy could cause injury,lockout/tagout procedures must be followed.

Lockout/Tagout ProcedureNotify affected employees that a LOTO sys-tem is going to be utilized and the reason itis necessary. The authorized person shouldknow the magnitude of energy that the ma-chine utilizes and shall understand the asso-ciated hazards. If the machine is operating, shut it down bythe normal stopping method.

Operate the switch, valve, or other energyisolating device so that the machine is iso-lated from its energy source(s). Stored en-ergy must be dissipated or restrained bymethods such as repositioning, blocking,bleeding down, etc. Lockout and tagout the energy isolating de-

vices with assigned individual lock(s) andtag(s).

After ensuring that no personnel are ex-

posed, and as a check on having discon-nected the energy sources, operate the pushbutton or other normal operating controls tomake certain the equipment will not operate.CAUTION: Return operating controls toneutral or off position after the test.

The equipment is now locked and taggedout.

Restoring to Normal Operation

After the servicing is complete and equipment isready for normal operations, check the area aroundthe equipment to ensure no one is exposed.

After all tools have been removed from the equip-ment, guards have been reinstalled, and personnelare in the clear, remove all LOTO devices. Oper-

ate the energy isolating devices to restore energy tothe equipment.

LOTO - Multiple LocksIn the preceding steps, if more than one individual isrequired to LOTO equipment, each shall placehis/her own personal LOTO device on the energyisolating device(s). When an energy isolating devicecannot accept multiple locks or tags, a multipleLOTO device (hasp) may be used. If lockout isused, a single lock may be used to lockout the equip-ment with the key being placed in a lockout box orcabinet that allows the use of multiple locks to secureit. Each employee will then use his/her own lock tosecure the box or cabinet. As each person no longerneeds to maintain his/her lockout protection, thatperson will remove his/her lock from the box orcabinet.

Temporary Removal of LOTO Device

In situations where LOTO devices must betemporarily removed from the energy isolat-ing device and the energized equipment, thefollowing sequence of actions will be fol-lowed.

Remove non-essential items and ensure

that equipment components are operation-ally intact.Notify affected employees that LOTO de-

vices have been removed and ensure thatall employees have been safely positionedor removed from the area.Have employees who applied the LOTOdevices remove the LOTO devices.Energize and proceed with testing or posi-tioning.De-energize all systems and reapply energycontrol measures in accordance with theLOTO system procedure described earlierin this brochure.

Different Types of Locks The type of energy that needs to be isolated willdetermine which type of lockout device shouldbe used. Some different types are shown below.Be sure that all energy sources are locked out.
