Who is HomeTown Health (HTH)? HomeTown Health, LLC (HTH) is an organization that was started 18 years ago to pursue ways to help rural

and small hospitals survive in this environment of tremendous budget cuts from the state and federal level.

With hospital members across various states, HomeTown Health also works with a number of hospitals

through grant work and education programs nationwide. In 2016, the Iowa Department of Public Health

(IDPH) decided to follow the lead of other states and issue a Request for Proposal to contract out the manage-

ment, oversight and coordination of the SHIP program activities and services. HomeTown Health, LLC was

selected through the competitive process to help manage and provide coordinated services for the Iowa SHIP program for years 2016-

2019. As Project Director/Grant Manager, HomeTown Health's responsibility is to provide comprehensive management and oversight of

the SHIP Program, such as reporting, budgeting and managing other grant requirements with HRSA. It also includes the coordination of

group purchases of goods and services that align with the participating hospitals’ designated activities, as submitted/requested on the

SHIP applications. HTH was tasked by the IDPH with developing one or more consortiums that will meet the needs of participating

SHIP hospitals to achieve savings and create efficiencies through brokering group purchasing activities or delivery of services. Like the

IDPH, we believe that this will simplify the SHIP administrative tasks for hospitals and better utilize funding to achieve program goals

and outcomes for participants. During Grant Year 2016-2017, HomeTown Health developed three consortiums based on hospital needs

and Pre-Assessment feedback, and also subcontracted training services out to various vendors, such as Global Partnership for Telehealth

and Health Management Associates. HTH also coordinates training, education, and other purchases that meet SHIP requirements for

hospitals with a variety of other “in-the-field” organizations and Business Partners that serve small hospitals.

What type of education does HomeTown Health provide? HomeTown Health also offers a variety of accredited education via live events, webinars, and online courses

through their education arm, HomeTown Health University (hthu.net). Healthcare professionals in a variety of

hospital realms, such as clinical, business office, IT, HIM, administration, and directors have access to more

than 500 online professional development courses. In addition, HTHU works to build custom programs for

other healthcare facilities, students, associations, and service providers. Many courses offer IACET accredited

CEUs, meeting requirements for professional organization continuing education. Schools and programs on

HTHU include:

Who is the Iowa Transformation Consortium (ITC)? The Iowa Transformation Consortium (ITC) was formed in 2016 as a part of

the Small Hospital Improvement Program (SHIP) Grant funded through

HRSA. The Federal SHIP Office encourages grantees to maximize the impact

of the SHIP Grant through the formation of networks or consortiums. These

consortiums are intended to support the same activities for all SHIP hospitals

or a variety of activities based on SHIP hospital needs, either through group

purchasing or specific training and educational activities. By pooling funds together, hospitals are able to take advantage of more

services than they might on their own. The Iowa Transformation Consortium is a group of hospitals and other stakeholders that work

together to provide support and reduce barriers for small rural hospitals in three primary focus areas: Quality/HCAHPS, Revenue

Cycle and Telemedicine. Participating hospitals were asked to select one consortium as their primary focus area, while also gaining

access to resources in the other areas. 72 small Iowa hospitals are participating in the Consortium this year, with a focus on laying the

foundation of education to meet initial needs and for future grant years in those three areas.

Sandy Sage, RN, previously worked for a rural hospital as a Revenue Cycle Manager. She has been a Regis-tered Nurse since 1990. Her clinical experience includes 6 years of experience as a Nurse Manager in the Emergen-cy Department and for a Medical/Surgical unit at Taylor Regional Hospital. Working as a Case Manager at Flint River Community Hospital, when Medicare first implemented OPPS, allowed her to work closely with the Business Office staff to negotiate the new rules for outpatient reimbursement. Analyzing charging and reimbursement led to her role as the clinical liaison between the medical and accounting sides of the hospital. Sandy also worked at Fairview Park Hospital as a Revenue Analyst from 2001 to 2010. Educating hospital employees and helping them to better under-stand the revenue cycle is her passion in her professional life. She became an official member of the HomeTown Health team in November of 2016.

8:45 - 9:30 am Session 1: Transforming Your Hospital: Lessons Learned from IRCTC Year One

Sandy Sage, RN, Revenue Analyst, and Jimmy Lewis, Chief Executive Officer - HomeTown Health, LLC.

In this session, Sandy Sage will provide a summary of what the IRTC team learned from both individual assessments and during the program year. She will reveal common needs and solutions for Revenue Cycle Management and Coding among the group, and will review some of the resources that were shared among the Consortium so that all members might benefit from them.

* Describe the financial state of small, rural hospitals on the national level

* Identify common themes of need among Iowa small hospitals in the areas of Revenue Cycle Management & Coding

* Compare and contrast the financial health of small Iowa hospitals against others nationwide

* Recall suggested resources available for the IRCTC hospitals.

Jimmy Lewis has roots in rural South Georgia where he was born in Cordele, Georgia. Having graduated high school in Cordele, he attended Middle Georgia College and then Auburn University. He received a Bachelor's Degree in Industrial Engineering from Auburn. Jimmy has held senior management positions in six Fortune 500 companies. He has been instrumental in passing key rural legislation and regulation for hospital based nursing homes, state mer-it, PPS and critical access hospitals. All of these have been key to the survival of rural hospitals and rural health care in Georgia. Under his leadership, HomeTown Health has grown to a Georgia-based virtual company where 60+ hos-pital members and 60 business partners cumulatively have over $800 million in sales. HomeTown Health activities range from the identification of best practice solutions, to education, collective purchasing, developing managed care strategies, reimbursement advocacy, as well as legislative representation. HomeTown Health's efforts have been very instrumental in saving many of Georgia's rural hospitals. Jimmy is married with two grown sons. He and his wife, June, live in Cumming, Georgia.

Jenan Custer, CCS, CPC, CDIP - Healthcare Coding and Consulting Services (HCCS)

Jenan Custer will provide this education session that will encompass how to transform your coding department through industry best practices and audit compliance. She will provide your facility with the tools to be successful and mitigate risk.

* Describe the importance and general impact that coding compliance in ICD-10 has on small hospitals

* Identify common areas of coding errors and how to properly measure areas of risk

* Identify the implications associated with auditing injections, infusions, and erroneous modifiers

Jenan Custer is an experienced and highly decorated Director, speaker, and leader in the Coding and Health Information Management industry. Her passion for the industry is contagious and permeates through the crowds as she speaks at National conferences and local engagements. Her presentations are both educational and fun; don’t be surprised if she incorporates an entertaining current-event to keep the audience engaged! She began her career as a coder over 14 years ago; through the years she has earned several certifications including the CCS, CPC, CDIP, AHIMA ICD-10-CM/PCS Approved Trainer, AHIMA Ambassador, and her AS in Health Information Management. Jenan provides coding education and training for facilities in Florida, Georgia, and Iowa. Currently, Jenan is the Director of Coding at HCCS (Healthcare Coding and Consulting Services) based in Fort Myers, FL. In this role, she oversees a coding team comprised of more than 200 remote coders and provides consulting and coding services for over 100 facilities all across the United States.

Sandy Sage, RN, Revenue Analyst, and Kristy Thomson, Chief Operating Officer - HomeTown Health, LLC., & the Iowa Revenue Cycle Transformation Consortium (IRCTC) Team of Experts

Each part of the revenue cycle within a small hospital is tied to the other, and can influence the overall financial health of the hospital. In this interactive session working in small groups, you will prepare a stra-tegic plan and recommendation to improve hospital financials based on a sample scenario. Together with the IRCTC team of experts, priorities and suggestions for solutions to common revenue cycle challenges will be shared and discussed.

*Describe various areas of the revenue cycle and their dependence on one another for financial improve-ment

*Identify best practice solutions to improve common Revenue Cycle challenges

Kristy Thomson joined the HomeTown Health team in 2006 and served for 9 years as the Director of Education where she has directed the education services offered to HomeTown’s hospital members and their affiliated clinics and physician practices. In this role, Kristy was responsible for course curriculum, content, technical set up and instructors, as well as directing live and online education services offered to HomeTown’s network of 60+ rural hospitals and their affiliated clinics and physician practices along with 60 supporting business partners. She was also responsible for compliance with the IACET/ANSI continuing education and training standards along with other accreditation standards and assisted in preparation, review and administration of state and federal grants. In 2015, Kristy moved into the Chief Operating Officer position for HomeTown Health where she is responsible for both the HomeTown Health operations and the education programs of HomeTown Health University. After graduating from Stephens County High School in Toccoa, Georgia, Kristy earned her degrees including a B.S. from Clemson Univer-sity and an M.S. from the University of Georgia, both in math and sciences. Prior to joining HomeTown, she worked as Special Projects Manager for the Town of Jupiter in Jupiter, Florida. She currently lives in Clarkesville, Georgia with her husband, John Lee, and her two sons, Jack and Luke. Among other community and industry related arenas, she currently serves as IACET Awards Committee Member and Georgia Medicaid RAC Provider Relations Commit-tee Member.

IRCTC: Revenue Cycle & ICD-10 Sessions

Day One: Wednesday, March 15, 2017

1:00 - 2:00 pm Session 4: Using your DDE To Transform Focus: Reimbursement Practices that Look Forward

Dale Gibson, Medicare Consultant, and Leanne Layne, Independent Hospital Consultant

Direct Data Entry (DDE) process allows providers to verify eligibility and check claim status online. In this session, Dale Gibson and Leanne Layne will discuss tips on how to get the most out of your DDE when managing denials and ultimately how to best utilize this data to improve your revenue cycle processes.

* Describe how to access the DDE for your hospital

* Identify key patient data that can be found in the DDE

* Recognize denial reason codes in the DDE and how to respond

* Identify methods of utilizing DDE data to prevent future denials

Dale Gibson started his own Medicare consulting business in 1997, and is now well -known among HomeTown Health’s network as the “Medicare Expert”. Dale began his healthcare career in 1980 as a Business Office Manager for a large hospital in Fort Walton Beach, FL that was part of Humana. He briefly worked for HHL in Miami Florida in 1989, be-fore returning to Humana as a Corporate Consultant for Business Offices. In 1993, he became a Medicare Consultant through a merger with HCA. Dale completed his B.S. from Troy State University. He resides in Pensacola, Florida.

Leanne Layne graduated from the University of Georgia with a BA in Political Science and an MBA in Business. She has worked in physician office settings as a Front Office Manager and Site Manager. Leanne has also worked in the hospital setting as a part of the accounting department and also as a Business Office Director. Now, she works as a consultant with Dale Gibson and works to keep up with Medicare Advantage plan changes. She has a passion for helping hospitals each day.

2:00 - 3:00 pm Session 5: The Transformation of Denials through Contract Management

Becky Tarr, RN, MBA, CPA, Executive Vice President & Chief Operating Officer - MedPerformance

A denial is a refusal to pay as a result of the provider not adhering to insurance company policies. This makes the management of contracts critically important to proactively preventing and managing denials. In this session, Becky Tarr, RN, MBA, CPA will discuss strategies for managing payor contracts that can also improve denials. She will also discuss the benefits hospitals might see from well-developed managed care contracts.

* Describe how understanding a hospital's managed care contracts can help to be proactive in managing denials in payment

* Identify areas of managed care contracts that are important to help to avoid denials

* Identify ways to manage hospital denials throughout the revenue cycle

Rebecca Corzine Tarr, RN, MBA, CPA has over 25 years of experience in health care, which includes 22 years in Revenue Cycle / Case Management at four different acute care hospital systems. She is currently the Owner, Executive Vice President and Chief Operating Officer of MedPerformance, a Revenue Cycle Consulting Company that specializes in denials and appeals through software and consulting services. Prior to forming MedPerformance, she was the Associate Vice President, Revenue Cycle, at a multi-hospital system in Florida for over six years. At this system, she had adminis-trative responsibilities for Managed Care, Centralized Scheduling, Pre-Registration, Patient Registration, Health Infor-mation Systems, Clinical Documentation Program, Case Management, Appeals and Denials, CDM, Revenue Integrity, and Patient Financial Services. She has both for-profit and not-for-profit health care experience and during this same time period, she was an adjunct faculty member at the University of South Florida, College of Business, where she taught in the Finance Department for over 20 years. Ms. Tarr received her Bachelor’s degree in Nursing from Loyola University in Chicago, a Bachelor of Science degree in Accounting and a Masters in Business Administration from the University of South Florida with a concentration in Finance. She also has postgraduate hours in accounting and finance. Mrs. Tarr holds both a Florida CPA and RN license, is married and lives in Valrico, Florida. She has three grown children and five wonderful grandchildren.

Swing bed programs can improve occupancy and productivity in addition to increasing facility revenue. How-ever, swing bed programs will do more harm than good if hospital leadership do not understand very specific skilled nursing regulations. The ability to utilize swing beds for CAHS not only reduces the Medicare costs per patient bed day in these facilities, but increases revenues and margins that can help support population health, wellness, and other services. In this session, Kerry Dunning will provide training and resources in order to help hospital staff to increase utilization and revenue for post-acute care services.

Ms. Dunning worked for HCA and HealthTrust hospitals in administrative roles; Horizon Rehabilitation and Service-Master Rehabilitation as a Sr. Vice President and Chief Operating Officer; with GPS Healthcare as the Chief Senior Ser-vices Officer; and has spent more than 20 years as an independent consultant. In addition to serving as an Adjunct In-structor in the College of Health at the University of North Florida, Ms. Dunning regularly leads workshops and webinars regarding Medicare, skilled nursing (including MDS), swing bed programming, and reimbursement cycle improvement. Sherri Ackerman joined the HTH/HTHU team in April of 2011 as HomeTown ’s liaison to the Georgia Health Infor-mation Technology Extension Center (GA-HITEC). Sherri works with grant funded program to support rural hospitals in their move to meaningful use of electronic health records and as an outreach and education partner, coordinating health IT live training events and webinars as well as providing courses on the HIT Online Training Center. Sherri holds a Bachelor’s of Arts degree in Journalism from the University of Georgia. Previously, she led Client Services at an Atlanta advertising agency, specializing in recruitment. Sherri and her husband, Nat, have 3 children, Troup, Sela, and Milli and reside in Alpharetta where she is active in Mount Pisgah UMC, her children’s schools, and several com-munity organizations including a volunteer group working on projects and events for the city of Alpharetta.

Kristy Thomson joined the HomeTown Health team in 2006 and served for 9 years as the Director of Education where she has directed the education services offered to HomeTown’s hospital members and their affiliated clinics and physician practices. In this role, Kristy was responsible for course curriculum, content, technical set up and instructors, as well as directing live and online education services offered to HomeTown’s network of 60+ rural hospitals and their affiliated clinics and physician practices along with 60 supporting business partners. She was also responsible for compliance with the IACET/ANSI continuing education and training standards along with other accredi-tation standards and assisted in preparation, review and administration of state and federal grants. In 2015, Kristy moved into the Chief Operating Officer position for HomeTown Health where she is responsible for both the HomeTown Health operations and the education programs of HomeTown Health University. After graduating from Stephens County High School in Toccoa, Georgia, Kristy earned her degrees including a B.S. from Clemson Universi-ty and an M.S. from the University of Georgia, both in math and sciences. Prior to joining HomeTown, she worked as Special Projects Manager for the Town of Jupiter in Jupiter, Florida. She currently lives in Clarkesville, Georgia with her husband, John Lee, and her two sons, Jack and Luke. Among other community and industry related arenas, she currently serves as IACET Awards Committee Member and Georgia Medicaid RAC Provider Relations Committee Member.

After graduating with a Masters of Social Work degree from Valdosta State University Sherrie Williams began working in the private mental health field and public school system. For nearly 15 years, she witnessed firsthand what happens to children and families when necessary medical and mental health care is out of reach. As more and more rural health providers disappear and fewer specialists move into rural locations, Mrs. Williams understands the critical nature of strengthening and expanding the telehealth network. By working with existing and potential partners to implement and design sustainability strategies for telemedicine programs, she believes that all will remain connected to healthcare.

Lloyd Sirmons received his Associates Degree in Computer Science from Columbus State University in1998. He immediately began his IT career working with TSYS of Columbus, GA where he worked as a mainframe program-mer. He worked a number of years with Columbus Regional as a System’s Analyst before entering into full time ministry where he served in pastoral leadership for nearly 14 years before joining the GPT team in August of 2010 as the BTOP Project Director. He completed his BS degree in Interdisciplinary Studies from Liberty University in 2009. Lloyd not only brings a sound knowledge of IT solutions, but also brings strong leadership and communication skills as the Director of the Southeastern Telehealth Resource Center.

Jonathan Neufeld, PhD, HSPP, is Director for the Great Plains Telehealth Resource and Assistance Center, or gpTRAC. Here he directs the team in their four-part mission to build telehealth awareness, promote education, pro-vide individualized consultation, and provide data specific to telehealth services in our region. From 2011 until 2016, he served as the Clinical Director of the Upper Midwest Telehealth Resource Center, a consortium of telehealth providers located in Illinois, Indiana, Ohio, and Michigan where he provided administrative oversight along with specific consultation in technological and administrative aspects of telehealth and clinical telemental health services. Dr. Neufeld has consulted on a wide range of projects related to rural health and telehealth over the past 10 years. In addition to his work with gpTRAC, Dr. Neufeld is a Licensed Clinical Psychologist in Indiana and California. He has operated a clinical health psychology practice in Goshen, Indiana and has been the Vice President of Information Technology and Integrated Care for Oaklawn Psychiatric Center, an 850-FTE Community Mental Health Center serv-ing Elkhart and St. Joseph counties in Northern Indiana.

Dr. Kevin Glenn is a decorated and honorably discharged member of the United States Navy. He has been with the University of Iowa since his days as a medicine resident and then Chief Resident. Following that he was a hospitalist and basic science researcher. Dr. Glenn recently received his Masters in Healthcare Delivery Science from Dartmouth College. In 2016 he became the Program Director of the Adult Hospitalist Program at University of Iowa Hospitals and Clinics. The Program, staffed by more than 40 physicians and 11 Advanced Practice Providers, pro-vides care for adult patients admitted to the University of Iowa Hospitals and Clinics and to the Iowa City VA Medical Center

Kate Klefstad is a Digital Health Service Line Manager in University of Iowa Health Ventures’ Signal Center for Health Innovation. She holds an undergraduate degree in Health and Human Physiology from the University of Iowa and a Masters of Healthcare Administration from the University of Iowa College of Public Health. Kate manages telemedi-cine strategy and implementation for several UI Healthcare service lines, as well as the telemedicine relationship between UI Healthcare and the hospitals in their Critical Access Hospital Network.

Ray Brownsworth is the CEO of Van Buren County Hospital in Keosauqua, Iowa where he has served since 2014. He has been in healthcare for 30 years of which 19 years have been in a rural setting. Ray has an MBA from the University of Dallas.

Rhonda Fellows is from Van Buren County Hospital in Keosauqua, Iowa where she has served as CNO most recently and prior to that as ACNO and Performance Improvement Coordinator. She has been in healthcare for 24 years. Prior to working with VBCH, Rhonda specialized in Maternal and Child Health as well as being a Lactation Specialist. In this session, Dr. Jean Sumner will join the session remotely to bring her expertise in the developing use of telemedicine in rural areas. As a former Georgia Composite Board Member, and former Telehealth skeptic, she will discuss the need for high quality clinical care and implications for the use of telemedicine.

Jean Rawlings Sumner, M.D., who is a member of Mercer University School of Medicine's first graduating class and a practicing physician in Washington and Johnson counties, has been leading a major telehealth initiative for the School of Medicine as associate dean for rural health. Dr. Sumner began her tenure as dean of the Mercer Univer-sity School of Medicine on July 1, 2016. A 1986 graduate of the School of Medicine, Dr. Sumner has been a communi-ty faculty preceptor for Mercer University School of Medicine since completing her residency in 1989 at the Medical Center of Central Georgia/Navicent Health in Macon, Georgia. She has served as hospital chief of staff and nursing home medical director, as well as a board member, chair and president of numerous community and statewide profes-sional organizations. At the state level, Dr. Sumner has served most recently as a governor-appointed member, then president and medical director, for the Georgia Composite Medical Board. Prior to attending Mercer University School of Medicine, Dr. Sumner earned Bachelor of Nursing and Master of Nursing degrees from the Medical College of Georgia School of Nursing. Dr. Sumner is extremely passionate about rural health and access to care. A native of Sandersville, Georgia, Dr. Sumner is a third-generation Georgia physician. She is married to Joe Sumner, a pharma-cist, and they have two children and four granddaughters. Dr. Sumner enjoys raising chickens, fishing, art, and creat-ing pottery.

Jimmy Lewis has roots in rural South Georgia where he was born in Cordele, Georgia. Having graduated high school in Cordele, he attended Middle Georgia College and then Auburn University. He received a Bachelor's Degree in Industrial Engineering from Auburn. Jimmy has held senior management positions in six fortune five hundred com-panies. He has been instrumental in passing key rural legislation and regulation for hospital based nursing homes, state merit, PPS and critical access hospitals. All of these have been key to the survival of rural hospitals and rural health care in Georgia. Under his leadership, HomeTown Health has grown to a Georgia-based virtual company where 60+ hospital members and 60 business partners cumulatively have over $800 million in sales. HomeTown Health activities range from the identification of best practice solutions, to education, collective purchasing, develop-ing managed care strategies, reimbursement advocacy, as well as legislative representation. HomeTown Health's efforts have been very instrumental in saving many of Georgia's rural hospitals. Jimmy is married with two grown sons. He and his wife, June, live in Cumming, Georgia.

After graduating from Early County High School in Blakely, GA, Meghan completed a B.A. in Speech Communications from the University of Georgia. Meghan’s career in healthcare began as a Human Resources Director in southwest Georgia where she was responsible for all aspects of employee rela-tions, payroll/benefits, workers compensation, annual education development and training, as well as serving as the HIPAA Compliance Officer. Jennie Price joined the HomeTown Health team in May 2014 with experience in healthcare risk management and marketing, and having worked with a number of member hospitals. Jennie’s specialties, outside of marketing, include professional liability such as medical malpractice, and property/casualty insurance. She holds a Bachelor’s degree from Berry College in Rome, GA in Communication, as well as Bachelor of Arts degree in Religious Studies. She has earned a variety of designations in the area of insurance and risk management, including the Accredited Advisor of Insurance (AAI), Registered Professional Liability Underwriter (RPLU), and Associate in Risk Management (ARM). Jennie lives in Decatur, GA with her husband Jon, but they spend many weekends at their cabin in the mountains “disconnecting” from the city with their 1 year old daughter, two dogs, family and friends.

Angie Charlet, Director of Quality Services - Illinois Critical Access Hospital Network (ICAHN)

Accurate, reliable, and timely information is vital to effective decision-making. When it comes to hospital quality data, this is also true, and the reliance on data for improving quality is ever-increasing. In this ses-sion, Angie Charlet will discuss common challenges related to data collection in small hospitals, and provide recommendations for tools and solutions to overcoming these challenges.

Angie Charlet, RN, holds a Master ’s in Healthcare Administration with a focus in performance improvement. She has over 22 years of experience in the healthcare industry, with most recent experience as Operations Officer prior to joining ICAHN in 2011 as the Director of Quality Services. Her past experiences demonstrate team building, network engage-ment, and evidence-based practice changes along with new models that promote rural health. Ms. Charlet sits on the Board of Directors for the National Association of Rural Health Clinics and is the president-elect (2013) for the Illinois Organization of Nurse Leaders. Angie holds a Master’s degree in Healthcare Administration and a Doctorate in Busi-ness Administration, Healthcare Leadership.

The patient experience is shaped by everyone on the health care team; but how do we keep that team en-gaged and purposeful? In this session, Jeanine Davis will discuss the leaders role in driving culture change and providing ongoing support for team members, including nurses, administrative staff, physicians, supervi-sor and others.

Jeanine Davis, RN, BAAS, CMCN, has more than 20 years of hospital, medical and managed care experience in healthcare organizations and is a Principal at Health Management Associates (HMA). Jeanine’s clinical experience is far-reaching and includes telemetry, management of home health care, director of utilization review, care/case coordina-tion, discharge planning, and quality management in direct care environments. She can provide expertise, guidance, advice and support in business planning, strategy, organization, staffing and medical operations in addition to writing policies and procedures, reviewing regulatory compliance and assessing staffing needs and resources. Jeanine joined HMA after serving as the director of Health Services Clinical Program - Dual Demo Medicare/Medicaid Programs at Amerigroup, part of the Anthem Government Business Division (GBD). In this role, she was responsible for coordination, creation, development, production, maintenance, and management of all clinical programmatic aspects of the three-way agreement to serve the dual eligible population simultaneously enrolled in Medicare and Medicaid Programs (MMP). She led the clinical program for all four of the MMP demonstration states — Virginia, New York, California, and Texas — and successfully launched three of the four MMP market implementations: California, New York and Texas. Previously, she was the chief operating officer of Georgia Health Plan where she directly managed coverage for approximately 325,000 people. From April 2009-December 2013, Jeanine served as the chief operating officer of New Mexico Health Plan, with a membership of 25,000 Long-term Supports and Services (LTSS) and Aged, Blind and Disabled (ABD) members. She is a member of the American Association of Managed Care Nursing and the American College of Health Care Executives. Jeanine earned her nursing degree from Centennial College in Scarborough, Ontario and her bachelor of applied arts and science degree from the Incarnate Word University.

Susannah Cowart joined Hometown in April 2013 and serves as the Director of Customer Relations. A native of Tifton, Georgia, she attended Abraham Baldwin College and continued to Brenau Women’s College in Gainesville, Georgia. Previously serving in a critical access hospital for 8 years, Susannah has diverse healthcare experience in materials management, marketing and community relations. Susannah and her husband, Jake, currently reside in Lake Placid, Florida.

During this session, Brain Gunning will lead a discussion around the proper use of video technology during a clinical encounter. Looking at how environmental configurations, camera placement, and microphones set-ups can be used effectively to ensure a quality professional clinical encounter. We will explore a variety of devices to make connections and discuss specific use case and techniques while using these devices from mobile tablets and fixed endpoints. The session will also include a series of use case and scenario demonstrations to help ensure understanding of ways to properly use the video systems to conduct clinical encounters.

For the past six years, Brian S. Gunnell has served as the senior video-conferencing engineer for the University of Vir-ginia Center for Telehealth. A graduate of Virginia Tech, Brian brings extensive experience in mobile and fixed endpoint vtc technology, communications and telepresence infrastructure as well as the clinical applications of mobile telehealth. Brian has played a vital role in the conception, development and application of the University’s emergency telemedicine capabilities, including the management of special pathogens, tele-stroke and neonatal interventions as well as the appli-cation of mobile vtc to enable NIH stroke scale application while the patient is in route. In addition, he has substantial expertise in developing videoconferencing in low resource environments and has spearheaded telemedicine program development in Guatemala, Tanzania, China, Bangladesh and in Appalachian Southwestern Virginia. Brian has published in the area of mobile tele-stroke application, the use of telemedicine in response to the Ebola virus and developed and published a widely used portal assessment tool for use in global public health initiatives.

An overview of the anticipated program design and services available to hospitals, live meetings, and options for individual project selection will be shared. Revenue Cycle experts in attendance and Hospital attendees will be asked to provide feedback on the top-ics up for consideration, and to offer suggestions for the upcoming year.

This session is recommended for attendees from the revenue cycle, business office, or other financial departments within the hospital.

New payment models are increasing the ties between reimbursement and quality care. One model, the CMS Medicare Transitional Care Management Services program, provides for new methods of reimbursement when reporting care management services provided to patients following discharge from a hospital, skilled nursing facility or community mental health center; outpatient observation, and partial hospitalization. In this session, Angie Charlet will provide an overview of the TCM services so that participants can consider or grow their participation in the program.

* Describe the requirements for the Medicare TCM Services

* List required components of a TCM program

She has also provided project support and program implementation in the areas of care management, population health, emergency department, occupational medicine, behavioral health, senior care, community health services, medical homes, telehealth, telemedicine and hospital operations. She continues to expand her skills in the health care arena by assisting clients with meeting their desired strategic clinical, financial, and operational goals. Ms. Jennings is co-founder and partner of VALIS, Inc, a comprehensive telemedicine consulting company. She has extensive experience and knowledge in conducting telemedicine needs assessments, comprehensive telemedicine programming and ongoing monitoring to evaluate financial and regulatory, risk and clinical impact of telemedicine services. Donna Jennings has more than thirty years of healthcare clinical and operational experience. She served as the Director of Quality Management & Service Excellence at a large consulting firm and supported the quality and risk management needs for more than 150 internally managed clients including acute care, critical access, LTC, home health, and skilled units/facilities. She served as a Chief Operating Officer for more than 6 years with direct oversight of all ancillary and support services departments.
