The FY 2012-2013 PERFORMANCE IMPROVEMENT AND QUALITY PROGRAMS have focused on:

Improving patient safety in relation to medication administration and patient falls

Following best practice standards and protocols for the care of patients with pneumonia, congestive heart failure, acute coronary syndrome, myocardial infarction, and stroke

DCH's MEDICATION ERROR RATE AGAIN MET GOAL and came in below the current state benchmark. All medication errors were category 0: No adverse consequences.

DCH WAS RECOGNIZED by the Iowa Donor Network for having a 100% appropriate requestor rate (Iowa Donor Network approaches families of eligible tissue donors) and a 100% referral compliance rate.

Dallas County Hospital was recognized at the TELLIGEN IOWA E-HEALTH SUMMIT for CPOE Best Practices, Patient Safety and Change Management. Jake Wendler, IT Manager, presented.

INFECTION CONTROL: Infection Control education initiatives have focused on hand washing, prevention of drug resistant infections, and adherence to isolation standards. Other successes include:

Dallas County Hospital was recognized with a HEALTHY IOWA AWARD for being among Iowa’s healthiest employers by the Wellness Council of Iowa. Dallas County Hospital was selected based on providing access to wellness, programming, financial commitment, measurability and sustainability.

Dallas County Hospital was named a TOP 100 CRITICAL ACCESS HOSPITAL (rural hospital) by iVantage Health Analytics, a national consulting firm. The award places DCH in the top 8% of Critical Access Hospitals across the county, with the selection criteria including clinical and operational performance, and financial and qualitative outcomes.

Departments with the highest patient satisfaction scores each quarter were recognized with a TRAVELING TROPHY that moves from department to department.

Dallas County Hospital completed an EMPLOYEE OPINION SURVEY this past fiscal year. The organization received a score of 4.20 out of 5.0 (compared to a score of 4.13 from three years ago), and had a 93% employee participation rate.

Dallas County Hospital recognized 18 EMPLOYEES celebrating 5, 10, 15, 25 and 35 year work anniversaries.

An EXPANDED HEART (Healthy Employees Are Really Terrific) Program provided FREE Health Assessments for all Employees, Lunch and Learns, as well as expanded Zumba Classes and healthy lunch options.

In FY 2012-13, Dallas County Hospital, in partnership with Dallas County EMS, provided First Aid booths or Water Stations at community events hosted throughout Dallas County. In addition, HOSPITAL VOLUNTEERS participated in many community events throughout our service area, including:

Marching in the Perry 4th of July and Adel Sweet Corn Festival parades.

Partnering with the Perry Police Department to provide bicycle rodeos and give away free bicycle helmets.

Providing car seat safety checks and new car seats to those who needed them.

During the holiday season, buying and donating toys for underprivileged children in Dallas County.

Supporting the Perry High School concession stand.

Providing health education at multiple Friday Fests.

Raising dollars for the Annual Perry Area Relay for Life event.

Hosting Senior, Teacher and Retired DCH Employee breakfasts throughout the year.

Participating in the Annual Safe & Healthy Halloween event, which provides a safe space and giveaways to over 1,500 individuals who participate each year.

IN TOTAL, DALLAS COUNTY HOSPITAL EMPLOYEES AND STAFF PROVIDED OVER 1,500 VOLUNTEER HOURS AND SERVED OVER 20,000 PEOPLE DURING THESE VOLUNTEERING TIMES.

BETTER CHOICES BETTER HEALTH

Dallas County Hospital and DMACC again partnered to host the HEALTH CAREERS ACADEMY AT DCH. Twenty-four area high school students participated in the program which entailed skills lab assignments, job shadowing in Med/Surg and the Emergency Department and observing surgeries. Upon graduation, the students were prepared to complete CNA certification testing.

Four SAFE SITTER CLASSES were held throughout Dallas County this past fiscal year. These Safe Sitter classes provide hands-on practice in lifesaving techniques designed to prepare babysitters to act in an emergency. Participants, ages 11-15, also received instruction on how a child’s age affects how to care for them, how to prevent problem behavior and how to run their own babysitting business.

BETTER CHOICES, BETTER HEALTHThe first BETTER CHOICES, BETTER HEALTH classes were held at Dallas County Hospital. Taught by Sally Swenson, Education Manager, six participants attended the weekly six-session interactive workshop that helps people with chronic health conditions learn how to take control of their health.

Dallas County Hospital PURCHASED BOTH THE ATRIUM BUILDING AND THE MEDICAL OFFICE BUILDING that houses the Mercy clinic. Ownership will help Dallas County Hospital financially and also allows DCH the flexibility to expand on campus.

The INTERFACE FOR LAB RESULTS was installed between Mercy Clinic - Perry and Dallas County Hospital. Results can now be posted to Mercy’s electronic health record system from DCH’s CPSI system.

Des Moines University 3rd and 4th year medical students and physician assistant students completed ROTATIONS AT DALLAS COUNTY HOSPITAL with Mercy Family Care Perry physicians. Throughout the year there were 6 students that worked with the Mercy physicians.

This past fiscal year, Dallas County Hospital saw growth in a number of areas:

Growth in the number of patients from specific geographical areas using Dallas County Hospital was seen this past year:

Dallas County Hospital saw an increase of 85% in the number of patients coming from Guthrie Center and 20% in the number of patients coming from Panora.In addition, a 12% increase in the number of Latino patients was also realized as compared to this time last year.

Dallas County Hospital became a participant of the MERCY ACCOUNTABLE CARE ORGANIZATION (ACO) LITE PROGRAM. The Mercy ACO Lite Program is intended to provide more access to affordable health care. Mercy employees and their dependents who are currently covered by Mercy’s health insurance, will now be able to receive their care at Dallas County Hospital and take part in the same in-network cost savings they would be entitled to if they received their care at Mercy Medical Center in Des Moines.

CLINICAL ROTATIONS were provided for eight students from Des Moines University, Mercy College, Iowa Central Community college and Iowa Central College for physical therapy, physical therapy assistants, radiology, nursing, and health information technology.

SENIOR HEALTH INSURANCE INFORMATION PROGRAM (SHIIP) had counselors at Dallas County Hospital on November 7, 2012. The counselors helped people compare Medicare Part D plans for 2013. Forty-two appointments were held.

service lines and programs offered.

Dallas County Hospital achieved its goal, realizing an OPERATION MARGIN OF 2.5%.

FACILITY AND EQUIPMENT ENHANCEMENTS continued throughout FY 2012-13 providing improved patient access and experience.

ENVIRONMENTAL SERVICES was relocated from second floor to the lower level to facilitate expansion of space to Genesis Development. Genesis leases the entire north wing of the second floor and provides all services, psychiatrist and counselors appointments from the second floor.

The INFORMATION KIOSK was moved to Patient Access to provide staff easier access to the workstation and patients with the ability to locate assistance and information more easily.

COMMUNITY PRESENCE OFFICES along with the Education Manager office and the Meditation Room were relocated in June 2013 to vacate space to facilitate the physical therapy department expansion.

Dallas County Hospital replaced the CATALYTIC CONVERTER on the hospital’s emergency generator. The catalytic converter will help reduce hazardous air pollutants.

SINGLESIGN-ON

SINGLE SIGN-ON was implemented. Single Sign-On (SSO) simplifies the Windows and EMR application log in and log out process by automating the username and password entry. The application improved caregivers’ efficiency and satisfaction and helped drive EMR adoption.

The Hospital’s DICTATION SYSTEM was also upgraded this past year. With the upgrade, voice recognition was introduced, which transcribes the physician’s text as they speak, providing increased efficiency.

The DALLAS COUNTY HOSPITAL FOUNDATION met goal this year, giving back over $110,000 in support to Dallas County Hospital. Specific fundraising avenues include:

Dallas County Hospital Foundation’s 9th Annual Golf Outing raised over $15,000, while the 4th Annual Bowl-A-Palooza event raised over $10,000. The Annual Employee Giving Campaign again raised over $25,000 WITH A PARTICIPATION RATE OF 87%.The Dallas County Hospital Foundation was awarded nearly $75,000 in grants.

Dollars raised were used to provide community focused programs, equipment and healthcare initiatives, including:

Seven scholarships totaling $5,500 to graduating Dallas County High School Seniors and College Seniors pursuing a career in healthcare

Scholarship to ADM High School in the amount of $10,000 to be used for the inception of the Project Lead the Way Biomedical Program

Free mammograms for uninsured and underinsured women

Pediatric immobilizer equipment for the Radiology Department Cataract surgery instruments
