Your acceptance into this academic summer program is a testament to your academic achievement and personal character, and we are delighted that you have expressed interest in joining us this summer. While on the island, faculty and staff members of the Wrigley Institute for Environmental Studies as well as residential counselors will supervise the program.    SPENDING MONEY:  Spending money is at the discretion of students and their parents. Students only will need (optional) money for any souvenirs. A list of gift shop items will be forwarded. Students are urged not to bring large amounts of cash with them.  PHONE & ELECTRONICS:  Students are able to use cell phones and listen to music during free time.  HEALTH SERVICES:  At Wrigley Marine Science Center (WMSC), highly trained Los Angeles County Paramedics are on call 24-hours a day in the community of Two Harbors and a small hospital is located in Avalon. If necessary, patient can be evacuated by helicopter from WMSC to a hospital on the mainland. Additionally, WMSC has a hyperbaric chamber, and all staff have basic medical and first-aid training. Parents will be notified if their student becomes ill or injured.  USC policy specifies that students under the age of 18 may not be treated without the written consent of their parents. In the event medical treatment is necessary the enclosed Parental Consent and Release Form and Medical History ad Consent Form will allow the students to receive medical care when and if needed.  Each student must have personal health insurance. USC requires that each student carry his medical insurance information with them at all times. We will carry the forms you complete with us at all times.

If your child does not currently have health insurance, please contact me so we may purchase insurance for the week.   To ensure medical care, it is essential that the completed Parental Consent and Release Form and Medical History and Consent Form be returned with completed application to the Sea Grant Office by June 17, 2016.

 CATALINA LETTER OF UNDERSTANDING

High School Marine Science Program Summer 2016    

Dear Catalina Participants and Parents:  

To ensure the most beneficial and enjoyable experience for all, we require that certain minimum standards of conduct be observed. Our experience has shown that a prior understanding of these standards by students and their parents will help contribute to a more effective learning experience.

 All participants are expected to follow the schedule provided to them at the beginning of the program. Attendance and participation at all classes and organized functions is required. Participants that are not willing to follow the schedule may be asked to leave the program at their parent’s expense.

 Participants are expected to clean up after themselves during the program. Participants are expected to rinse and put away wetsuits, snorkel gear, and kayaks after each outing.

 Participants are expected to be respectful & courteous to fellow students, staff and scholars on the island. Participants are not to touch or tamper with projects and personal belongings of others. Racial and/or derogatory remarks to others will not be tolerated and may result in being asked to leave the program at their parent’s expense.

 Participants are not to bring any portable game devices to campus. Students are not allowed to use cellular phones or other communication device during classes, meetings, or academic field trips. Use of cell phones or other electronics are allowed during free time. If a student doesn’t have a cell phone opportunities can be provided for them to use a phone. Violation of these rules will result in confiscation of the item until the end of the program.

 Participants are not allowed to leave the campus without permission from a staff member. Under no circumstances are students allowed to leave the campus alone. Participants are not allowed to snorkel, go swimming, or kayaking without a counselor or instructor.

 Alcoholic beverages and narcotics or other controlled substances are not permitted. Participants found to have bought, consumed, used, or possessed such items will be required to leave the program at their parent’s expense.

 Possession or use of firearms, ammunition, BB guns, air rifles, firecrackers, explosives, slingshots, knives, or other weapons of any description is prohibited. Participants found in violation of this rule will be required to leave the program and may be subject to prosecution.

 These regulations should not be considered all-inclusive and participants should conduct themselves in accordance with all laws. The success of the Catalina Program requires the cooperation of everyone involved.

 Additional rules regarding individual conduct may be given. Violation of any of these rules or the additional rules may result in disciplinary action up to and including termination from the Program. It is understood and agreed by the undersigned student and parent that any photos, video and audio taken by USC photographers or staff during the program may be used for promotional purposes.

 Please sign below to indicate your understanding and acceptance of these regulations. Return the signed copy of this letter to the Sea Grant Office with the forms packet and keep a copy for your reference.

RELEASE FROM LIABILITY (University of Southern California)

As the parent/guardian, I, the undersigned, acknowledge and agree that in consideration for permission to participate in the Activities (defined below), I, my spouse, assignees, heirs, guardians, and legal representatives hereby voluntarily indemnify, release from liability, agree to defend, and hold harmless the University of Southern California, The USC Wrigley Institute for Environmental Studies and any organization affiliated therewith, including all of their respective agents, employees, administrators, representatives, officers, trustees, students and assigns (collectively “USC”), for any accident, injury, illness, death, loss, theft, damage to person or property, or other consequences arising or resulting directly or indirectly from any activities which I may engage in, on, about or by access through any property owned, operated or managed by USC (whether permitted or not permitted by USC), including, without limitation, activities such as laboratory experiments where hazardous chemicals or other substances may be present, hiking, swimming , diving, snorkeling, scuba diving, wading , or boating and transportation to and from those activities (collectively, the “Activities”), including but not limited to claims arising from or related to USC’s negligence and/or products liability, including strict products liability. In the event that I am injured, I agree to assume any financial obligation, either through my health insurance, or through some other means, for any medical costs that I incur. USC assumes no responsibility for any medical expenses, injury or damage suffered by me in connection with my participation in the Activities. IT IS MY INTENTION BY SIGNING BELOW TO EXPRESSLY ASSUME ALL RISK OF PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE UPON MYSELF, TO THE EXCLUSION OF USC, AND TO EXEMPT AND RELEIVE USC FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH. I understand that none of the Activities are endorsed, sanctioned, guaranteed, supervised or monitored by USC. I acknowledge and affirm that I am not required to participate in any of the Activities as a condition to obtaining any academic degrees. I further acknowledge and affirm that I am not to be considered and employee of USC and that no benefits customarily afforded to employees of USC will be extended to me by virtue of my participation in the Activities. As an individual who actually IS employed by USC in a capacity unrelated to the Activities, I acknowledge that participating in the Activities is not a condition of my employment. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. Photo Consent for High School Marine Lab Experience  The  high  school  marine  lab  experience  is  a  collaboration  of  USC’s  Wrigley  Institute  for  Environmental  Studies,  USC  Sea  Grant  and  The  Center  for  Dark  Energy  Biosphere  Investigations.  

USC  Wrigley  Institute  for  the  Environment’s  mission  is  to  encourage  responsible  and  creative  decisions  in  society  by  providing  an  objective  source  of  marine  and  environmental  science  and  fostering  an  understanding  of  the  natural  world  among  people  of  all  ages.  

USC  Sea  Grant’s  primary  responsibility  is  to  contribute  to  solving  the  problems  of  the  Urban  Ocean,  while  recognizing  the  opportunities  of  coastal  commerce,  recreation  and  improving  the  quality  of  life  in  coastal  regions  such  as  Southern  California.    USC  Sea  Grant  does  this  by  funding  research  of  the  coastal  regional  and  disseminating  the  results  to  educators,  policy  makers,  and  the  public.  

The  Center  for  Dark  Energy  Biosphere  Investigations  is  a  National  Science  Foundation  funded  Science  and  Technology  Center  whose  mission  is  to  explore  life  beneath  the  seafloor  and  make  transformative  discoveries  that  advance  science,  benefit  society,  and  inspire  people  of  all  ages  and  origins.
