PHOTO ON FRONT COVER: Kathryn LaVoie, MA, CCC-SLP, uses iPad® therapy with a stroke patient in Acute Rehabilitation McLaren Northern Michigan is one of only two hospitals chosen to spearhead the Transition of Care (TOC) project. TOC identifies progress of the stroke patient in the hospital and at home, and identifies improvements needed in the hospital-to-home transition. Thanks to a donation by Howard and Letty Hulsman, 10 iPads® were purchased in support of this program.
The iPad® therapy program facilitates stroke recovery for those with aphasia (stroke-induced deficiencies in speaking, listening, reading and writing without affecting intelligence). This program improves aphasia symptoms, as well as tracks progress through scoring and graduated difficulty levels. Inpatient progress increased immediately when the iPad® was used in therapy.

Board Trustee and Foundation Donor since 1989.
For Ham Schirmer, supporting the McLaren Northern Michigan Foundation is personal. Ham, a
Board Trustee and former Chair, has been donating to the Foundation for almost 30 years. He has always been philanthropic and placed emphasis on supporting local charities.
“Not only is it important for the community to have a strong, healthy hospital, McLaren Northern Michigan is also the largest employer in the community which helps fuel the local economy,” he said.
Ham recalled the day Julie Jarema called and talked him into becoming a Friend of the Hospital.
“After that conversation, I increased my donation just because of her persuasive and knowledgeable presentation. From a selfish standpoint, it is nice to have someone from the Foundation be there to provide support and guidance,” he said.
Ham recently started making contributions to the Foundation through an IRA Charitable Rollover.
“It’s a real tax advantage to make contributions directly out of my IRA. I did it this year and I plan to continue to give this way in the future because of the tax advantage,” he said.
Ham said if you look at all the challenges healthcare is facing, hospitals must have a charitable arm to be viable. Because of this and his personal experience, he is committed to supporting the Foundation today and into the future.
“Being a Friend of the Hospital naturally brings you closer to the healthcare team. It really makes a difference when they know you when you come through the door, regardless of the reason you are there.”

>> Gifts will qualify for all or part of your Required Minimum Distribution.
>> Gifts are tax-free up to $100,000 per year.
>> Gifts must be outright gifts made directly from your IRA to a charitable organization.
>> Distributions may only be made from traditional IRA or Roth IRAs, not 401(k) or 403(b) plans.

For McLaren Northern Michigan patients, gifts from our donors provide for advanced equipment, new technology and programs crucial to patient care and recovery. Many donors are passionate and deeply committed to supporting a particular area of need. To ensure that resources are available to meet future needs, these donors have established a Donor Designated Fund.
Donor Designated FundsA Donor Designated Fund can be established with a minimum gift of $5,000. A Donor Designated Fund is often named by the donor, to honor or memorialize a loved one or to pay tribute to a health care provider. After the fund is established, a balance of $5,000 is required to remain in the fund.

Cancer is a word no one wants to hear. For some, understanding
the cancer journey with skilled guidance can be critical.
Melissa DeSimone, RN, BSN, OCN, CBCN, is one of two nurse navigators with the Karmanos Cancer Institute at McLaren Northern Michigan. These positions are funded by the Offield Family Foundation through the McLaren Northern Michigan Foundation. Melissa is specially trained and experienced in oncology care to assist patients through every step of cancer diagnosis, treatment and recovery. She offers support, encouragement, resources and advocacy that help patients make informed treatment decisions.
“My job is to help patients overcome barriers to care, connect them with resources, provide education about each step of the care continuum and expedite referrals and required testing whenever possible,” Melissa said.
Breast Cancer Nurse Navigators become involved with patients at the time of diagnosis. They contact primary care providers or appropriate ordering providers to ensure timely patient notification and referrals after diagnosis. They also screen to determine if patients are candidates for genetic testing.
“Once patients have been notified of their cancer diagnosis, I call them and complete an initial navigation assessment which screens for financial, insurance, transportation and emotional barriers. If a barrier is identified, I connect the patient with the resources he or she needs.”
For Heather Giammalva, having Melissa at her side has made all the difference. Heather was diagnosed with breast cancer in December of 2015.
NAVIGATING
In 2015, Melissa assisted 126 breast cancer patients. She has assisted 62 breast cancer
patients since the beginning of 2016.
Your Cancer Journey
“I had to be at the hospital at 5 a.m. for my mastectomy and couldn’t believe when I saw Melissa waiting for me. Her presence eased much of the anxiety I was feeling at the time,” Heather said.
Melissa has been with Heather every step of the way and has never missed an appointment.
“I recall a follow-up appointment I had with my Oncologist,” Heather said. “Literally minutes after the meeting, I received a call from Melissa asking if I had any questions about what was discussed. I appreciated this so much because I had questions and she answered them very sincerely.”
“I feel so blessed that God brought Melissa into my life,” Heather continued. “She is so helpful, kind, understanding and resourceful.”

Hospice of the StraitsCheboygan Hospice House
Through the support of generous donors the Cheboygan Hospice House was built to provide an alternative residence for community members at end-of-life.
The eight-bedroom Cheboygan Hospice House is a place tailored to meet the physical, emotional, and spiritual needs of each patient. In this home-like setting, patients are active participants in their own care, making choices about meals, leisure time activities and other decisions that create a true sense of autonomy.
Thanks to the continued donor support, the Cheboygan Hospice House provides a choice for end-of-life comfort and symptom management.
This year, we are raising $50,000 to support Hospice of the Straits. Please consider making a gift to help patients live their final days surrounded by their loved ones, while under the care of staff dedicated to providing the medical, emotional and spiritual care that is needed most.

McLaren Northern Michigan Foundation welcomes community fundraising initiatives to raise awareness and funding for programs and services that benefit northern Michigan residents. All proceeds raised by these events directly benefit our patients and their families.
The Mission of The Cheboygan Guild is to support the McLaren Northern Michigan Cheboygan Campus. The Guild recognizes the importance of the Cheboygan Campus and is dedicated to raising funds to support its services. In 2015, thanks to the generosity and fundraising efforts of the Cheboygan Guild, many patients were positively impacted by the following items purchased for Cheboygan Campus: • EMS radio recorder• Portable ventilator in the ED • Surgical instruments• Big wheel patient stretcher• Gas cards to help patients in need get to appointments
