Policy

The purpose of this policy is to ensure a process is in place to manage the production and distribution of identification cards and access control cards at the David Braley Health Science Centre (DBHSC) located at 100 Main Street West, Hamilton ON L8P 1H6

Procedure

1} The party requesting an Access and Identification Card shall;

• email Jennifer Atkinson at gPeki.<li@..rncm., st r.ctt to request an appointment to

obtain an ID/Access card.

2} The requesting party shall;

• produce their signed Photo ID & Card Access form (see Appendix A} and employee

number (if a McMaster Employee) to this scheduled appointment.

• produce government issued photo identification.

3} DBHSC Management staff shall;

• take a photograph of the party requesting an Access and Identification Card.

• Once a photograph is obtained, the ID Card Form will be brought to the person with

signing authority by the requesting party.

a. DFM -Education: Nancy Devlin

b. DFM -Research: Laura Cleghorn

c. DFM -Administration: Margaret Moscardini

d. DFM -Clinic/Maternity Centre: Jennifer Frid or Jill Berridge

e. DFM ALL DEPARTMENTS: Jennifer Gough

f. Public Health/City of Hamilton: Kait Martin or Michelle Baird

g. Education Services -FMPE: Erin McLeod

h. Education Services - CHSE: Angela Silla for Sheila Laffan

i. Education Services -MERIT: Anita Riddell

j. Education Services -MacCare: Carrie Grigg
