1. Document 004113 – Bid Form – Stipulated Sum (Single Prime Contractor) a. Remove and replace with the new form attached (2 pages).

2. Document 004323 – Alternates Form

a. Remove and replace with the new form attached (2 pages). This form supersedes all previous statements and references in the drawings to alternates.

a. Delete “Bidding Information” in its entirety. Replace with the following:

b. Add the following statement under “Bidding Information”: “Base bid is to include all installation and assembly indicated for the bus stop shelters. The owner is responsible for the cost of the shelter including shipping. (See Sheets A301 and A302.) The contractor is to order, receive, assemble, and perform all coordination.”

2. Sheet A 301

a. Add General Note 8: “8. The bus stop shelters will be owner furnished-contractor installed (OFCI). Contractor is to include in its bid to perform all coordination with supplier, ordering, receiving, storage, delivery to actual site, etc. including any activity required for final assembly of shelters.”

3. Sheet A302 a. Delete Key Note 2 and replace with the following:

“48” sidewalk continues from the shelter slab to the east and north, parallel to the existing curb and gutter until it meets continuous sidewalk looping the stadium. Sidewalk must slope to work with existing drainage patterns and must meet all ADA requirements for cross-slopes and accessibility.

4. Sheet A901

a. Add key note 6:

“Base bid for the tile will include everything except Break Room 104 (previously called “Conference 104” in the original drawings. Tile as depicted in 104 will be bid as an add alternate.” (See specifications).

1.2 BID FORM SUPPLEMENT

1.3 DESCRIPTION

A. The undersigned Bidder proposes the amount below be added to or deducted from the Base Bid if particular alternates are accepted by Owner. Amounts listed for each alternate include costs of related coordination, modification, or adjustment.

B. If the alternate does not affect the Contract Sum, the Bidder shall indicate "NO CHANGE."

C. The Bidder shall be responsible for determining from the Contract Documents the effects of each alternate on the Contract Time and the Contract Sum.

D. Owner reserves the right to accept or reject any alternate, in any order, and to award or amend the Contract accordingly within 60 days of the Notice of Award unless otherwise indicated in the Contract Documents.

E. Acceptance or non-acceptance of any alternates by the Owner shall have no effect on the Contract Time unless the "Schedule of Alternates" Article below provides a formatted space for the adjustment of the Contract Time.
