Meaningful Use Overview

We are at the forefront of the patient engagement era.

The American Recovery and Reinvestment Act of 2009

included the Health Information Technology for Economic

and Clinical Health (HITECH) Act, which gives the

Department of Health & Human Services (HHS) the authority

to establish programs to improve healthcare quality, safety,

and efficiency through the promotion of health IT, including

electronic health records and private and secure electronic

health information exchange (HIE). In order to accomplish

these goals, the Centers for Medicare & Medicaid Services

(CMS) and the Office of the National Coordinator for Health

Information Technology (ONC) established Meaningful Use

(MU), which uses a combination of financial incentives and

penalties to encourage healthcare providers to comply with

recommended standards. MU Stage 1 laid the foundation

for EHR technology, and in Stage 2, we are seeing more

of a push to focus on improving clinical outcomes.

MU Stage 2 has led to an increased focus on consumer

engagement, including requirements that providers meet

consumer uptake percentages.

InstantPHRTM was developed to enable healthcare providers

to deliver high quality patient care. The product’s ONC

2014 Edition Modular EHR Certification designates that

the software is capable of supporting healthcare providers

with Stage 1 and Stage 2 Meaningful Use objectives and

measures. Meaningful Use requires the utilization of ONC

Certified Health IT products.

The last year to begin participation in the Medicare EHR

Incentive Program is 2014 with the last payments disbursed

in 2016, and the last year to begin participation in the

Medicaid EHR Incentive Program is 2016 with the last

payments disbursed in 2021. With $27 billion available for

incentive payments from CMS, eligible professionals (EPs)

can receive up to $44,000 in Medicare payments and

$63,750 in Medicaid payments. Hospitals can potentially

qualify for incentive payments of $2 million or more.

InstantPHR’s Automated Numerator Recording

InstantPHRTM is a certified technology for automated

numerator recording. This feature enables EPs and hospitals

to accurately track percentage-based objectives by

providing the number of unique patients who have used the

patient portal to:

1. View, download and/or transmit their health information

2. Securely message their provider

3. View educational material

CMS Definitions of Unique Patient for EP

Reporting Purposes

If a patient is seen by an EP more than once during the

EHR reporting period, then for purposes of measurement,

that patient is only counted once in the denominator for

the measure. All the measures relying on the term ‘‘unique

patient’’ relate to what is contained in the patient’s medical

record. Not all of this information will need to be updated or

even be needed by the provider at every patient encounter.

This is especially true for patients whose encounter

frequency is such that they would see the same provider

multiple times in the same EHR reporting period.

Using InstantPHR’s Automated Numerator

Recording Reports Assists with Attestation

InstantPHR’s automated numerator site enables providers

to easily run reports to track unique patients that have

used the patient portal to engage in Core Meaningful Use

patient engagement objectives. The reporting site is linked

to specific EPs’ and hospitals’ instance of InstantPHRTM

upon purchase. Users are able choose a reporting period by

selecting desired dates and MU2 objective(s). For hospitals

that are attesting for inpatient and ambulatory incentives,

reports are run separately as the criteria has different

requirements. InstantPHR’s numerator recording, provides

a downloadable CSV file.

Note: EPs, EHs, and CAHs will be required to provide

the denominator when attesting for Meaningful Use.

This is not provided by InstantPHRTM as the platform will

only have access to data related to the patient portal,

not the full patient population.
