Mechanical Integrity Questionnaire:

Please look into the following questions and see the concerns you need to address for thesafety, fitness-for-service and ongoing mechanical integrity of the process equipment:

Are you in compliance with OSHA 1910.119?

Does your process equipment meet all the requirements of Process Hazard

Analysis?

Is your process equipment corroded beyond the nominal corrosion

allowance specified on U-1 form?
