Broad ObjectivesTo ensure a better quality of life for the people of the target area and to ensure that the community, including families infected/affected by HIV/AIDS, have access to quality basic medical and holistic care.

Specific Objectives• To provide affordable quality basic medical and holistic care to the sick• To provide holistic healing to HIV infected/affected families through provision of

medical, social, spiritual, emotional and psychological support.• To sensitize the community through creation of awareness on the cause, mode of

transmission, signs/symptoms and disease progress of HIV infection• To reduce stigma and discrimination through AIDS education and Education for Life

process workshops/seminars, thereby encouraging positive living.• To provide care, assistance and educational support to orphans and vulnerable children • To empower the community through active participation and ongoing education in the

care of children • To promote self sustainability of families to care for their children • To empower the communities in Kapsoya and Chepkoilel locations and enable the

households become independent and self reliant.

St Mary’s management team comprises of the business administrator, dispensary coordinator, social unit coordinator, and the director. The team meets monthly to discuss the day to day running of the centre and respond to issues at its level.

St Mary’s dispensary offers basic curative services at a low and affordable cost to the local community . The cost sharing from the dispensary helps to offset some of the running expenses.

LOCATION St. Mary’s Medical Centre Kapsoya is situated in Kapsoya Estate next to Kapsoya secondary school along Elgeyo Marakwet Road 5 km from Eldoret town – Eldoret East district of Uasin Gishu county -Rift Valley province – Kenya.GEOGRAPHICAL AREA/POPULATIONST. Mary’s covers 10 villages within Eldoret – Kapsoya, Munyaka, Kenya Service, Kambi Nyasi, Cyrus, Illula, Border farm, Kimumu, Cheburus, and Chepkoilel. Most people here are women and have no meaningful regular source of income. For these women to access medical care when they are ill or support for their children with the basic needs becomes extremely difficult as the majority are widowed or single. This also impacts on the HIV transmission. Most often some give in to casual sex in order to provide for their children, others engage in risky behavior that contribute towards HIV progression and transmission. Teenagers, especially girls need a lot of support. They are more vulnerable to early pregnancy and sexually transmitted infections. Many of the families that we care for live in a one roomed rented mud house. Children of all ages share the same sleeping space. This causes both boys and girls to leave the family early and this sometimes results in an increase in street children. General information on HIV/AIDS countrywide

ANC/PMTCT 822 mothers attended antenatal clinic 265 were attending for the first time 240 were counseled and tested for HIV 08 were positive for HIV and were referred to the District for ART prophylaxis. 02 couples were counseled and tested for HIV. They tested negative 407 Were given fansidar for the prevention of malaria in pregnancy. 335 were given T.Toxoid. 159 mosquito nets were supplied 271 had information on family planning.

SOCIAL DEPARTMENTThrough clear defined goals and objectives, activities aimed at empowering clients were well executed as support and networking with other agencies proved vital. Activities home visits,

school visits, psychosocial support, workshops, networking meetings, education support, Income generating activities ensured clients were adequately catered for.

Social Assistance: Through the social office we were able attend to 932 clients (697 female and 235 males). The issues they presented were dynamic in nature. The issues were education support, medical support, social support, psychosocial support, domestic issues, I.G.A support, and house rent support.

Psychosocial Support: In ensuring clients cope well in life and avoid stress, 61 clients were helped to overcome challenges that were bothering them. The social workers were able to offer psychosocial support to both OVCs and their guardians for the following issues domestic violence, child negligence, unemployment, lack of food, school fees, house rent problem and Indiscipline at school

Ngomongo to assess clients health status, social status, and economic status. Intervention that deemed necessary in assisting the clients after assessment of their needs was provided.

School Visit:. 64 visits were made in various schools by the social workers to identify various needs of the OVCs and take appropriate approach in solving them. This visits also entailed subsequent visits to some schools. The schools visited were Kapsoya primary, Munyaka primary, Central primary Chebarus primary, Illula primary, GK Magereza primary, Border primary, Wareng high school, Hill school, Kapsoya secondary, R.V.T.T.I and Ancilla vocational training.

Medical Support: We ensure the health of the clients we support is taken care thus enabling them to continue with their daily activities. Some clients were assisted to meet their medical fee; others were assisted in purchasing of drugs. A total number of 110 clients were supported as follows,102 clients were treated at our clinic and 8 clients were referred to others hospitals like Sabatia, MTRH and Kenyatta for specialized treatment.

Educational Support: The success of future generation in life highly depends in education excellence. OVCS support was as follows 45 OVCs in secondary was, 45 nursery school, 35 primary , 12 vocational training 9 special school . 40 OVCs were supported with uniform/learning materials and 27 with back to school shopping from different families were Visits to various schools

I.G.AWith the aim of reducing dependency, 74 clients were empowered with different small businesses for them to be self reliant. The businesses were: selling of charcoal, firewood, kiosk, small shops, renting of land and provision of seeds/fertilizers. To ensure proper utilization of support given 33 supervisory visits were conducted in various places

MEETINGS

• St Mary’s social workers collaborated with caregivers in reaching community. The care givers were empowered with skills on how they can carry out their work effectively. This was done in their meetings throughout the year

• 12 CHW’s meetings in which vital information i.e. health talks, behavior change, social support, drug adherence, strategies in reaching clients were passed to them. They also shared strengths, challenges and strategies for improvement. The CHW helped the social workers to have easy access to clients

• We had Guardians’ meeting for OVCs supported by St. Mary’s where the following issues were shared: parental responsibility, support groups, OVCs education support and feedback giving.

WORKSHOPS• One day youth workshops were organized during their holidays in August and December

at Pastoral Centre in Langas. The workshops focused on youth empowerment, discipline issues, relationships, performance at school and the importance of getting good pass marks.

• A five days T.B/ART Adherence workshop was held at Kapsoya Parish hall. Attendance was satisfactory 84% 21 participants (8 male and 13 female) of the expected participants attended out of 25. Out of 16% (4) of those who did not attend one was admitted in hospital and 3 were not feeling well.

• Quality Improvement Training was held in St Mary’s in September. It was organized by Ampath. It focused on ensuring quality services are rendered to OVCs by all networking organizations. It was attended by location chief, DCO, Child helpline, SOS, St. Mary’s, Morning Star, CHW’s, Kapsoya Police and Village elders.

Locational OVCs stakeholders meetings were held to address issues affecting children in the Kapsoya Location. The stakeholders were: St. Mary’s, SOS, Christ the Redeemer Catholic church, Queen of Angels street children’s home, special need children representative and, DCO Eldoret East. The issues discussed were: school dropout, child labor, networking, child abuse, sensitization in schools and children’s rights.

• Six contact teachers meetings were held ,three for Nursery teachers and three for primary teachers. The meetings focused on the progress of OVCs in different schools, networking, children rights, children self awareness, teachers challenges and their roles.

• St Mary’s partners with Uasin Gishu Children Forum. It is the only forum of its kind in Uasin Gishu County with three districts. Eldoret East district , Eldoret West and Wareng. The chair person and the assistant are from Moi university. The forum discusses issues children in all levels: Orphans and vulnerable children, street children, children in orphanages, the procedure for starting orphanages, children’s rights and children’s discipline. District Children Officers (DCOs)from the three districts attend the meetings

1. U.G forum organized a forum week of different activities: 1st day media, 2nd day cleaning of street in Eldoret town, 3rd day sports day 4th day was planting trees in Open Arms International orphanage/school and the 5th day was fundraising day.

2. Different staff and OVCs from St Mary’s participated in three days of the activities: media day Business administrator participated, sports day social worker with 15 OVCs participated in soccer game, sack race, spoon race and balloon blowing. They emerged number 4 out of 13 homes that participated. The tree planting day the director and one staff participated

CELEBRATIONS

• St Mary’s participated in the day of the African Child at Eldoret Special School where the ceremony. The theme was: “The Rights of Children with disabilities, the duty to respect, to promote, to protect and to fulfill”. The theme coincides with the venue where the students are mentally and physically challenged.

• St Mary’s celebrated World International Peace Day on 21st September with the theme of “Amani Yawezekana”. (Peace is possible)

• We also celebrated world’s AIDS day in December 7th with clients, OVCS, guardians,

care givers, Community Health workers and St Mary’s staff. The theme of the day was “Zero HIV, Zero AIDS and Zero Stigma”. More than 60 people attended

1. The OVCs presented poems and songs about clients not taking care of their health instead of taking nutritious food the take alcohol, quarrels in families and showing bad examples to children and the importance of trusting in God’s mercy and forgiveness.

• On 10th Dec we had an exchange visit between St Mary’s social workers and SOS social workers. Information pertaining caregivers and CHW’s was shared with an aim of giving quality services to the communities we serve

• On 28th February 4 of St. Mary’s staff, 4 Community Health Workers and 3 Caregivers went to Nyabondo on exchange visit to learn from one another on the HIV/AIDS service delivery.

OVCSs progress: Number of visits by Social workers were made in different schools as a way of monitoring and evaluating OVCs s progress academically and offer quality support. This was

realize by the cooperation from school management and some parents Good cooperation also ensured issues affecting OVCs were well addressed and approached.

Support groups: St Mary7’s through social unit was able to monitor 8 support groups in different areas. Several visits carried out to ensure client interests were at heart in the operation of these groups. Issues like domestic violence, drug adherence, I.G.A, children welfare and health matters were discussed among the group members.

ACHIEVEMENT • Most clients are observing our criteria for support and this have made it easy for St Mary’

to give support to genuine clients.• The number of clients receiving food support has reduced substantially which is a clear

indicator that more clients are becoming self reliant.• The clients brought the 20% of the harvest to St. Mary’s as agreed upon which is a clear

sign of growth to clients and ensures OVCs in such families will have enough food.• A latrine was built for a poor family who were using a nearby bush thus preventing the

spread of communicable diseases

SUCCESS STORYSt Mary’s core goal is that of empowering clients, make them take charge of their lives and be self reliant

• To achieve this goal a good number of clients were assisted with renting of land for those who did not have seeds and fertilizers for agricultural reasons.

• This will ensure food security in their homes and consequently be empowered enough to be able to buy their own fertilizer, seeds and lease land for the next cultivating season.

Some clients have already harvested, leased land, bought seeds and fertilizer. They are now waiting to prepare the land for the planting time.

Capacity building

Two social workers attended trainings: 1 Resource mobilization training and HTC course 2 HTC

CHALLENGES• Reduction on funding ( Constituency funding ended in May 2012 and AMREF ended in

June 12 )• A number of OVCs who cleared class 8 could not continue with education due to limited

funds which led to some to despair in life.• Some guardians are drinking alcohol and as a result they have abandoned their duties to

support OVCs thus forcing some OVCs to collect metals in order to feed.• Due to various stressors of life, most guardians have found it difficult to cope with their

children and as result beating them and denying them basics in life.
